PODIATRISTS IN THE NEWS
Self-Inspection and Monitoring is Key for Diabetics: IA Podiatrist

Diabetes affects many systems of our body with complications to the eyes, the kidneys, and the heart, but when you look at those, there are so many more complications to the feet," says Valerie Tallerico, DPM, of UnityPoint Clinic Podiatry. "It is important from a diabetic point of view that these patients closely monitor and inspect their feet on a daily basis. If they can't see their feet, they need to use a device to allow them to see the bottom of their feet. "

Dr. Valerie Tallerico

"Diabetes affects nerves, muscles, and skin. Patients can lose sensation. Patients can develop vascular diseases and ulcers which can lead to infections and amputations," says Dr. Tallerico.

Dr. Michael Paris, a podiatrist practicing in Hanover, was recently appointed to the Pennsylvania State Board of Podiatry by Governor Tom Corbett. Dr. Paris, became the only podiatrist in the York/Adams region, and one of just four podiatrists statewide to be appointed to the board.

Dr. Michael Paris

INTERNATIONAL PODIATRISTS IN THE COMMUNITY
Aussie Podiatrist Helps Rocky Horror Show Actor Get Back on His Feet

Kevin Whittle paid a tough price in the name of show business – in the form of five inches. But now the Gold Coast-raised actor can confidently strut around in high heels, an achievement that still gets a lot of laughs when he tells people the story. Whittle was performing in the Rocky Horror Tribute Show when he started to lose feeling in his feet because of the show-stopping heels he was wearing. Turning to Southport structural podiatrist Matt McGuire of Hip to Toe, he was able to get back in heels again and gain function in his feet.

McGuire is the only specialist of his kind in Queensland, addressing the positioning of the foot rather than using orthotics and insoles to correct problems. He said his technique involved completely realigning the foot through adjustments. “One of the key problems with heels is they do put a lot more stress on the muscles and tissues,” he said. “It’s not everyday you get to put a man back in high heels, and being able to help him get back to work was a real sense of achievement.”

PODIATRISTS AND INTEGRATIVE MEDICINE
PA Podiatrist Discusses Home Remedies

Dr. Maasi Smith heard so much about home remedies that he decided to write a book called Feet Naturally: Nature's Guide to Healthy Feet. Maasi says if it helps, use it. While these remedies are not a substitute for seeing a physician, Dr. Maasi says that some home remedies actual work. "Baking soda can be used to soak the feet and relax the feet. It can also be used as a paste applied to the feet as a deodorizer."

Dr. Maasi Smith

"The best thing about ginger is that it an excellent anti-inflammatory and can be used to treat all types of arthritis and degenerative disease. It can be eaten with foods or used as a tea," says Maasi.

IN THE COURTS
Supreme Court to Hear Obamacare Subsidy Case

The U.S. Supreme Court announced Friday it will hear King v. Burwell, a case that many say could gut the nation's Patient Protection and Affordable Care Act, leaving millions of Americans uninsured. The question in King v. Burwell is whether the language of the act allows consumers to receive premium tax credits in states that have not established their own exchanges and instead are relying on HealthCare.gov. Many states have not created their own exchanges.

One part of the law says the tax credits are available only to Americans who enrolled “through an exchange established by the state.” But the Obama administration argues that the law's clear intention was to offer subsidies and expand coverage to Americans in every state, and that other sections of the law indicate that subsidies are available to people in states served by the federal exchange. In July, a 4th U.S. Circuit Court of Appeals panel in Virginia unanimously ruled in favor of the administration in King v. Burwell, saying subsidies should be allowed in all 50 states. The plaintiffs in the King case appealed to the Supreme Court.

CODINGLINE CORNER
Query: Bilateral Night Splints

When a patient presents with bilateral plantar fasciitis, is it acceptable to dispense and bill for two night splints (L4396)?

Charles Perry, DPM, Cambridge, OH

Response: While the policy would allow for dispensing bilateral night splints, I have in the past only dispensed a single night splint to any given patient, telling them to wear it on the right side on even days, the left side on odd days, or whatever means of reminder works for them. I do not suggest dispensing bilateral night splints.

You need to separate medical necessity and reimbursement policy from practicality. Do you really expect the patient to remove the night splints (both of them) at 2 AM if they need to go to the bathroom and then re-apply them? If kept on, one of these splints is difficult enough to sleep with and navigate; can you imagine two?

YOU CAN'T MAKE THESE THINGS UP
RE: Absurd ICD-10 Code of the Day

W55.41XA: Bitten by pig, initial encounter?.

What did you think was going to happen?

First, be sure that the patient is restrained from doing whatever he or she may have done to provoke the pig in the first place. Security should be placed on alert. Also, what was this person doing in a farm setting in the first place? Pigs are not pets.
