Podiatrists Encourage Asian Women to Seek Regular Care for Diabetes

"Asian Americans have an elevated risk of being diagnosed with diabetes," said APMA President Laura J. Pickard, DPM. "Combine that with a tendency to overlook self-care, and Asian women are a population at very high risk for serious complications from diabetes." Asian women tend to put their families first, meaning they often overlook their own health needs. There are also common misconceptions about diabetes and its treatment options within the Asian community.

"Diabetes can cause serious complications in the feet, including non-healing wounds, infection, and even amputation," said Priya Parthasarathy, DPM, chair of the APMA Communications Committee. "That's why it's so important to educate this population about how they can manage their diabetes and protect their feet, which will keep them at the heart of their families."

Going Barefoot at Home is Often Best: MA Podiatrist
 
Going totally shoe-free at home is often the best bet. "Being barefoot is great for overall foot health and helps to increase foot muscle strength, tissue tolerance, and joint mobility," says Alissa Kuizinas, DPM, a board-certified podiatrist in Concord, Massachusetts. Try taking a gradual approach if walking around barefoot is uncomfortable because you're not used to it. "Go for 15 to 30 minutes a day and work up from there," she says.

If you have foot pain, flat feet, or ankle arthritis, consider having a pair of supportive slippers or shoes that stay in the house. Shoes should have flat, flexible soles, a wide toe box, and low or minimal cushioning, Dr. Kuizinas says. "If you require a slipper, I would recommend a stiffer sole, possibly a forefoot rocker or toe spring, and some cushioning," she adds.

TUSPM Student Faces Stress from the Imposter Syndrome
 
Chelsea Nicole Walker learned early on that she had to look after herself before entering the Temple University School of Podiatric Medicine (TUSPM). The 29-year-old student is pursuing a specialization in podiatry and a graduate certificate in hematology. Although she is not yet a doctor, even the process of becoming one has been stressful.

“I feel the impostor syndrome every single day. I don't see faces that look like me. And I had a really difficult time getting my foot in the door,” said Walker, who is of Black, Choctaw, and Báxoje ancestry. “... I came from a community where a lot of people don't go to school, don't go to college. It's very rare for people to get a master's and even rarer to pursue a doctorate.”

 Study Shows Efficacy Of ColActive Plus Powder In Resolving Non-Healing Wounds

Doctor Groups Turn up Heat on Congress to Halt 2023 Pay Cuts
 
The Centers for Medicare & Medicaid Services (CMS) finalized the cut in its Physician Fee Schedule rule released Tuesday. Groups afterward implored lawmakers to act in an end-of-the-year spending package that must be passed by Dec. 16. CMS was required to install the cut under budget neutrality rules, leaving groups to turn to lawmakers for help. Congress has previously stepped in to stave off Medicare cuts, and there is a bipartisan bill in the House to do just that. 
 
Groups are also hoping to get a delay to a 4% decrease to payments as part of the PAYGO law, which mandates cuts if federal spending reaches a certain threshold. Physicians say the cuts come amid lingering financial challenges. Physician groups are hoping for a long-term fix that won’t require them going to Congress to get rid of any cuts. “This annual brinksmanship with Medicare payments is not sustainable and does not support better care for patients,” said Jerry Penso, MD, president and CEO of AMGA, in a statement.

PRACTICE MANAGEMENT TIP OF THE DAY
 Six Steps to Ensure Your Future Practice Success
 
Your success or lack thereof will rest upon patient loyalty. Here are six steps to take:
 
3. Outservice your competition by finding your glue. Why do your patients stick with you? If you don’t know, ask them. You must nurture loyalty.
  • Listen with your ears and your eyes. Give the patient in front of you your undivided attention. Make that patient feel special. Start on time, and run on time. If you are running behind – even just a couple of minutes – apologize. And always thank your patient for entrusting their care in you.
  • Make it personal. Add reminders in your notes – e.g., patient enjoys playing golf at Lakeside, daughter is a freshman at LSU – to affirm your patient is more than just a chart number.
  • Expect your staff to do the same. They are an extension of you and can make or break you easily. Employees who do not connect with your patients are not the employees you want.
  • Use online registration and other tools to eliminate the dreaded ‘arrive 30 minutes early for your appointment’.
  • Avoid banker’s hours. Like it or not, you will need to offer hours more convenient for patients.

Query: Denials For Post-operative X-Rays
 
Which ICD-10 code should one use for post-operative x-rays after a bunionectomy, a hammertoe repair, or amputation? UnitedHealth Care denied payment for post-op x-rays. This hasn’t happened before.

Response: The carriers are starting to deny based on improper ICD-10-CM coding. So, it is no surprise when I audit records only to find the same diagnoses used to do surgery no longer exist for the patient after surgery, as the problem has been resolved by the surgery.
 
You have to ask, why am I taking that x-ray after surgery and what is the best diagnosis? In the scenarios you stated, the hammertoe diagnosis, hallux valgus diagnosis, or the infected toe/gangrenous toe diagnosis are not the reasons for taking the post-operative study. There are “Z” codes to use in the post-operative patient. I typically recommend for the visits:
Z47.1 Aftercare following joint replacement surgery - use additional code to identify the joint (Z96.6-)
Z47.2 Encounter for removal of internal fixation device
Z51.89 Encounter for other specified aftercare
Z48.89 Encounter for other specified surgical aftercare
Z47.89 Encounter for other orthopedic aftercare
 
My "go to" diagnosis is Z47.89. Make sure you document an order as to why the x-rays are medically necessary and make sure you document the foot left and/or right, each view named (AP, lateral, oblique, etc.), and then your interpretation.

I would like to comment in response to the numerous doctors who wrote commentary as to a case of malpractice in Virginia. The thrust of the responses focused upon the testifying expert, in our case podiatric surgeons, who provide their expertise as opinions in medical malpractice cases. It appears that the primary complaint from the authors of those commentaries was that experts lie in their testimony in depositions and in court. As a former board-certified podiatrist as well as a current attorney, I must respectfully disagree with the term "liar."
 
Experts, whether retained for the plaintiff or for the defense, are asked to provide their opinions based upon the facts presented to them. As we all realize, opinions vary. In my 25 years of being an attorney representing plaintiffs in malpractice cases (for clarification for those who do not know me, I was advised by our own insurance mutual company to represent plaintiffs and...
