MO Podiatrist Provides Children's Shoe-Buying Advice

Dr. Scott Friedersdorf is a podiatrist with the Hannibal clinic. He says it's important that you have your children's feet measured regularly when they're young. Friedersdorf says kids need shoes with a good fit, which means snug-fitting heels and a thumb's width of room in the front of the toes.

Friedersdorf suggests snug-fitting sneakers or athletic-type shoes with a mid-sole, with flexible toes. He also suggests leather or canvas shoes. He says kids don't need to wear shoes until after they start walking - normally between 9 months and the first year of age.

NC Podiatrist Easily Re-Elected to City Council
 
Four fresh faces will soon appear on the Gastonia City Council, after a municipal election that will yield the most dramatic change to local leadership in at least six years. Incumbents Keith Tidwell and Dale Burkett were voted out, while Dr. Dave Kirlin and Brenda Craig earned convincing victories, according to unofficial results.

Ward 2 Councilman Dave Kirlin ran away with the victory in being re-elected to his sixth consecutive term. Kirlin, 57, a local podiatrist, was first elected in 1999 and will become the city’s longest continually serving existing councilman. He picked up 51 percent of the vote, easily beating Tom Thornburg and Daniel Ware, who garnered 24 and 23 percent, respectively.

Gout Linked to Increased Risk for Diabetes, Renal Disease

Two studies of veterans presented on opening day here at the American College of Rheumatology 2011 Annual Meeting show that uncontrolled serum uric acid levels are associated with a nearly 20% increased risk of developing diabetes and a more than 40% increased risk of developing renal disease. Bhavik J. Pandya, MD, from Takeda Pharmaceuticals International, in Deerfield, Illinois, and colleagues studied 1923 patients in the Veterans Integrated Services Network 16 database.

All were adult males with gout, and all had two serum uric acid measurements taken between January 1, 2002 and January 1, 2011. The study population was 52% white, average age was 62.9 years, average body mass index (BMI) was 30.6 kg/m², and average follow-up time was 80 months. Major co-morbidities at baseline included hypertension (93%), hyperlipidemia (64%), cardiovascular diseases (30%), and smoking (8%).

Failure to Report Test Results Increases Malpractice Risk

Failure to communicate diagnostic test results increasingly puts physicians at greater risk for medical malpractice claims, researchers report in the November issue of the Journal of the American College of Radiology (JACR). Taking advantage of available "semi-automated critical test result management systems" could improve patient safety, shorten hospital stays, and reduce risk by providing legal documentation.

"When reportable test results arise, healthcare organizations need clear policies that define the responsibility of reporting and referring providers to ensure patient follow-up," the authors wrote in the report that JACR cited as CME Activity of the Month. "Recently developed technology may help healthcare providers reduce the incidence of missed notifications."

Reconstructive Foot and Ankle Surgery Fellowships (Two Available)

Each of these two fellowships is a one-year experience during which the Fellow will develop a rational approach to revisional and reconstructive foot and ankle surgery.  The Fellow will function as a clinical instructor and participate in the teaching of Podiatric medical students and residents, and have ample opportunity to be involved in clinical and basic research projects.  The fellowship will provide the candidate, further expertise in Charcot reconstruction, soft tissue coverage, trauma and deformity correction.  The fellow is expected to complete two clinical or basic research projects during the year.

Duration:  1 year (July 1, 2012 - June 30, 2013) Stipend: $44,100 Application DeadlineNovember 21, 2011  Interviews:10/1/2011 – 12/12/2011 

Requirements:  Completion of 3 year residency; a commitment to a part-time/full-time academic career in Podiatric Medicine and Surgery. Query: Lateral Knee Pain, Right Side

I have a 55 year old overweight male patient with lateral knee pain of his right knee who has existing orthotics. What modification would one make on his next set of orthotics to try to alleviate this knee pain? 

A biomechanical exam displays medial subluxation of the talo-navicular joint upon standing. The right foot exhibits 1 degree forefoot varus and 3 degrees rearfoot varus. The left foot exhibits 2 degree forefoot valgus and 3 degree rearfoot varus. Any ideas would be appreciated.

Query: Brewer's Yeast for Hyperhydrosis

Do any of my holistic colleagues have any information or experience using Brewer's Yeast for hyperhydrosis? I have had a couple of patients recently ask me about it, and I am unfamiliar with the connection and can find nothing online about using the yeast to slow sweating.

The PT tendon plays a great role in de-accelerating the internal  rotation of the tibia, and for that reason, I tend to repair, or at least fixate to the navicular, even after a fusion. Also check the knee, as the increase in rotation allows plantarflexion and abduction of the talus (center of the body). The eversion of the calcaneus can strain the medial knee. Thus, sometimes you need to "Remember the Knee" when dealing with this pathology.

RE: Best Running Shoe for Orthotics (Joe Borden, DPM)

I think the question is not, "What is the best running shoe for orthotics?", but rather, "What is the best running shoe for my patient?" 
 
Some patients will need more control and a more firm shoe. Some will do better in a shoe that has a flare to the sole (either medial or lateral). Some patients need shock absorption... you get the point.
 
Podiatrists prescribing foot orthotics owe it to their patients to become familiar with the footgear that they will be worn in. You don't need to be an expert on every shoe in the market, but you must be able to recognize the characteristics of the shoes your patients are wearing. If upon examination, the shoe is not suitable, you can send the patient to the store with a list of desirable characteristics. The staff in any good running shoe store should be able to help your patients choose the best shoe to fit your recommendations.

After seeing this posting regarding the billing of higher deductibles and co-payments, I wanted to let everyone know what we are doing in our practice with significant positive results. Four years ago, in an effort to share resources, several of us got together and started a company called IVS. This company specializes in the verification of insurance coverage and benefits levels.

Over the past several years, by using IVS, we have seen our accounts receivables reduced from many thousands of dollars to a fraction of that number. The need for patient billing has also decreased dramatically. We generally have all the information regarding eligibility, deductibles, and co-payment amounts, as well as coverage for durable medical equipment by the initial patient visit. This allows our front desk staff to collect the appropriate amount from patients before they are put into the treatment room. It has been the single most productive and cost-effective practice management tool we have utilized in over 20 years of practice.

RE: Early Diagnosis of Bunions Can Prevent Foot Surgery (Jeff Root)

As early as 1993, articles like Picciano’s in J Orthop Sports Phys Ther: "Reliability of open and closed kinetic chain subtalar joint neutral positions and navicular drop test" have proven Root's STJ neutral to be unscientific, poorly reproducible, and that Root’s “neither pronated nor supinated” or his 1:2 ratio have minimal place in examining for or developing treatment options for orthotics.

Mr. Root: How many articles have you published on biomechanics? How many have you or are you currently funding or researching? Other than your name, what makes you an authority in biomechanics whose opinion should be recognized?

Biomechanical authorities such as Payne, Kirby, Nigg, Mertz, Dananberg, Glaser, McPoil, Shavelson, and others have declared Root’s STJ Neutral Theory...
