AT THE COLLEGES

CARES Act Brings Remote Learning Labs to KSUCPM Students

When the Kent State University College of Podiatric Medicine (CPM) moved to fully remote instruction in March 2020 in response to the COVID-19 pandemic, the entire building was closed and inaccessible to students. All faculty worked very hard to successfully transition classes to an online format using web-based conferencing platforms such as Blackboard Collaborate Ultra and Microsoft Teams. However, when the anatomy and microbiology and histology laboratories were closed to students for the remainder of the Spring 2020 semester, the faculty in those courses faced a difficult challenge as to how to remotely teach content that typically requires hands-on experiences for students.

It became apparent very quickly that the infrastructure in the pre-clinical sciences laboratory spaces was not sufficient for this type of instruction. Through a collaborative effort between the Division of Preclinical Sciences and the college’s IT department, CPM received more than $500,000 from the CARES Act to improve and enhance the technology in pre-clinical instructional spaces.

Podiatric Care Can Reduce Falls: NY Podiatrist

Fall injuries create a heavy financial burden, accounting for $2 billion in annual hospitalization charges and $624 million in annual outpatient emergency department charges. Reducing falls could come down to podiatric care. “Podiatrists are uniquely positioned to help reduce the risk of falls by treating the foot, providing patient education, promoting health, and encouraging mobility,” said Lockwood-based podiatrist and NYSPMA President James Hanna, DPM. “While muscle weakness, arthritis, and a decline in foot strength and flexibility are all a main cause, falls are a predictable and preventable problem. Simple interventions, including podiatric care, could help to reduce those falls and hospitalizations.”

Dr. James Hanna

In fact, a 2017 study by the NYSPMA fall prevention, showed a 36 percent reduction of falls over a 12-month period for individuals with an increased risk of falls who received podiatric interventions. Additionally, the study showed that a routine assessment and management of foot issues reduces the risk of falls, leading to improved health and decreased frailty among the aging.

PODIATRISTS IN THE NEWS - PART 2
OH Podiatrist Discusses Heel Pain

According to Alison Niemeyer, DPM, "There are several factors that can increase your risk of developing plantar fasciitis. Sudden changes in activity such as a new walking or running program, changes in footgear, age (most common in those 30-60 years old), obesity, and prolonged standing can all lead to plantar fasciitis. Luckily, there are some simple at-home treatments that can help improve your symptoms. These include stretching of the Achilles tendon; icing the heel or rolling your foot on a frozen water bottle; and wearing good, supportive shoes. Sometimes, over-the-counter anti-inflammatory medication, such as ibuprofen and naproxen, is needed to help reduce the inflammation in the foot as well."

Dr. Alison Niemeyer

"While plantar fasciitis is the most common cause of heel pain, there could be other problems causing the pain you are experiencing. Ligament tears, heel spurs, stress fractures, pinched nerves, or other boney abnormalities can also cause pain to the same area. So, if you find yourself limping in the morning, it’s best to get an appointment with your podiatrist to see exactly what the cause is. They can help formulate a treatment plan that will be best fitted for you and your lifestyle," says Dr. Niemeyer.

INTERNATIONAL PODIATRISTS IN THE NEWS
Aussie Podiatrist Discusses Edema of the Feet

Swelling, also known as edema, in your feet usually comes down to your veins not being able to pump blood properly, podiatrist and ambassador for the Australian Podiatry Association Charlotte Bodell explains. If you do suffer from swollen feet, she recommends keeping them elevated when seated, and making sure you exercise at least twice a day — 15 minutes in the morning and 15 minutes in the afternoon.

Charlotte Bodell

Not sure what to do? Bodell recommends starting off with calf raises, explaining that the calf muscle pushes blood flow back up to the heart. If the swelling persists, Bodell says you may need to consider purchasing surgical stockings or talking to your GP about medication to reduce fluid in your legs.

PRACTICE MANAGEMENT TIP OF THE DAY
Things to Consider When a Physician Goes Part-Time - Part 3

There are many factors to consider in coming up with an effective part-time/reduced modality policy that works with the physician practice. The following are some concepts to consider:

3. Once a physician switches, how long may the physician remain in this new position?
  • Some practices use this approach as a step towards complete retirement and might limit the change in status to a maximum amount of time (usually two-to-three years) before requiring full-time retirement.

RESPONSES/COMMENTS (MEDICAL-LEGAL)
From: Pete Harvey, DPM

Before writing that Rx, imagine the worst possible things that could happen if the patient uses the medication. Is it worth it? It deserves strong consideration to allow some other doctor that privilege. We carry insurance for possible train wrecks. However, it makes no sense to get on a train that you know might crash.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Purchasing Alcohol for Neuroma Injections
From: Philip Larkins, DPM

Earlier this year, there were several posts on PM News regarding where to purchase alcohol for neuroma injections. I will not discuss the merits of this therapy or at what strength the injections should be given. This post is strictly about the availability of the denatured alcohol product. Previously, the main manufacturer of this product was American Reagent. They are no longer able to manufacture this reagent.

A company by the name of Belcher Pharmaceuticals in Florida now has exclusive rights to manufacture and sell a brand name ethyl alcohol injection with the trade name Ablysinol. It is being marketed to cardiologists to treat conditions like hypertrophic obstructive cardiomyopathy and other heart rhythm abnormalities, I believe. They were granted this exclusive right under the Orphan Drug Designation by the FDA. This exclusivity end date is 6/2025. Of course the price for this designer ethyl alcohol is staggering. A whopping $1,000 for one single 5mL ampule, normally sold in packs of 10. Anyone still selling alcohol from other sources in the U.S. is just selling old inventory.

RESPONSES/COMMENTS (CODINGLINE CORNER)
From: Jeffrey Kass, DPM

“If you do multiple procedures, then there is a fee reduction for doing multiple surgeries on the same day. If you do them individually, then this would not apply… but that could imply that you are doing so to avoid the multiple procedure reduction. That could be considered abusive billing.” - Tony Poggio, DPM

I opine that the fee reduction imposed by the insurance company for multiple procedures is the abusive practice.
