IN Podiatrist's Condition Upgraded From Critical to Fair Condition
 
Dr. Thomas Freeman, II
 
Podiatrist Dr. Thomas E. Freeman, II's condition is improving, according to hospital staff. The Muncie doctor has been in critical condition at Miami Valley Hospital in Dayton, Ohio, since a motorcycle accident Oct. 17. Currently, his condition is classified as fair, hospital personnel confirmed Friday afternoon, meaning his vital signs are stable and he is conscious. The accident occurred in Wayne County. Freeman was traveling southbound on U.S. 35 North and Round Barn Road north in Richmond. His 2008 Kawasaki motorcycle collided with a 1999 Chevrolet Cavalier. Freeman is the podiatrist at Preferred Footcare in Muncie.

NC Podiatrist Joins Rocky Mountain Practice
 
Rocky Mount-born podiatrist Dr. Jennifer Purvis has returned to the city and joined a practice that includes her father. She will offer surgery through Nash UNC Health Care and Vidant Edgecombe Hospital. Purvis earned a doctorate in podiatric medicine from Temple University in Philadelphia. She also completed a podiatric medical residency at Western Pennsylvania Hospital in Pittsburgh and a foot and ankle surgical residency at Joint Township District Memorial Hospital in St. Marys, OH.

Purvis comes to the center after eight years of private practice in Alexandria, VA. Her father, Dr. Carl G. Purvis, is the center’s founder. She is board certified in foot surgery by the American Board of Podiatric Surgery and a fellow of the American College of Foot and Ankle Surgeons.

Dr. James Yakel will begin taking appointments at the Sedgwick County Health Center (SCHC) on Wednesday, November 12. He graduated from the Ohio College of Podiatric Medicine in Cleveland, Ohio, and completed his residency at Mt. Sinai Medical Center, also in Cleveland. He has been in private practice for 18 years. He is past president of the American Academy of Podiatric Sports Medicine, and lectures locally and nationally on sports medicine topics. He is a Fellow of the American Academy of Podiatric Sports Medicine, Diplomate of the American Board of Podiatric Surgery, and a Fellow of the American Professional Wound Care Association.

FL Podiatrist Indicted in Murder of Wife
 
A grand jury has indicted podiatrist Dr. Adam Frasch in the death of his French model wife. Eight months ago, Samira Frasch was found dead floating in the swimming pool at her upscale Tallahassee home. Her husband, Dr. Adam Frasch, has denied guilt for the incident since February. He testified before the grand jury that indicted him Friday. Frasch turned himself in to detectives at the Leon County jail after the indictment. He was handcuffed and led away. His attorney, John Leace, has always contended Frasch is innocent.

OR Woman Sues Nail Salon After Pedicure Leads to Infection and Amputation
 
A woman who claims a pedicure at a North Portland nail salon led to an infection and then to the amputation of one of her toes and eventually her right leg has filed a $280,000 lawsuit against the salon’s owners. Madhu Kumar got a pedicure at Nails Trap, according to her lawsuit filed last week in Multnomah County Circuit Court. “When the pedicurist was attempting to use a sharp instrument to shave calluses off both of her feet, the pedicurist negligently lacerated the media distal tip of (Kumar’s) right hallux,” states the suit.

Although Kumar's suit doesn't state if she had an existing medical condition, health experts say those at greatest risk of contracting serious infections from nail salons often have problems such as diabetes, poor circulation, or numb feet. "People with neuropathy may not feel the pain of the infection, leaving them unaware of the problem until it's too late," wrote Dr. Neil Scheffler, a Maryland podiatrist in a newsletter for the Amputee Coalition.

QUERIES (CLINICAL)
Query: Arthritic Sesamoids and Hallux Limitus
 
A 45 year old male developed hallux limitus. Podiatrist #1 performed a Valenti procedure in 2011. Pain persisted. Podiatrist #2 lengthened the EHL and “cleaned up” the joint. Pain persisted. Podiatrist #2 then determined that most of the residual pain was from osteoarthritis of the hallux IPJ, and performed a fusion. After 9 months, the patient presented to my office with severe pain. The hallux IPJ demonstrated no clinical motion, but radiographically was never fused. I asked the patient to determine if his pain was from the hallux IPJ or the MTPJ. Answer: The IPJ. I fused the IPJ, which is now asymptomatic.
 
He then stated that he still had severe pain. The pain was at the MTPJ. I asked him to determine if the vast majority of pain from from the primary joint or from the sesamoids. Answer: The primary joint. Since the IPJ was already fused, I was reluctant to fuse the MTPJ, and therefore performed an arthrodiastasis. He now has good ROM at the MTPJ (measured when he is NWB) with no crepitus, but severe pain from the sesamoids. I cannot be certain that there is no pain at the primary joint as well.
 
Dilemma: I am concerned about fusing the great toe joint, since the IPJ is also fused. I am concerned about excising both sesamoids, which may lead to a transfer of WB pain to the second metatarsal. Nevertheless, I can think of no feasible alternative to doing both. Suggestions?
