NY Podiatrist's Compositions to be Showcased at Concert
 
"Staten Island Treasures," the upcoming 10th anniversary Staten Island Philharmonic concert, will showcase new music — all 21st century works — by  local composers. The "Treasures" composers are Sanchie Bobrow, violinist and educator; Michael Sirotta, percussionist and longtime LaMaMa composer; and Dr. Howard Fox, a  moonlighting podiatrist who has been writing music for many years now.

Howard Fox, whose Scenes of the Greenbelt will be the afternoon's major work, was a music major in college until he noticed that there were easier ways to make a living. He switched to medicine, but never gave up studying and writing music. Forty years later, now "semi-retired" from podiatry, the Dongan Hills resident has time to devote to composing. Musically, he is drawn to the late Romantics, Berlioz to Ravel, and characterizes his own approach as "more Monet than Picasso."

Podiatry Ranked 5th Riskiest Job for Your Health
 
Some jobs intrinsically have more health risks than others. A nurse working in a hospital is more likely to catch an infectious disease than a lawyer working in an office, for example. To rank the most unhealthy jobs in America, we used data from the Occupational Information Network, a U.S. Department of Labor database full of detailed information on occupations. In order to analyze jobs by their impact on workers' health, we took measures of six health risks in each of the 974 occupations in the database.
 
The top 5 most dangerous jobs are:
1. Dentists, dental surgeons, and dental assistants (unhealthiness score: 65.4)
2. Flight attendants (unhealthiness score: 62.3)
3. Anesthesiologists, nurse anesthetists, and anesthesiologist assistants (unhealthiness score: 62.3)
4. Veterinarians and veterinary assistants and technicians (unhealthiness score: 60.3)
5. Podiatrists (unhealthiness score: 60.2)

Physicians Decry Broken Promise of Medicare Raise in 2016
 
The law that repealed Medicare's sustainable growth rate (SGR) formula for physician pay called for an annual raise of 0.5% from 2016 through 2019 as part of a transition to value-based reimbursement. When Congress passed the law in April, some leaders of organized medicine noted that the modest raise lagged behind the inflation rate, but said it was better than nothing. It was certainly better than the disastrous 21% pay cut that the SGR formula would have triggered in 2016.
 
Medical societies sold their membership on the legislation, called the Medicare Access and CHIP Reauthorization Act (MACRA), in part by saying it would stabilize Medicare rates for several years. However, the promised raise of 0.5% turned into a 0.3% pay cut in the fine print of the final 2016 Medicare fee schedule released last week. The reason? The Affordable Care Act (ACA) and several other laws that set Medicare reimbursement policy trumped MACRA.

Query: Tapered Orthosorb Pins
 
My partner and I were surprised earlier today when we were handed a new product in the OR when we asked for tapered Orthosorb pins. We have successfully used tapered Orthosorb pins as our primary form of fixation of distal 1st metatarsal osteotomies for many years. Compliance has been good and we have experienced no significant complications. Instead of the PDS-based product, we were given a PLLA/PGA pin labeled "Orthosorb LS, 1.3mm Tapered Pin Kit." Not only was the pin made of a different material, the familiar K-wire guide pin lagged to the purple tapered Orthosorb (PDS) was gone. The clear pins came only with a separate sizing K-wire with no guide or plunger.  
 
The OR staff said that this is what was shipped to them using the previous product number. The hospital is investigating, but it appears that the original, traditional tapered Orthosorb pin has been discontinued. I am looking for feedback and opinions from my colleagues who have used the product.

Query: Use of Multi-Dose Vials
 
A recent newsletter from PICA detailed a case in which a patient developed an infection following a steroid injection. The podiatrist had used a multi-dose vial, and indeed had a second patient who developed an infection from that vial. Without discussing the merits of the case, I am wondering what others do? Do you use multi-dose vials? Do you only use single dose vials?  And with that, what about the single dose injection kits that were being pushed a year or two ago at an outrageous reimbursement?

RE: Italian Doctors to Hold One-Day Strike

it is reported that on Dec 16th, Italian doctors will hold a one-day strike to protest government cuts in healthcare. What an opportune time for American doctors to do the same in light of the recent announcement of future Medicare cuts. If I were on the executive board of the APMA, I would be on the phone contacting the AMA, and any other physician associations I could think of, to make this happen. At some point in time, doctors have to make a stand. The Italians are doing it and it is in the media. The domino effect of such media attention, I opine, would be good.  
 
Of course, we could just do nothing and continue on this wayward path that we are currently on. After all, it's always easier to do nothing.

I agree with Dr. Markinson that we should be lobbying to change the rules. As they are now, patients cannot understand them. I spend more time explaining to a married couple why the husband is covered for routine foot care and the wife is not than I do treating them. It gets even more bizarre when I tell someone that Medicare will pay for cutting 5 nails but if the patient wants me to cut all 10, they have to pay for the second 5. The patients also find it strange that I tell them they have to see their internist every 6 months or they are not covered.
 
Debridement of nails should either be covered or not covered - not subject to interpretation of bizarre rules. What is the history of the rules for routine foot care and mycotic nail care? Are any other Medicare-covered treatments subject to similar rules?
