PODIATRISTS IN THE NEWS
MD Podiatrist Provides Advice on Bromhidrosis

When your feet sweat or get wet, bacteria build up and cause odor, says Kelly Geoghan, DPM, a podiatrist at Mercy Medical Center in Baltimore. The best solution? Keep your tootsies as moisture-free as possible. Dry them well after every shower and be sure to wipe in between your toes. If sweat’s your problem, spray your feet with a deodorant or antiperspirant, Dr. Geoghan suggests. An excess of foot sweat can even lead the dry skin between your toes to ball up into another icky foot issue: toe jam.

Dr. Kelly Geoghan

Avoid wearing socks and heavy shoes outside in the heat if you can help it. If these methods don’t get rid of the stench, says Dr. Geoghan, a prescription-strength roll-on made for body odor can probably kick it.

PODIATRISTS AND FOOTWEAR
FL Podiatrist Discusses Plantar Fasciitis and Footwear

What can you do to alleviate the pain associated with plantar fasciitis? Footwear can be a starter. According to Dr. Grace Torres-Hodges, a Pensacola, FL-based podiatrist, while there is not a specific shoe type that addresses plantar fasciitis, some important things to look for involve rearfoot support including deep heel cups, firm heel counters, flared heels, good cushioning, and arch support, whether it’s a sneaker, loafer, or oxford. “Bottom line,” she said, “that knocks out your flip-flops and sandals.”

Dr. Grace Torres-Hodges

Dr. Torres-Hodges also recommends shopping for shoes later in the day when feet tend to swell. And while these may seem like footwear basics, she recommends patients not rely on the size of their last pair of shoes, since manufacturer sizing differs. “Get both feet measured, and buy for the larger foot,” she emphasized. Next, try on shoes with socks or the hose you expect to wear, as well as any orthotic devices that have been prescribed, since these can alter the fit and comfort of a shoe.

PODIATRISTS IN THE COMMUNITY
CA Podiatrist Recognized for 36 Years Service to Native American Health Group

Fifty years ago, local Native Americans could get medical care close to home, thanks to a consortium that came to be known as Riverside-San Bernardino County Indian Health, Inc. A golden anniversary celebration to recognize the 50 years of healthcare services was recently held at the Morongo Band of Mission Indians Community Center.

Specialty services have been added as needed. Podiatrist Lyle Parker, DPM was recognized for his 36 years of service. Dr. Parker also presented a historic slide show before he and 18 other employees were recognized for more than 25 years of service.

IN THE COURTS
Lawsuit if You Don't Disclose Your Lack of Expertise?

How much information must you reveal to patients about your experience with specific procedures, your track record, and outcomes? More courts are now saying that it's possible that lack of disclosure violates informed consent rules. In an Iowa heart surgery case, a surgeon had never performed a procedure he recommended. The patient experienced severe complications and sued for both negligence and lack of informed consent.

In a 5-4 decision, the Iowa Supreme Court in June 2018 ruled that the patient had the right to know about the surgeon's lack of experience. Although a doctor's experience generally hasn't been considered part of the informed consent process, the law is evolving. The issue is often decided on a case-by-case basis. Still, more disclosure of experience, and even outcomes, may be required.

QUERIES (NON-CLINICAL)
Query: Injections for Fibromas

I am considering adding Vitrase (hyaluronidase) and/or Xiaflex (collagenase) to my injection treatment offered for fibromas. I would appreciate any comment on dosage and cost, or information on a supplier for both.

RESPONSES/COMMENTS (BILLING AND CODING)
From: Michael E. Graham, DPM

There has been so much controversy on the use of sinus tarsi implants, and one of the greatest is how to bill for the procedure. Orthopedic surgeons routinely bill the procedure as a subtalar fusion and have no issue with it. I agree that this is wrong. There is an argument to make on the use of CPT 28585 as it describes the procedure being performed, BUT the AMA and APMA have stated that this is not an accepted code. My company has stopped advocating that code once that clarification was made, even though we did have support from some insurance carriers who agreed it was a correct code.

I have spent considerable time, money, and effort to get a real code for this procedure and the best we can get right now is 0335T. That is the only “approved” code to bill for the insertion of a sinus tarsi implant – period! We need to pull together as a profession to publish more research and to keep pushing the insurance carriers to cover this procedure. Don’t sit on the sidelines and complain or commit insurance fraud. Patients will pay for the procedure - they pay billions for plastic surgery and dental care. My opinion is that foot alignment is more important to the body than a slightly crooked tooth. We need your help to fight this battle.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
From: Allen Jacobs, DPM

It is not appropriate to willfully misrepresent facts to a patient. Tell the patient the truth. You are paid less for multiple procedures performed concurrently. You are not willing to accept the reduced payments. The patient should be made to understand that you utilize the same degree of care, time, cost, and expertise for each procedure.

If you are found negligent in the performance of a second or third procedure, I do not believe the jury award or settlement by the carrier is reduced 25 or 50 or 75 percent. Just tell the patient the truth. Misrepresentation is always unethical regardless of any well-intentioned reasons for doing so.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
From: Dieter Fellner, DPM

I empathize deeply with Dr. McCormick and his frustrations. The problem is, however, much more pervasive. Primary certification, with this exam is frankly absurd. I have over 30 years’ experience. I know my 'stuff' and I keep up-to-date. In 2016, I sat for their exams, as required. The system is flawed. I want to challenge those sitting comfortably on their lifetime certificate to submit to the same process (without cheating).

Let me be quite plain: the computer simulation is ridiculous. The marking of answers is inherently biased against the examinee. The hapless test taker is required to search frantically for those elusive buzz words that resonate with the marking. Doctors: that has nothing to do with clinical and surgical competency. The computer systems also tend to malfunction.

I have practiced surgery for decades. I have never been sued. I know of exam-taking superstars who get sued repeatedly. So, what exactly does the Board Certification provide? It is not clearly a yardstick for surgical competence. In England, where I first practiced, we have a system of periodic peer review. Assigned podiatric surgeons will visit the facility every two years for an on-site evaluation of surgical practice. This is a supportive function, as much as it is an audit. After 4 years of school and 3 years of residency and endless exams, when will America have confidence to allow their doctors to be doctors?

RESPONSES/COMMENTS (EMR)
From: Ron Werter, DPM, James Koon, DPM

I’m a very satisfied Sammy user and highly recommend it to everyone. It’s efficient, does what’s needed, and saves time - which is money.

Ron Werter, DPM, NY, NY

When I was in private practice two-and-a-half short years ago, we were using MacPractice MD. Our entire office was Mac-based and I loved it. It was easy to navigate, had all of the features that made it compliant with Meaningful Use/MIPS, and always worked. Their support was always available and competent and not terribly expensive (about $4,000/year for the two practitioners and electronic billing). Because everything was Mac-based, everything always worked. We used an on-site MacMini for our server and backed up off-campus every night. Being that I've been away from them for almost three years, I am only guessing when I say they likely have cloud-based server capability.

The program itself was very customizable, a feature I loved because...
