Insoles can alter the inside of your shoes by adding some much-needed arch support, which "can help bring the foot’s arch back up to a more neutral position," explains NYC podiatrist Jacqueline Sutera, DPM. "Insoles can also offer added cushion, comfort, shock absorption, and padding."

If you have high arches — aka cavus foot — excessive weight is placed on the ball and heel of foot when walking or standing, according to the American College of Foot and Ankle Surgeons. Things to Consider When a Physician Goes Part-Time - Part 6
 
There are many factors to consider in coming up with an effective part-time/reduced modality policy that works with the physician practice. The following are some concepts to consider:
 
6. A physician-owner's change to part-time can affect a practice's compensation formula in many ways.
  • In some cases, a simple proration of a set base salary or share of income may be all that is needed. In determining a policy, it is important to take into account the sources of income and expenses for the practice and the particular physician.
  • For example, perhaps some of the ancillary services will no longer be used by the particular physician who drops a modality. Should he share in the income or the expenses of the modality? Additionally, a physician that goes part-time may be leaving the practice with unused examining rooms and staff, and perhaps the practice wants to hold such physician responsible for a full-share of the overhead costs despite the change in status.
  • Each practice must think through carefully how the proposed change will impact the remaining physicians and the way in which the group operates.

Query: Off-loading the Second Toe 
 
What suggestions are there to off-load the second digit after a hallux amputation? I have tried orthotics, moldable custom silicone toe wrap, and different shoes. Any help would be appreciated.

QUERIES (MEDICAL-LEGAL)
Query: How Much to Charge Ciox for Chart Requests
 
What is a fair fee to charge Ciox for chart requests considering staff time diverted from regular duties. Paper chart vs. EHR? Copying involves more than simply photocopying in my case. I still use paper, so staff must pull chart, disassemble demographics sheet stapled to folder, copy all pertinent sheets, then reassemble. I review each chart to be sure all are signed and dated as required. I charge $10 chart and have received my fee after refusing to allow their rep to come to my office to scan due to COVID-19 and HIPAA concerns. I have read previous responses of up to $25 chart vs. a $2 per page fee.

My opinion is that podiatrists can treat any foot condition. PVD, absent pedal pulses, etc. might be due to COVID-19. As long as any foot condition might involve COVID-19, such testing should be able to be ordered by a podiatrist.

It has been published that these multiple requests from insurance companies for our records is so that they can upcharge for what they are being paid for procedures we have not billed for. If this is true, and they are not looking for fraud (which seems correct), we must be under-billing. Maybe, if they are getting paid more for our services than WE bill for, we should be privy to that and bill those services we performed as well. If they get paid, should WE not be paid too? 
 
And, more importantly, if I got a huge number of requests ,I would look at my billing policies. Remember, these dudes only make money from paperwork, not patient care. It is just paperwork; they provide no patient care. They do nothing for the patient, the quality of care, and probably nothing for the improvement of healthcare in the country. They profit by BILLIONS of dollars. Don't believe me? UnitedHealthCare is in the top 5% of profitable companies on the S&P over the last ten years; CNBC in 2019 gave them the #1 best profit investment over ten years. This is not an opinion, but actual numbers, black and white.

This issue has come up many times in the past. I have two solid arguments against providing this information to Ciox or outside contractors working for third-party payers. Contracts with third-party payers often contain language regarding the need to provide those carriers with charts for claims processing. But the information they are seeking is not auditing your practice nor used to process claims.
 
As for the second argument, I admit it is a bit weaker. Ciox and others are contracted by the third-party payer. Contracts often, but not always, stipulate that the payer may use an agent or subsidiary which may request records and that you consent to that. Again, that may or may not be enforceable because the agent (Ciox) is not using the data to audit your chart. This is purely done for data mining. My advice which worked since this harassment started was to do the following:
