WI Podiatrist Part of Team That Saved Diabetic's Leg
A 10-month process saved Joe Kasuboski's right leg from amputation thanks to care Joe received last year at the newly-launched Agnesian Wound Care services in Fond du Lac. His most recent battle against diabetes was centered on the fight to save his right leg. It all began in June 2014 with the appearance of a sore on his little toe. Joe's primary care physician, Richard Gauthier, MD, a family medicine physician who saw Joe through the entire process, sent him to Jonathan Stroebel, DPM, a podiatrist (foot and ankle specialist) with Agnesian Wound Care.

"Gangrene had set into the wound by his toes," his wife Barb says. "Things weren't looking good."As a result, all the toes on Joe's right foot were surgically removed by Dr. Stroebel. Joe received healing hyperbaric oxygen treatments five days a week for 10 weeks. In addition to the hyperbaric oxygen treatments, a wound vac (suction device) was attached to Joe's open sore during his months of treatment.

Wearable Technology Can Help Monitor Injuries: FL Podiatrist
Robert Duggan, DPM notes that, while wearable tech started with the very healthy — like his son, a swimmer at Stanford University whose entire workout is tracked by wearable devices — the opportunity it presents for treating the ill is exciting. In his office, wearable technology is a common component of his patients’ treatment plans. The Orlando-based foot and ankle surgeon finds step counters particularly useful in pressuring his patient athletes to cut back on their activity levels when dealing with an injury.

“A lot of them do have the wearable technology, because a lot of them are tracking their performance and their level of intensity of exercise,” Duggan says. “So I’m actually able to dial down their level of activity so that we decrease the number of foot strikes on the ground, or we decrease the intensity to allow the body to kind of catch up with the injury.”

Medicare Extends Deadline to Appeal Two Penalties
The Centers for Medicare & Medicaid Services (CMS) has extended the deadlines for physicians and group practices facing two different Medicare penalties in 2016 to request an informal review if they believe the government made a mistake. The penalties, which whittle down reimbursement, are levied under Medicare's Physician Quality Reporting System (PQRS) and the Value Based Payment Modifier (VBM) program. The original deadline for an informal review of both penalties had been November 9. Now it is November 23.
In PQRS, Medicare penalizes physicians for unsatisfactory reporting of clinical quality data. The penalty in 2016, based on performance in 2014, will lower fee-for-service payments by 2%. Physicians, medical groups, and accountable care organizations can learn if they are due for a pay cut by obtaining a PQRS feedback report for 2014. The CMS website explains how to obtain the report. Requests for an informal review can only be made online through the Quality Reporting Communication Support Page of CMS. CMS promises a decision, which is final, within 90 days.

Five Hiring Tips for Medical Practice Managers- Part 3
• Team effort. When it comes to hiring right, input is the key. Managers should never try to go it alone, says Bo Snyder, a practice management consultant in Kalamazoo, MI. "Peer interviewing is very effective," he says. "Get your best team players and your most patient service-focused employees to interview all of your candidates. They will help you guard the gates." The act of soliciting their input, says Snyder, has the added benefit of boosting morale. "It's an honor and a perk to be asked to sit on that team," he says.

Query: ICD-10 Diabetic Codes and Nail Care
Previously, I used ICD-9 250.60 (Type II diabetes with neurological manifestations) and ICD-9 357.2 (polyneuropathy in diabetes) to qualify my diabetic patients for nail care. With ICD-10, it appears that E11.42 (Type 2 diabetes mellitus with diabetic polyneuropathy) is the most specific, and therefore the only required code. My software converts ICD-9 250.60 to E11.40 which appears to be an unspecified code. Any thoughts as to whether both E11.42 and E11.40 need to be used, or whether E11.42 is sufficient?

Response: E11.42 is to be used because it's the most specific code; I would not use E11.40 since it's the "unspecified" code.

It sounds like the representative was 'over the top', but rules are rules and possibly she may be putting her position on the line if it was revealed that an attendee actually did not attend. Hindsight is 20/20, but the resident, realizing the presentation was over, should have asked if it would be all right to order a meal on the company's tab.

The entire incident shows a lack of respect by the representative of the company involved. There was no reason to act like this and embarrass the resident. I would never allow that representative to call on me or use their products until a complete and public apology was made by the individual and the company. We do have some control... our pocketbooks.

Dr. Gorey - the point of my story was to reveal the behavior of the compliance officer. The "whole room" was not aware of the incident as there was a lot going on. The resident himself may have offered to pay for the meal, I was not privy to the entire dialogue between the two, but the maitre'd stated the meal was on the house, and the four podiatrists seated at my table all offered to pick up the tab. But, thanks for your "vote" of confidence in your colleagues.

American physicians should regard the happenings in Great Britain as junior doctors ballot for a strike to oppose unsafe working conditions in hospitals. Long hours, reduced overtime, a paltry pay raise has the junior doctor's (residents) Union ready to vote. They have the full support of the BMA. They're not talking about a one-day strike which would go unnoticed. The advantage they have is their strike is against the government which salaries NHS doctors.
Who is the one-day strike in America directed against? The only effective response American physicians can offer is non-participation in all insurance plans, and not just for a day. An insurance company that has no participating physicians is out of business. PPACA reform began and ended with insurers. The fallout from PPACA on the landscape of healthcare wasn't accidental.
EMR and MU are the government's 'common core' response to physician-administered healthcare. They are administrative burdens which only help bean counters, while leaving doctors to drown in a sea of confusion. Like the good lemmings we are, we march dutifully to our own demise.
