NYSPMA Foundation Funds New NYCPM Recruitment Videos
Thanks to a generous grant from the NYSPMA Foundation, NYCPM Student Services commissioned new student recruitment videos, coordinated by Dean Lisa Lee. The four new videos were created by Digital Seven from footage shot at the College, Bridges (our student housing), Metropolitan Hospital, and College events. These events include footage from the White Coat Ceremony and Welcome Cruise for the Class of 2022.

The videos will be used by Student Services on the College's new YouTube channel (called NYCPM Enrollment), at open houses, on college campuses, and via a social media campaign. This will start in early November.

AZ Podiatrist Teams with Orthopedic Surgeon
Renowned local orthopedic surgeon and philanthropist, Dr. John Cory, is expanding his business, The Arizona Institute of Motion, from Glendale to North Scottsdale. Dr. Cory has joined forces with podiatrist, Dr. Albert Eulano, and will expand services to include Achilles tendon reconstruction, diabetic foot and ankle problems, foot and ankle reconstruction, neuropathic fractures, and sports injuries of the foot and ankle. The pair has renovated a 3,000 sq. foot office and will officially open Dec. 1, 2018.

CMS Delays E/M Coding Changes in New Physician Payment Rule
Physicians hoping the Centers for Medicare & Medicaid Services (CMS) would modify some proposed changes to evaluation and management (E/M) codes got their wish Thursday when the agency issued its final Physician Fee Schedule rule for 2019. On Thursday, CMS' Seema Verma said the agency had tempered its initial approach.
"After listening to the concerns of the provider community, we will maintain a separate level of payment for the most complex patient care -- that's level 5 -- and we are also assigning equal value to add-on codes for primary and specialty care," Verma said. Essentially, CMS will ultimately adopt a three-code approach to E/M reimbursement in place of the proposed two codes, and the agency won't implement the new structure until 2021.

Convincing Patients to Respond to Surveys is Easier Than You Think - Part 1
You might be surprised to learn that 86 percent of Americans say they would likely take a survey from a healthcare provider, if asked. A West survey of 1,036 adults in the United States showed patients are more open to taking surveys than most providers realize. The following recommendations, based on West’s survey findings, show three key ways healthcare teams can grow response rates by focusing on factors patients say influence their decision to participate in surveys. 1. Make it convenient and easy for patients to respond. Convenience is a must-have when trying to convince patients to complete surveys. Four in ten Americans (43 percent) say they would be more likely to take a survey from a healthcare provider if they could do it on their own time. This makes online surveys an attractive option for patients. Online surveys give patients the flexibility to respond when it is convenient for them, rather than receiving an unexpected call from a live person and being expected to stop what they are doing to answer survey questions.

Query: Plantar Fasciitis and Workers Compensation
I have been requested by a Workers Comp attorney to comment on plantar fasciitis and job requirements. The patient is a 33 year old, obese but healthy woman who worked at the post office where she stands and walks daily on hard floors.There was no history of trauma. I saw her two years ago but conservative care failed, and she declined surgery or other modalities, and left work. I haven’t seen her in two years, but get the impression from the attorney’s letter that she is still suffering from plantar fasciitis. The contention is that the onset of this condition was work-related, even if work contributed even minimally. The attorney states it is established Federal law that if work contributed even “minutely” to a condition, it may be stated to be a case of Workers Compensation.
There are so many things contributory to plantar fasciitis such as equinus, pronation, lack of shock absorption, body weight, and walking surface. This is not a place for “opinion” as much as fact. Does anyone know of any recent literature, or perhaps other rulings or studies? Is there anything besides opinion, since we see overweight people with PF as well as thin people, sedentary people as well as active ones, runners, and non-runners.

I am an ABFAS certified podiatrist with training in ankle reconstruction. Because of state laws, I have not done a traumatic ankle repair in many years. Does anyone know of a certified hands-on refresher course to recommend?

RE: Dealing with Pay Reductions for Multiple Procedures

When we do more than one procedure on a date of service, we typically get a 50% reduction on the allowed insurance amount on the second procedure and about 75% reduction if a third procedure is also performed on the same time. For example, we recently did a wart excision and ingrown nail excision on a patient at the same visit and received only $121 for the ingrown procedure. I did bilateral bunions at the surgery center and was allowed $550 for the first bunion and $275 for the second! It's frightening to see how little we get paid on the 2nd or 3rd hammertoes when more than one toe is done on the same date of service!
As a result, my billing/collection staff keep requesting that I don't do more than one procedure at a time on our patients whenever possible. If a patient has two or more of anything, such as two ingrown toenails or hammertoes, they want me to do only one toe or procedure at a time and have them back for the additional procedure(s) so that we can get 100% of the allowed amount for each procedure.
I do not want to have to practice this way - one toe at a time, but we are losing a lot of money otherwise. How are others dealing with this dilemma? Are you just doing everything that patients need on a date of service and not worrying about (and accepting) how little you get reimbursed on the multiple procedures?

Dr. Sung states that we are living in dark times, and the current administration does not support our profession and our patients. It this so? The Trump administration has increased Medicare payments to providers, and has allowed veterans to see outside providers. Trump has confirmed his support for pre-existing conditions coverage. I do not feel so dark and hopeless under Trump. Most of the nonsense that we hated has started under President Obama. Various social programs such as Medicare, Medicaid, and others, are still functioning pretty much the same. What is Dr. Sung talking about?
