AT THE COLLEGES |
AACPM Grants Western University Access to AACPMAS Application Service
Western University of Health Sciences College of Podiatric Medicine has taken an important step in expanding its application pool. The college, which will welcome its first students in fall 2009, will participate in the American Association of Colleges of Podiatric Medicine’s central application service known as AACPMAS.
|
Dr. Lawrence Harkless |
“I believe this is significant in our ability to attract the best and the brightest of students,” said CPM Dean Lawrence Harkless, DPM. “This will give each student who applies to the colleges of podiatric medicine an opportunity to evaluate us and decide if they want to train in an interprofessional environment. I expect the number of applications to double as this allows each student applicant access to Western’s CPM.”

PRACTICE MANAGEMENT TIP OF THE DAY |
Why We Don't Hear Others - Part 1
If you want to listen so you really hear what others say, make sure that you're not a(n):
- Identifier. If you refer everything you hear to your experience, you probably didn’t really hear what was said.
- Comparer. When you get side-tracked accessing the messenger, you’re sure to miss the message.
- Derailer. Changing the subject too quickly tells others you’re not interested in anything they have to say.
- Sparrer. You hear what’s said but quickly belittle it or discount it. That puts you in the same class as the derailer.
- Placater. Agreeing with everything you hear just to be nice or to avoid conflict does not mean you’re a good listener.

QUERIES (CLINICAL) |
Query: Plantar Fascial Ruptures
I had a healthy 33 y/o male with no history of heel pain or plantar fasciitis present with bilateral plantar fascial ruptures. He states he was walking in the mall in his flip-flops when he heard a loud "pop" and felt immediate sharp, severe pain in both feet. The only medication the patient is taking is Doryx, which is a doxycycline prescription. The patient has been on the medication for 3 weeks. I've heard of Levaquin causing tendon ruptures, but I can't find anything about tetracycline causing tendon weakness or ruptures. Has anyone else had any similar patients?

RESPONSES / COMMENTS (CLINICAL) |
RE: Painful Callused Heels (Shari Kaminsky DPM)
From: Multiple Respondents
The condition described is known as hyperkeratosis traumatica marginalis calcis. It is thought to be caused by the wearing of loose-fitting shoes such as sandals. Friction between the loose fitting shoe and the heels produces the hyperkeratinization. It is also seen in patients who walk barefoot or in obese individuals who wear open back footwear. Patients should be instructed to only wear closed back shoes with firm counters and not go barefoot.
Epidermophyton floccosum (Epidermophyton inguinale) may affect the groin with a pruritic dermatitis and will produce plantar hyperkeratinization. Culture the skin and if the organism is isolated, try an oral antifungal. Topical antifungals do not penetrate the keratotic skin and are therefore not effective.
Mike Boxer, DPM, Woodmere, NY, mcbdpm@aol.com
Make sure that this is not Haxthausen’s disease, AKA keratoderma climactericum. This can initially affect only the heel from what I’ve read in dermatology texts.
Nicholas Taweel, DPM, PT, Philadelphia, PA, dpmpt@hotmail.com
I've had very good results with daily vinegar--water soaks and Vitamin A & D ointment h.s. under occlusion along with prn debridement.

RESPONSES / COMMENTS (NON-CLINICAL) |
RE: Non-Payment of Associate
From: Steven H. Goldstein, DPM
I was disgusted to hear of such nonsense by members of our own profession. This happens all too often. This is why legal contracts need to be drawn. In today's society, someone's handshake means nothing. I would file a complaint against the doctor with the state board of medical examiners and file a claim in small claims court. I would be very vocal to others of the type of person who would do such a thing. It is a shameful act of a despicable person.
