The Key to Wearing High Heels is Moderation: NY Podiatrist
Our feet have been experiencing some serious high heel hibernation due to the global pandemic. Gone are the days of night clubs and social events that require a powerful pair of heels. Don’t worry, heels aren’t going anywhere, but comfort has definitely shifted to the forefront considering the times. The key is about moderation and alternating heel heights depending on your activity.
|
|
Dr. Suzanne Levine |
For standing up to eight hours, your ideal heel height should stay at two inches; Increase the height and you should limit time spent on your feet. “It’s like having your cake, but not making it a daily activity,” says Suzanne Levine, DPM. “It doesn't have to be done every day or in excess, but there is nothing wrong with wearing a fabulous pair of heels!”

JAPMA Articles Highlight Increased Amputation Risk During COVID Pandemic
An article published in the Journal of the American Podiatric Medical Association (JAPMA), online ahead of print, by Dominick Casciato, DPM and colleagues at Grant Medical Center in Columbus, Ohio compared the patient load on the inpatient foot and ankle service before and during the pandemic. A total of 270 patients with diabetes were included. The authors found that the odds of undergoing any level amputation during the pandemic was 10.8x higher than before the pandemic. In the patients that required amputation during the pandemic, it was more likely to be a major amputation. The authors attributed this to disruption in the care and delays in patients seeking treatment.
In a rare JAPMA editorial that accompanied the article, editors Lee C. Rogers, DPM, Robert J. Snyder, DPM, and Warren S. Joseph, DPM stated that diabetes-related amputations are "a pandemic within a pandemic" and that "it is clear that delays in care for diabetic foot ulcers have catastrophic effects. Increased awareness, proper prioritization, and efficiencies of care in lower risk settings are the first steps in mitigating this parallel pandemic of diabetes-related amputations."

The indictment alleges that Lynn performed routine foot care and then billed Medicare for unperformed surgical procedures. The indictment alleges that Medicare was billed in excess of $1.1 million and paid out in excess of $850,000. Lynn has pleaded not guilty.

| PRACTICE MANAGEMENT TIP OF THE DAY | |
Reducing Medical Practice Embezzlement Risk with Cash Controls - Part 1
Front-desk embezzlement schemes are not uncommon and they are typically the result of loose protocols and lack of oversight. Reduce your financial risk by tightening cash controls using these proven practices.
1. Confirm that all encounter tickets for the day have been posted and "closed."
-
At the end of each day, generate a "missing ticket" report from the practice management system and verify that all encounters have had charges and payments posted to them. This indicates the ticket has been closed.
-
If your practice uses paper, ensure each ticket has a number.
-
If your practice is paperless, the practice management system will generate and store an electronic tracking number.
-
Tickets for patients who cancelled or did not show up for their appointment should be closed with a reason code for "cancelled" or "no-show."

Query: Point of Sale Systems
We are a low volume office looking for a cost-effective POS system. I am interested to know what is used by my colleagues. Clover, Merchant Services, Square have been suggested. What is the best option?

| RESPONSES/COMMENTS (CLINICAL) | |
From: Charles Morelli, DPM
Depending on the foot type and how extensive the amputation is (whether or not it may include a partial ray resection as well), these procedures will often cause the patient to lose medial stability with potential subsequent collapse of the medial column, again depending on the patient and their foot type. In most cases, I will insert a subtalar implant or stent (depending on preference) and have found that these work well in stabilizing the foot in these particular cases, and will also augment off-loading with a custom orthotic.
Charles Morelli, DPM, Mamaroneck, NY

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A | |
From: Jack Ressler, DPM
Sending charts to Ciox or the other companies is a big inconvenience, no matter the price you ask. Dr. Moglia, you should reconsider raising your price. At $10/chart, Ciox might agree to pay that, causing you and your staff a great deal of work, especially if a larger chart number pull is requested. When I get a call from Ciox to negotiate a cheaper price, it seems that $20/chart is their limit, if indeed they pay at all. Obviously as mentioned many times, do not send charts until payment is received. If you are still doing paper charts, I would not start any copying or chart preparation until you see the check.
On a final note, in my opinion, do not let their staff in your office to copy your charts. They do not belong in your office. I have heard stories about some of these companies getting a little nosey in doctors’ offices. Obviously, you are not hiding anything but you never know what they can hear from your staff or a patient passing in the hallway. Do not look at this as an opportunity to make some extra money. I do not think it is worth your time.
Jack Ressler, DPM, Delray Beach, FL

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B | |
From: David Gurvis, DPM
I currently charge Ciox $30 per chart. Some charts have a minimal amount of visits, some have many. I win some and lose some. I do not send the requested material until the check is in my hands.
