APMA NEWS
APMA Celebrates Victory on E/M Codes!

APMA celebrated another major advocacy win today as CMS released the 2019 Medicare Physician Fee Schedule final rule. The final rule did not include the originally proposed podiatry-specific E/M codes, which would have reimbursed at a significantly lower rate than codes for the same care provided by other physicians.

Dr. Dennis Frisch

"Today we saw proof that when APMA members work together toward a common goal, we can do great things," said APMA President Dennis R. Frisch, DPM

PODIATRISTS IN THE NEWS
Foot & Ankle Care Interacts with Systemic Conditions: TX Podiatrist

Although the focus of podiatric medicine and surgery is on injuries and diseases that affect primarily the lower extremities, "it's really a broad profession," says Charlton Woodly, DPM a podiatrist in private practice and founder of Woodly Foot & Ankle Specialists in Weatherford and Richland Hills, Texas. Podiatry also intersects with many other aspects of medicine. For example, podiatrists who specialize in wound care or helping diabetics with peripheral neuropathy may interact regularly with the patients' primary care physicians, nephrologists, and other specialists.

Dr. Charlton Woodly

"You would think, 'What do a foot doctor and a nephrologist have in common?' Well we have a lot in common because we treat the same people," Dr. Woodly says. And treatment of one condition can impact the other.

PODIATRISTS AND SPORTS MEDICINE
CA Podiatrist Discusses Cycling "Hot Foot"

Painful burning of the ball of the foot (a.k.a. "hot foot," or metatarsalgia) is usually a result of hot weather or poorly fitting shoes—or both—on long, hilly rides. "Pressure can pinch nerves in one or both feet and shut down a ride fast," Amol Saxena, DPM, a time-trial cyclist and podiatrist in the department of sports medicine at the Palo Alto Medical Hospital in California, says. "If hot foot strikes while you're on a ride, there isn't much you can do other than stop, take off your shoes, and let your feet cool down."

Dr. Amol Saxena

Dr. Saxena will take these precautions to keep his feet cool and comfortable when he races in the Duathlon World Championships.

PRACTICE MANAGEMENT TIP OF THE DAY
Questions to Ask Staff

One of your greatest assets to expand and strengthen your office is right in sight, your staff. On an annual or semi-annual basis, take out each staff member for lunch or meet for thirty minutes or so in your office. Promise that anything discussed that should be confidential, will remain that way. Ask the following questions and be sure to take notes.
  • How can we improve the front desk?
  • How we can we improve the back office? How can we improve our billing process?
  • How can we better internally and externally market our office?
  • Is there any technology we should implement to improve our office?
  • What can I do to make this a better place for you to work?
  • How can I be a better boss?
  • What challenges would you like us to consider to add to your job description?
Compile the answers to the questions and have an office meeting to review them with a primary goal of making the office a better place to work as well as improve efficiency and increase the bottom line.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Elliot Udell, DPM

I feel Dr. Borreggine's frustration and anger when a patient walks out of the door and never pays his or her bill and, in some cases, has no intention of ever paying it. We had one patient who not only told me that the check would be in the mail, but he took a whole bunch of foot creams and never paid for them either. I physically and emotionally write off these cases by saying to myself that I took an oath to be a healer and that money is secondary.

What got me really angry was when that patient who never paid for the visit nor the foot creams popped up in my office a month later demanding that I give him a copy of the x-rays so that he can go to another doctor. Legally, I had to give him his records and x-rays. I suspect he probably travels from doctor to doctor doing the same thing.

Elliot Udell, DPM, Hicksville, NY

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Charles Morelli, DPM

With total respect for Dr. Borreggine, I am surprised that question is still being debated. With the exception of those who only take cash and do not contract with any insurers, the business of medicine and the times have certainly changed from what we all were accustomed to. They changed years ago.

Many will give examples of going to the store and asking to only pay 80% for their groceries, while others will comment on shopping at the mall and asking the lady (sorry "person") at checkout if they can pay when they come back to shop again in 2-3 months or they will suggest...

Editor's note: Dr. Morelli's extended-length letter can be read here.

RESPONSES/COMMENTS (NEWS STORIES)
RE: APMA Celebrates Victory on E/M Codes!
From: Wenjay Sung, DPM

Congrats to APMA and our advocates! I just learned from the APMA news blast that our advocacy groups have won a battle against CMS. Although it’s a politically ambivalent time for our profession against an administration that doesn’t support our profession or our patients, APMA and its leadership has stewarded the way for us through this dark time. I’m happy to electively pay my national, state, and local society dues for these action and results. Keep fighting the good fight!
