No Scientific Research That Barefoot Running Reduces Injuries: APMA

As barefoot running has grown in popularity, it has drawn criticism from podiatrists who say there is no scientific research that shows barefoot running reduces injuries. The American Podiatric Medical Association recently released a statement urging runners to be wary of the method, noting the lack of peer-reviewed studies and expressing concern that barefoot running could lead to puncture wounds from road debris or added stress in the lower extremities.

“The surfaces that we tend to run on now are different than what our ancestors were running on when they were running barefoot,” said James Christina, a podiatrist and the director of scientific affairs at the podiatric association. Jeffrey Ross, a podiatrist in Houston who specializes in treating runners, said he had seen six patients over the last three months with injuries that he attributed to running barefoot or in minimalist shoes. “Now it’s become a standard question, what are you running in?” he said.

Query: Mortality Statistics for Charcot Surgery

We are all familiar with the mortality statistics associated with major lower extremity amputations. Given the patient pool (multiple co-morbidities, obesity, non-compliance, psychological issues, etc.), I was wondering if anybody was familiar with the mortality statistics (5-year survival) associated with reconstructive Charcot surgery (frames, beams, and all)?

I currently employ two podiatrists who are partners in another location and pay them on a percentage basis, with checks written out to their group (not to doctors personally), and at the end of year they are issued a 1099. It was brought to my attention recently that this may not be a legal way to reimburse for services provided. Any suggestions?

I have deeply investigated over 15-20 different EMR systems. The fear of EMR transition like Dr. Marc Katz has illustrated is an unfortunate point that many doctors are running into. But I also think you can be worse off if you wait. Forget about the meaningful use reimbursement; getting an EHR to increase the efficiency and flow of your office as well as increase your ability to bill more efficiently is paramount to becoming a successful office in the new healthcare industry we are developing into. The transition will not be easy and it will not be smooth, but significant and effective change never is. Doctors who are scared to make a change are going to find themselves outpaced.

The meaningful use reimbursement is tiered over a 5-year scale and totals $44,000 over those 5 years. The biggest year is 2011 at $18,000 per provider, and you must have 90 days consecutive use of meeting the meaningful use requirements within that year. That means you must be up and fully operational by September 1, 2011. If you don't, you'll miss out on the $18,000.

I have no tie to any particular EMR company and have no financial incentives, but I have been through the headache of looking for a new EMR. I would be happy to share what I know with anyone who is interested.

RE: NYCPM Establishes Academic Department of Orthopedic Surgery

I applaud Dr. Positano on the formation of this department at the New York College and that orthopedic surgeons will be instructing students about orthopedic surgery. But correct me if I’m wrong, it appears that the orthopedic surgeons at the Hospital for Special Surgery place podiatry mainly in the “Non-Operative Foot & Ankle Service.” Looking at their website, if a patient is interested in foot & ankle surgery, the links list only orthopedic surgeons and no podiatrists.

I commend the effort to cultivate a relationship with orthopedic surgeons at this hospital but I just find it ironic that the hospital/ortho department does not recognize surgically board-certified podiatrists as being foot and ankle surgeons and does not list them nor have them on staff under foot and ankle surgery. I am not from New York nor do I know anything more about the situation at this hospital except from this media excerpt and the hospital's website, so feel free to clarify any of my observations.

RE: Wrong-Patient, Wrong-Site Procedures Persist Despite Safety Protocol (Henry Slomowitz, DPM)

For the past 32 years, I have seen the patient before any sedatives and asked one simple question: "Which foot do YOU think we are scheduled to work on today." I then mark a “NO” on the non-operative foot and a “YES” on the operative extremity. If I am going to be working on the posterior of the leg or foot wherein the patient may be prone, then I mark the posterior of the extremity. I have even added arrows to a foot if many toes are going to be done.

My own chart notes, my own office consent, and my own marks on the extremity take precedence over anything a nurse or an OR tech may have marked. This hasn't failed me yet, and I hope I haven't jinxed myself now saying that out loud.

My podiatric surgeon, Dr. Pedram Hendizadeh, saved the day by simply writing "YES" on the correct foot and drawing arrows pointing to the digits that were to be operated on. Because of a previous surgery, the admissions desk at the surgical center had filled out paperwork relating to the wrong foot, and the pre-op nurse had included an extra digit. I am really glad that my doctor was on his toes!

I have to agree 100% with Dr. Menn. When it comes to the game of golf, as in the game of tennis, both sports deliver such abnormal positions and stresses to the foot and ankle structures that eventually something will "give up". No amount of therapy, corrective footwear, strapping, bracing, or even surgery, will conquer the potential of permanent damage that these sports may cause, with continued use and abuse, to the lower back and extremities.

I agree with Dr. Menn's assessment. The only REAL innovation in golf shoes that was historic was the "Weight Rite" golf shoe. In 1990, the company put these on John Huston and he wore them at the Honda Classic, at least in one round. The USGA/PGA disqualified the shoe because it contained a device that assisted the golfer. Weight Rite immediately sent John Huston shoes without the "addition" that made them "swing enhancing." He had to wear regular golf shoes, and oh by the way, he won the tournament without the artificial help.

What was this "revolutionary" devise? A valgus wedge in the shoe to pronate the foot so he would not sway off the ball on the backswing so weight transfer would be easier. I once had a superball partner in a tournament show up in a pair of boat shoes with a slick bottom. He has a long flowing balanced swing, swung easily at the ball, and made 6 natural birdies to help our team win. Afterwards, he said that he was throwing away his spikes.

If you have the time and interest to review the literature regarding drug and device manufacturers, you will conclude that they over-emphasize the benefits of their products and downplay the risks. Pharmaceutical and device manufacturers are for-profit companies which have fiduciary responsibilities to their investors to maximize profits. They are only in the business of education if it improves their bottom line.

It is difficult to scan high impact publications monthly and not read something about...
