15% of Diabetic Wounds Progress to Amputation: OH Podiatrist
The Wound Healing Center at Ashtabula County Medical Center (ACMC), a member of the Healogics network, participated in the recent Healogics National Diabetes Campaign. “Every day we see the devastating effects of those suffering with diabetes and diabetic foot ulcers,” said Dr. Michelle Dunbar, podiatric surgeon and panel physician for ACMC’s Wound Healing Center.

“Studies show 15 percent of diabetic wounds will progress to amputation which is why we encourage the collaboration of caregivers and physicians to recognize the signs of diabetic foot ulcers and seek appropriate treatment as soon as possible; together we can reduce the risk,” she said.

NY Podiatrist Seeks Re-election to Monroe County Legislature
There are nine contested seats in the Monroe County Legislature this year. Legislators serve four-year terms and make a base salary of $18,000 that can be supplemented by filling certain leadership positions. Voters will make their decisions on Tuesday. Monroe County Republicans have maintained a strong majority in the chamber for years, and that is unlikely to change on Tuesday.

16th District covers most of the town of Irondequoit and has been represented by a podiatrist, Dr. Joseph Carbone since 2012. Carbone is running on the Republican, Conservative, Working Families, Independence, and Reform line. His top priority is creating and retaining good paying jobs and keeping the property tax rate flat.

CA Podiatrist Speaks to Sky Hoop Campers
Beverly Hills podiatrist Dr. Michael Levi recently discussed the field of podiatric medicine to campers at Kareem Abdul Jabbar's Sky Hoop camp in the Angeles National Forest. Jabbar is former captain of the Los Angeles Lakers and multi-time NBA Champion. The camp provides an opportunity for underprivileged children to spend a week in the wilderness.

Dr. Levi has been practicing for 25 years in Santa Monica. He is on staff at St. John’s Hospital, where he is chief of the podiatric surgical section. He is the team podiatrist for the Los Angeles Clippers of the National Basketball Association, the UCLA Tennis team, and the Los Angeles Ballet Company. He has volunteered for over 25 years at the Venice Family Clinic, which offers low income families the ability to seek medical care.

OH Podiatrist Discusses ICD-10
The U.S. medical coding system was updated last month for the first time in 36 years at hospitals and physicians’ offices across the country, a switch-over that in many ways was invisible to patients. According to Upper Valley Medical Center Vice President and Chief Medical Officer Dan Bailey, DPM, the new system includes nearly 177,000 codes for various diagnoses and procedures, which is up from 17,000 codes in the former ICD-9 Coding Nomenclature.

Using the example of a broken bone, Dr. Bailey explained how ICD-10 requires doctors to note the type of fracture — closed, open, displaced, non-displaced; specific anatomic site; laterality; routine versus delayed healing; non union; mal union; and the type of encounter – initial, subsequent, or sequel.

Allen Jacobs: Today, we all live with EMRs, and I despise them. Often, I read charts with entries that clearly look like they were auto-filled. I think EHRs have made the notes worse. What keeps a plaintiff's attorney away is a chart that reads crystal clear, explains what the doctor is thinking and communicating. This gets attorneys thinking that it not worth investing their own money in a case with great medical records. Also, photos are golden and often can make the difference. 2 Patients of PA Podiatrist Admit Role In Pain Med Scheme
Two Fayette County men who were among seven accused in a pain medication scheme involving a Uniontown podiatrist entered guilty pleas Friday. Randell Keith Rice Jr., 28, and Carl E. Santavicca, 63, both of Uniontown, pleaded guilty to one count each of acquisition of a controlled substance by misrepresentation and conspiracy. Both men were patients of Dr. William Ainsley, 40, of North Union, who investigators allege routinely conspired with patients by prescribing them thousands of hydrocodone tablets and receiving up to half the tablets back in return. Ainsley is awaiting trial.

Query: Chronic Inflammatory Ulceration
Several years ago, this 53-year-old healthy female underwent 1st MPJ fusion by another physician. Over time, she developed enlargement of the hallux, centered around the IPJ, with a large exostosis. She came to my office for treatment. I took her to the OR one year ago for an exostectomy at the IPJ. She healed well and was doing great. The toe returned to almost-normal size. Six months after surgery, she developed ulceration over the IPJ with recurrence of edema and inflammation. I was able to reduce the ulceration in size with wound care, but it will not close.

The patient has daily exudate which is cream-colored. Cultures are negative for bacteria and fungus. The exudate does not contain urate, only fibrin and small calcific bodies. Blood work is all normal. The toe remains inflamed. A Tc99 bone scan lights up the IPJ, but an Indium111 scan is negative, ruling out infection. I ordered an MRI today. Any thoughts?

I agree with Dr. Caldwell that the CORA needs to be addressed and the head of the first metatarsal needs to be realigned. The issue is that the IM angle will still be significant and the first metatarsal will be short. The medial cuneiform appears atavistic; therefore I would fix the CORA of the first metatarsal, then do an opening wedge osteotomy of the medial cuneiform to fix the IM and get adequate length. Then a second surgery of a metatarsal head resurfacing instead of fusion as I'm unsure how bad the cartilage is on the base of the phalanx. This would allow for continued motion.
