Podiatrists and Students Participate in Fall Festival for Special Olympics
 
Recently, American Academy of Podiatric Sports Medicine (AAPSM) Past President Jay Spector, DPM and Rebecca Van Dyke, DPM, along with students from Temple University School of Podiatric Medicine (TUSPM) volunteers, participated in the Fall Festival for Special Olympics.

UTHSCSA  Announces New Residency Director
 
Graham Hamilton, DPM has been named Program Director of the Podiatric Medicine and Surgery Residency at the University of Texas Health Science Center at San Antonio (UTHSCSA). Dr. Hamilton has more than 20 years of experience as a teacher of students, residents, and peers. He is currently a trauma faculty member of the Arbeitsgemeinschaft für Osteosynthesesfragen North America (AONA) where he co-chairs the AONA course for DPMs.

The UTHSCSA residency program recently celebrated 50 years and has graduated more than 230 residents, including some of the profession's notable researchers, deans, and association/society presidents.

IL Podiatrist Featured at Dine with a Doc Event
 
Ashley Smith, DPM, a surgical podiatrist at the Lawrence County Memorial Hospital, spoke to seniors during the monthly Dine With a Doc program Friday. The program is held on the first Friday of each month at the Lawrence County Senior Center and features a different doctor with a topic related to senior health and needs.

Dr. Smith spoke about foot and ankle needs and what it is like when you walk in to see her for an appointment. She also discussed options for patients to help them take care of the foot pain they may be having.

Podiatrists Discuss Ski Boot and Skate Modifications at AAPSM Meeting
 
American Academy of Podiatric Sports Medicine (AAPSM) President Mike Donato, DPM and Dalia Krakowski, DPM discussed modifications for skates.and ski boots at the recent AAPSM) meeting in Washington DC. Dr. Krakowski has a background as a ski racer and coach. She said that although boots are tight to allow maximum power transfer to skis, there are options for stretching and padding hot spots. She prefers not to have any external posts or heel lifts.

Dr. Donato comes from a hockey family. His brother played on the Boston Bruins and shares his passion for hockey. Mike spoke about adjustments that can be made to skate blades and inside the skates, as well as the developments with heat-molded skates. The workshops complemented the didactic portion of the seminar and provided interaction with the faculty and attendees.

8 Tips for Medical Malpractice Defense
 
Although no physician can manage all the possible risks that lead to malpractice lawsuits, best practices include making sure you have the best insurance policy, working on patient-communication strategies, and following the right steps if a lawsuit is filed. Check out these medical malpractice best practices.
 
8. Prepare for deposition or pre-trial statement - The next most important step of malpractice defense after a lawsuit has been filed is to prepare thoroughly for the deposition or pre-trial statement, Peter Kolbert, senior vice president for claims and litigation management for Healthcare Risk Advisors said.
 
Although malpractice suits may hang like a dark specter in the back of a physician's mind, Kolbert said that the number of adverse outcomes is larger than the number of claims on average, which are a fraction of 1% of patient encounters. "Most of the time physicians are doing a good job, but there's always room for improvement," he said.

Bret, I could not agree with you more. The pious faith in our field has been our downfall. Healthcare has pivoted but the physicians have not. Our leaders are failing us. APMA wants our money and wants our voices to help make change, but is it really listening? As I said in my previous post, enough is enough. We as physicians need to make a stand against this bullying. Medicare Advantage plans just received another increase in pay from Medicare. This money does not trickle down to physicians. It lines the pockets of the executives of these plans. When LA Care has huge signage on a skyscraper in Los Angeles, next to US Bank, what does that say to you. How are they spending money that should be earmarked for us?
 
I challenge our leaders to set up a meeting with all the other medical associations and create a new path for us. The Writers Guild in Los Angeles also received a pay raise in addition to the other industry that Bret mentioned. Let’s get one too. And it better be in the double digits!

Many of us have been saying this for years and it starts and ends with the absurd laws which prohibit doctors from really unionizing in a manner in which they can collectively bargain and strike. Under the current structure, these powerful tools are kept at a distance and all we can do is gripe and act independently but not with a collective force that a real union can effect. Without the ability to collectively bargain and possibly strike, we are in effect powerless to stop this continuous downward spiral. Nurses, residents, and others in healthcare can unionize and have walked out on the picket line. Physicians must find a way to fight back and get that right back! This downward spiral cannot be allowed to continue.
 
So yes the UPS driver and others with some seniority can make almost as much as a family practitioner and actually eclipse them with overtime. They additionally have no worries about liability, who may or may not show up for work, and all the other responsibilities a small business owner has to saddle.
 
Even employed physicians have to work harder, longer hours to keep up with absurd EHR requirements to see more patients in less time and are effectively being paid less per patient as they are on an hourly or RVU cycle. We are now in parallel with our colleagues in the former Soviet Union, where the joke was that bus drivers made more than doctors. Well, that joke is now at our doorsteps and it is no longer funny!

The doctor treated a fracture of the proximal phalanx of the right hallux. He billed codes 99203, 28510, and L3100. The complaint by the patient about the fees charged and it being a "surgery code" are something we see frequently. Patients frequently don't understand that some things they don't consider surgery such as partial nail avulsion under anesthesia or cauterization of pyogenic granulomas are coded as surgery, and that is usually very easy to explain to the patient.  
 
Dr. King said he agreed with the codes the doctor billed, however code 28510 is NOT the code for a hallux fracture. Closed reduction of a "great toe" fracture without manipulation is CPT code 28490. Code 28510 is for Closed, other toe (2-5) without manipulation.  
 
Regarding billing an initial office visit in addition to the hallux fracture code 28510, I have always billed for an initial visit in addition to the fracture care. I don't think I have ever had an insurance company reject it as being included in the fracture code. I frequently bill for subsequent office visits rather than a fracture care code depending on how often I think the patient will be returning and the amount of time I spend with the patient. A patient with a complex fracture or an anxious patient may need to come in more often. Deciding how to bill it is usually up to the doctor.
