Research By UT Podiatrists Offers Free Treatment for Patients
In a tough economy, people may have a difficult time paying for food and shelter, let alone medical insurance and doctor visits. Some people have found a creative way to get free medical care and earn some extra cash at the same time. "A group of podiatrists in the valley got together and organized a group called Mountain West Foot and Ankle," Dr. Steven Royall, a Salt Lake City podiatrist, explained. "Part of our group was to solicit research opportunities through clinical research companies like Jean Brown."

Currently, Royall is conducting a bunion study with Jean Brown Research. And because his practice is large enough that he sees a diverse group of patients, Royall will solicit clinical trial participants through his private practice. Research is an important part of medicine. "We have to have new subjects to study new medication, drugs," he said.

1) Avoid the terrible "toos" - don't do too much, too soon, too often, too fast, too hard, and with too little rest. 2) Wear the proper running shoe for you! - ensure proper fit - sized by a pro, and choose the right shoe for body type, ability, mileage, and foot type 3) Increase training slowly - increase time, or mileage by only 10% max/per week.

Nicholas Sol: The current protocol in our office is to perform epidermal nerve fiber density via skin biopsy. Then we determine if patients are candidates for a course of therapy using Metanx. Of all the treatments that are out there, Metanx is the only one that has the ability to change the future. I have seen it regenerate nerves. I have seen patients who have zero nerve fibers at baseline, but six months later have measurable nerve fiber density that they didn’t have before.

That's probably a small area of maceration or even blistering of the epidermis. Some water gets in under the skin from the incision or a small fissure in the area and macerates the skin, making it white. The skin dries out, so it goes away.

RE: Insurance Request for Charts

I worked with USFHP and Texan Plus (Selectcare of Texas) for years. They routinely requested odd numbers of charts like 68, 89, and even 102. Each time they too came to the office and scanned in the charts. Each time we received feedback that my documentation was excellent and never did any of them ask for refunds. I don't think that you need to be concerned if they request more than 20 charts.
We never needed an attorney, and I don't think that chart request has to correlate with suspected fraud. A couple of insurances come yearly and survey the office, and some even send a pre-audit survey of standards they are looking for. The best thing to do, in my opinion, is to cooperate, and if your documentation and billing is in order, you won't have a problem. You must be transparent.

There is no doubt in my mind based on the wording of the question that this is a routine request by (probably) a Medicare Advantage company to search for non-billed diagnoses in order for the MA company to maximize their own reimbursement from CMS. This has been discussed many times in PM News. There is absolutely no cause for any concern at all by the DPM. This has been going on for about 2-3 years, and almost every podiatrist today who is in practice is familiar with this. The number of charts is meaningless. It just means he is seeing a large number of these patients.

Bradley Bakotic, DPM, DO a "Podiatric Hall of Famer" hit another one out of the park this past Thursday night. Bako labs not only sponsored the Nassua/Queens division, NY podiatry meeting, he personally agreed to match a 7,000 dollar "PAC" donation if the respective divisions were able to raise $7,000. The divisions were up for the challenge and met it.
I applaud Dr. Bakotic for his commitment to the profession. I don't know of any individual who currently is impacting the profession as he does from both an educational stand point and financial one. Thank you, Dr. Bakotic - your efforts are noticed and appreciated.

RE: Best Running Shoe for Orthotics (Robert Chelin, DPM)

A "neutral" shoe? Neutral in what way? Or, are you just following the marketing of shoe companies? A cushioned shoe is in no way "neutral" because stability and motion control shoes do not have a varus or valgus built into them. What's there to be neutral about?
As for a shoe for orthotics, you first must understand that the shoe must provide a stable foundation for the orthotic.The operative word is "stable", i.e., side-to-side motion is controlled.
Putting an orthotic in a "neutral" cushioned, unstable shoe, will cancel out the correction the orthotic is attempting to correct. Except for that very occasional cavus rigid foot, no functional orthotic should go into a neutral cushioned shoe.

Reconstructive Foot and Ankle Surgery Fellowships (Two Available)
Each of these two fellowships is a one-year experience during which the Fellow will develop a rational approach to revisional and reconstructive foot and ankle surgery. The Fellow will function as a clinical instructor and participate in the teaching of Podiatric medical students and residents, and have ample opportunity to be involved in clinical and basic research projects. The fellowship will provide the candidate, further expertise in Charcot reconstruction, soft tissue coverage, trauma and deformity correction. The fellow is expected to complete two clinical or basic research projects during the year.
Duration: 1 year (July 1, 2012 - June 30, 2013) Stipend: $44,100 Application Deadline: November 21, 2011 Interviews:10/1/2011 – 12/12/2011
Requirements: Completion of 3 year residency; a commitment to a part-time/full-time academic career in Podiatric Medicine and Surgery. Texas License eligible (Test date April 16, 2012 with an application deadline of February 16, 2012). ABPS Qualification eligible in Foot & Rearfoot / Ankle Surgery (Test dates TBD).

RE: Most Docs Would Limit Access Under SGR Cuts: Survey

My practice is basically wound care, especially limb amputation prevention. This means that a majority of my patients are elderly and thus on Medicare. This cut that is being recommended, or has already been approved for Medicare patients, will most likely close down practices such as mine. I find this very confusing. Prevention of amputations should be saving Medicare a substantial amount of money. If the goal is to reduce Medicare spending, I think it would be prudent to spend the dollars more carefully.
I have been hearing from some of the local cardiologists that their residency spots are not filling up as they used to. Medical physicians as well as other specialties are seeing the same inclination. I think Medicare has found a way to...

RE: Early Diagnosis of Bunions Can Prevent Surgery (Jeff Root)

I have to laud Mr. Root on his honesty regarding foot orthotics, especially because he has an interest in an orthotic lab.
He stated that (orthotics) have the potential to prevent or retard progression of bunions. Conversely, (orthotics) have the potential to accelerate bunions, and occasionally patients have reported that bunions got worse. It is clear why insurance companies and MDs question orthotics. I agree with Mr. Root that much more research is needed to prove that orthotics work.
