According to Eveline Tan, DPM, "the feet can be an indicator of overall general health. A lot of times, it's something nobody looks at, especially when you go to your primary care physician. A lot of things go undetected. The foot is probably the most unrecognized part of our body."

"If you look at a callus, it's something as simple as an overgrowth of skin that actually tells you a lot - that there's an inbalance in your foot, whether it be a biomechanical imbalance or a structural imbalance. A lot of times, people say that their foot is dry, but it's actually not dry skin; it's an underlying skin disease. They look at their nails and don't realize it could be a sign of an underlying pulmonary disease," says Dr. Tan.

SC Podiatrist Discusses Scope of Practice Gains
According to Michael McCann, DPM, "The field of podiatry has made tremendous leaps and bounds over the past decade in the medical field. Podiatry is already becoming integrated into multiple level-one trauma health systems to assist in treating their patients. During my training, it wasn’t out of the ordinary to fixate an ankle fracture, surgically correct a flatfoot, and surgically close a diabetic foot wound/ulcer with a skin flap."

"Last year, the South Carolina state legislature passed legislation to increase the scope of practice for podiatry, which now mirrors the states bordering South Carolina. The new scope of practice includes the ability to perform surgical intervention on bone and soft tissue structures in the rearfoot, including fixation of ankle arthritis through ankle arthroscopy or arthrodesis (fusion), fixation of ankle fractures, repair of Achilles tendon ruptures, and other rearfoot conditions," says Dr. McCann.

NY, MN Podiatrists Speak at the PFA Virtual Symposium
More than a dozen national experts took the stage for the first-ever virtual delivery of the Annual Pedorthic Footcare Association (PFA) Symposium. The virtual event was the PFA’s pandemic response to its annual traditional in-person clinical and technical educational conference for professionals specializing in the management and treatment of conditions of the foot, ankle, and lower extremities requiring fitting, fabricating, and adjustment of devices. Paul Kesselman, DPM discussed 21st century biomechanics and the newer technologies that are disrupting the clinical footwear industry. Three-dimensional printing is allowing clinicians to create customized orthoses in their offices within hours, rather than having to create a mold and send it off to await a laboratory-fabricated product in weeks. “This is not science fiction,” he said. “This is today’s reality.”

Paul Langer, DPM discussed how new scanning and 3D printing technology will become increasingly accessible to clinicians, retailers, and consumers. This access will allow for better precision in fitting shoes and the ability to produce customized footwear on demand on-site. The health benefits of comfort and the ability to address pathology were also discussed. “And comfort,” he explained, “is the opposite end of the pain spectrum. Comfort is very important and can even affect self-esteem.”

Medicare Part B Premiums To Rise 2.7%, Deductibles to $203 in 2021
The Centers for Medicare & Medicaid Services has announced Medicare Part B premiums for 2021, and the base premium increases just 2.7% from $144.60 a month to $148.50 a month. That $3.90 monthly increase compares to a big $9.10 monthly increase last year, after a $1.50 monthly increase the year before.
The annual deductible for all Medicare Part B beneficiaries is $203 in 2021, an increase of just $5 from the annual deductible of $198 in 2020. What kept the increases in the Part B premiums and the deductible in check this year? As part of the short-term budget bill in October, Congress capped the increases. Yet Medicare spending is expected to grow this year as people seek care they may have delayed due to COVID-19, CMS says.

What are the guidelines for telemedicine for existing and new patients?

Response: Well, even though the question is fairly simple, the answer is not.
Each insurer has its own policy regarding the codes that are covered and the software that is acceptable – and to some extent can even vary by CPT code. So, while this is probably not what you want to hear, you need to go to the website for each major insurer serving your geographic area and see what their policy is. Using an Excel spreadsheet is a good way to track it. You can either have one tab per Insurance or separate the chart by insurance.

We use Square in our office. This system is user-friendly with options for connectivity to a smart phone, I-pad, or separate desktop touch pad. It provides instant notification of transactions via emails, and their website provides in-depth accounting. I cannot attest to their fees but I’m sure they are competitive.

For about 8 months now, I have been involved as a professional associate with a company that has been proactive in preparing for the time when doctors' offices can take and get paid for both COVID-19 antibody and antigen testing. This company has worked with institutions and countries to supply tests for years and years. Their clients are fully prepared, licensed, and certified to offer these tests, seamlessly now that the FDA granted an emergency use authorization (EUA) in October. My place in the organization is to mentor and service those who qualify to test but are not familiar about the how, what, when, and why of this matter to get on board this important and profitable train.
Currently, the only test DPMs, MDs, and DOs (depending on state law) can perform is the 15 minute rapid antibody finger stick test (no antigen test yet but coming). It is taking about two weeks to become certified and to organize practice(s) to see if it can maintain FDA- and CLIA-compliance. This is working well in urban areas due to the high demand for antibody testing (not reimbursable) but my suggestion is to get involved on this level early, especially if you are having difficulty re-opening during COVID-19 because it will place you on the tarmac for the antigen EUA that will happen so that many more will take advantage of it.
