PRACTICE MANAGEMENT TIP OF THE DAY
Six Steps to Ensure Your Future Practice Success

5. Measure and share your quality scores. This could be a key differentiator, especially if your scores are consistently in the top quintile or decile. It’s peace of mind for a patient knowing their doctor is at the top.

CODINGLINE CORNER
Query: Challenges Billing CPT 11057

How are we supposed to bill CPT 11057 to Medicare to get paid? For the typical patient, we currently use the ICD 10: E11.42, E11.51, L84, R26.2. We bill this as its own claim. We put our podiatrist as the referring physician. We do not put any modifiers and we previously use Q8 (when appropriate), but it was denied. What does the proper 1500 form look like for CPT 11057?

Codingline Member

Response: Without seeing the actual EOB with denial reasons, it's tough to say why your claims are being denied.

Some thoughts:
-Make sure you use the -59 modifier when billing combo with nail care, if needed.
-I would only list ICD-10 codes that are needed. Other codes may trigger certain billing criteria. You listed diabetes with vascular and neurological findings but the Q8 would be used in error for a neurological risk code.
-Adding the abnormal gait code R26.2 doesn’t help get your claim paid as it is not a risk factor, and hence that could get your claim kicked out.
-Computers are programmed very strictly as what to pay on such that anything that may be “off” with your claim could get it kicked out.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Edwin S. Hart, III, DPM

Sadly, those who do not know history in medicine and surgery are doomed to repeat it. The gray hair represents, most of the time, a historical frame work reference in matters. Often, I see a lack of interest in that perspective in training of students. They do not realize what they might be missing. So some of these ideas will be lost until a future generation rediscovers them. As my wise father said, "don't get rid of your narrow ties, they eventually come back in style." Just like MIS, I guess.

Edwin S. Hart, III, DPM, Bethlehem, PA

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Martin M Pressman, DPM

I have also been summarily rejected from these surveys because of my age (75) and years of practice (47). These “surveys” are not gathering information for academic purposes. They are underwritten by pharma or industry and aimed at practitioners to identify practice patterns for the purpose of selling them on the industry’s own product. They know we seasoned  practitioners generally don’t fall for those games. I take that as a compliment.

RESPONSES/COMMENTS (NEWS STORIES) - PART 1A
From: Robert D Teitelbaum, DPM

Dr. Kuizinas' observations may have validity for many patients but I find that in my Florida practice, going barefoot is problematic. Many people move down from the Northeast where they commonly have wall to wall carpeting and fall in love with some tiles that are very common here. It's warm, airy, and sunny, and they go barefoot quite often. In a short time, they're in my office complaining of heel pain. They are middle aged, and it's America, so many of them are overweight. I have to straighten them out when it comes to this subclinical stress they're putting themselves under. I find that half of the cure for heel pain syndrome is 'between their ears'. I straighten them out by telling them, "You get away with everything when you're younger, and nothing when you're older."

To be sure, there are many Floridians who have gone barefoot their entire lives and get away with it, possibly from their intrinsic muscles getting a great workout from early in life. But I don't see many of them in my office for that condition.

Robert D Teitelbaum, DPM, Naples, FL

RESPONSES/COMMENTS (NEWS STORIES) - PART 1B
From: Ivar E. Roth, DPM, MPH

Going barefoot at home is a bad idea. Sooner or later you’re going to suffer an injury due to that recommendation. I guess the older you get, the wiser. While going barefoot might sound like a great idea, experience has shown me that it is not. Additionally, the more often you support your feet, the better your feet will function with the extra benefit of less injuries and accidents.
