Hypermobile Joint Disease Rarely Recognized: MI Podiatrist
 
"Do you remember the kid in school who entertained everyone with his stupid tricks? He was the one who could place both elbows well behind his head or fold himself like a pretzel. Entertaining fellow, but possibly the victim of a serious, potentially debilitating disease. Although the diagnosis is infrequently made, the research shows this is simply a failure to accurately recognize the condition, formerly known as Ehlers-Danlos Syndrome," says Conway McLean, DPM.

"As is often the case, science often changes its mind and changes disease names, a logical move as we learn more about a disease. This constellation of inherited conditions, all closely related, is now termed hypermobility spectrum disorders (HSDs). The term hypermobility refers to an increased active or passive movement of a joint beyond its normal range. These are an array of connective tissue disorders, with the notable characteristics being joint instability and chronic pain," says Dr. McLean.

He has never done better than third, but since his first open final in 2010, in Biarritz, where he had (already) taken third place, Ian Fontaine is the surfer who has played in the most finals at the French open championships: five in the last twelve editions, that of 2020 having been canceled due to the COVID-19 pandemic.

At the recent French Championship, he could not do better than to obtain a new bronze medal, his third in open, with a slightly bitter taste at the time. “As in every final in fact, I made a few mistakes. There, I wanted to wait for some good lefts to score better, but in fact, compared to what happened next, I should have stuck to my option of taking some good straights. Physically, I was also a bit burnt out with the opening of my practice (podiatry) at the end of May."

CMS Cracks Down on Medicare Advantage TV Marketing
 
CMS is cracking down on deceptive marketing practices and will no longer allow Medicare Advantage or Part D prescription drug plans to advertise on television without agency approval first. The new policy is effective Jan. 1 and was discussed in an Oct. 19 memo from CMS to MA and Part D providers. The agency said it issued the new policy after reviewing thousands of beneficiary complaints regarding confusing, misleading or inaccurate information from plans — plan sponsors are also responsible for all marketing activities from brokers and third-party agencies.
 
"CMS has conducted so-called 'secret shopping' by calling numbers associated with television advertisements, mailings, newspaper advertisements and internet searches to monitor the experience beneficiaries have engaging these entities," the agency wrote. "Our secret shopping activities have discovered that some agents were not complying with current regulation and unduly pressuring beneficiaries, as well as failing to provide accurate or enough information to assist a beneficiary in making an informed enrollment decision."

Query: Gait Training
 
Our surgical patients are having an issue scheduling physical therapy in a timely manner for gait training. We want our staff to perform the gait training in our office. We are using the CPT 97116 CPT, but Medicare is denying the claims because a modifier is required. Can DPMs use the -GP modifier?

Response: Podiatrists can bill physical therapy services as long as the plan of care for physical therapy has been outlined for your patient. So, in this case, you can bill a gait training as long as part of physical therapy plan of care. In auditing, most podiatrists do not document the physical therapy separately as a plan of care unrelated to the E&M visit. I would say that more often in audits, the gait training is considered part of your evaluation and management service and is not billed separately. However, if you are doing a plan of care for physical therapy for the patient, then you would append the modifier -GP.

I strongly agree with Dr. Robertozzi. The percentage of liars who act as experts is unusually high. They are doing a great disservice to not only our profession but sometimes to the patients who have been traumatized and abused with great pain and deformity. It is about time that one of our organizations, in my opinion ABFAS, passes rules and regs that would punish outright lying while acting as an expert.
