Many Foot Conditions are a Result of Tight Calves: TX Podiatrist
 
"200 million Americans will have a foot problem sometime during their lives," says Zach Barnett, DPM with Texas Foot and Ankle Associates. "This is because the foot, unlike the hand, is a weight-bearing structure. As you walk, you put a load on structures like ligaments and tendons, so you can have chronic type injuries based on foot position, or trauma like falls."

"Some of the most common things I see including Achilles injuries, plantar fasciitis, and foot pain are generated from a tight calf. That condition happens over time and because of the tightness, you start compensating by rotating the hip, and you wind up with bursitis of the knee and hip," says Dr. Barnett.

 NY Podiatrist Discusses Atherosclerosis and the Feet
 
Atherosclerosis is a condition that causes fatty deposits in your arteries and prevents adequate blood flow to your feet, causing poor circulation. When enough warm blood doesn't flow to your feet, it can cause cold feet.

"The arteries in your feet are the smallest, which means they are most likely to be affected first. When these arteries are blocked, you could experience cold and sometimes painful feet," says Danielle DesPrés, DPM, a Board certified foot and ankle surgeon and podiatrist in NYC on faculty at the New York College of Podiatric Medicine. According to Dr. DesPrés, risk factors for atherosclerosis include: Smoking, high blood pressure, high cholesterol, and old age.

As the number of coronavirus cases in Scott County continues to rise, the local health department is urging residents to wear masks when in public places. In a message shared via social media on Tuesday, the health department stated “it is imperative we, as a community, take every precaution necessary to help slow the spread of COVID-19 in our community."

DMEMACs Coverage Policy to Include Payment for Alginate, Fiber Gelling Dressings  
 
The Center for Medicare and Medicaid Services’ (CMS) Durable Equipment Medical Administrative Contractors (DMEMACs) updated its “Local Coverage Article: Surgical Dressings” to include payment for secondary, as well as primary, use of alginate and other fiber gelling dressings. Prior to the October 15, 2020 Policy Article Update (A54563), these products were only reimbursed when used as primary dressings - limiting healthcare providers’ discretion and choices in properly treating their wound patients.
 
By providing coverage and thus payment for these important wound care products, the updated policy now allows wound care clinicians further access to them to treat their patients. In addition, the DMEMACs’ coverage policies will now be in line with manufacturers’ existing instructions for use (IFUs), which specified that alginate and other fiber gelling dressings were intended for both primary and secondary use.

Query: The Basics of L3260
 
I am reading conflicting information regarding the dispensing of a post-operative/cast shoe. Medicare never seems to pay for this but commercial carriers usually do. But I am reading that this shoe is NOT separately payable when it is dispensed in conjunction with a surgical procedure code. You cannot have the patient sign an ABN and charge the patient for the shoe. Please clarify the do’s and don’ts of using the L3260 HCPCS code.

Response: Your question is really about dispensing a post-op shoe; is it a separately billable expense always? My answer is that it depends on the circumstances. If you take the patient to an OR inpatient, outpatient, or ASC and the facility never provides the L3260 and you do afterwards, then you should be able to get reimbursed for that shoe. Next, Medicare will never cover L3260 as it is statutorily excluded, so no ABN is required (you can do one if you feel you must), and the patient pays for that surgical shoe on dispensing.
 
If you bring the surgical shoe to the operating room on the day of the surgery, the facility is technically supposed to pay for and dispense that device. However, in the spring of this year, I communicated the general DME question with my good friend and colleague Dr. Kesselman about dispensing in an ambulatory surgical center (ASC) when it comes to DMERC/Medicare. This was his response: “As long as they are only dispensing to their own patients, the physician may dispense from the ASC.” If you dispense from an ASC, make sure that patient understands they are going to pay for that L3260, and I would have them sign a proof of delivery at minimum.
