PA Podiatrist Participates in YMCA Diabetes Awareness Event
 
To recognize November as National Diabetes Awareness Month and to honor the memory of their former chaplain Tom Rivera, the Spring Valley YMCA held a diabetes awareness event of their own. The  event also included information about foot care and eye screenings which are two things recommended for individuals with diabetes.

Doctor of podiatric medicine Renata Milman represented the Comprehensive Foot and Ankle Center of Collegeville. Milman said it’s recommended for those with diabetes to check their feet every day to make sure they didn’t step on anything. She said diabetics have nerve damage and sometimes won’t feel it when there’s a cut or scrape, which can lead to infection.

Lowell Weil Jr: Our goal is not to create 28 robotic people. Each person is an individual who does it his or her own way with successful outcomes. What we have tried to do as an institution is to develop care pathways. Within each pathway, there are a few options. The benefits of having a care pathway is that we can take that pathway to insurance companies and referring doctors and show them what we will provide to patients. We can show them what percentage of patients will progress to a cure at this level and how much it will cost. These care pathways are discussed and agreed upon at our provider meetings.

Joe Piscopo’s career soared with Saturday Night Live, but stalled in Hollywood (Johnny Dangerously), impressed on Broadway (Grease), and took some bizarre turns (Celebrity Wife Swap).

He was happily performing at casinos and clubs around the country, he said, when his pal Dr. Rock Positano, the foot doctor to the stars, suggested he consider getting into radio. Piscopo, who studied broadcasting in college, began visiting a midtown studio to hone his skills when Curtis Sliwa abruptly left AM 970 last winter.

IN THE COURTS
MA Nurses Union Objects to Mandatory Flu Shots
 
A nurses' union is suing a Boston hospital to preserve its employees' ability to decline influenza vaccination after the hospital made it a condition of employment. While Massachusetts law lets employees opt out of workplace flu shot initiatives, Brigham & Women's Hospital has made it mandatory due to an employee vaccination rate that is lower than that of other regional hospitals.
 
The Massachusetts Nurses Association filed a civil lawsuit in state court on Sept. 22 to block the implementation of Brigham & Women's mandatory vaccination policy. The union represents approximately 23,000 nurses and other providers statewide, including 3,200 of the hospital's employees.

Diagnostic non-surgical suggestions:
 
1. Try a period of non-weight-bearing and see if the pain resolves. Have the patient use crutches, a knee scooter, or similar.
2. Try lidocaine injections, distal to proximal to pinpoint the area/s of pain.
3. Have an MRI taken. This is essential.
4. Try a custom orthotic with sesamoid relief.
5. Shoe modification:  Sesamoid pocket in shoe, and tall toe-box.
6. Prescribe a physical therapy referral, including daily home self therapy, and NSAIDs.
7. Order a neurological consult: R/O nerve entrapment, RSDS/CRPS, etc..

I disagree with my friend Dr. Kesselman on his response about having a patient split the wearing of a night splint between legs. Really? Have we all just bowed down to the "insurance gods" and given in? I am sure if Paul were to come down with plantar fasciitis, he would sleep with two splints.  
 
Maybe the next time I get my eyes checked and they find only a problem with one eye, the doctor will prescribe a monocle. Maybe if I have a patient with a wound on both legs, I could share the VAC one day left, one day right. Please, we must never lose sight of the fact that we must do what is best for the patient. If the insurance company only wants to pay for one night splint, I leave it up to the patient to complain to the insurance gods. I tell the patient that this is the cost of the second splint and have them get it. The motivated patient wants to get better quickly, and the physician who treats patients to a speedy recovery will have a significant advantage today and in the future.
