FL Podiatrist Faces Drug Charges for Huffing While Driving
St. Johns County deputies said a local doctor crashed his car Thursday night while allegedly huffing (inhaling a chemical substance) and driving for the third time this week.The third crash happened near U.S. 1 and International Golf Parkway just hours after Dr. James S. Militello was released from jail for driving under the influence in another crash. Deputies were working late Thursday night to get an arrest warrant for Militello. After Thursday night’s crash, Militello was found outside his car. Deputies said was it was a single car crash and the 61-year-old was huffing a can of electronics cleaner. Investigators said he threw the can into the woods to hide it, but deputies quickly found it.

CMS Issues ICD-10 Progress Report
For the first time since the ICD-10 compliance deadline went into effect Oct. 1, the Centers for Medicare and Medicaid Services on Thursday released data indicating that the code transition is going smoothly for those providers that have submitted claims to CMS. The agency made public metrics detailing Medicare fee-for-service claims from Oct. 1-27. Over the nearly four-week period, CMS reported that 2 percent of total claims submitted were rejected due to incomplete or invalid information—the same rejection rate according to the historical baseline.
In addition, CMS revealed that 0.09 percent of total claims submitted during that timeframe were rejected due to invalid ICD-10 codes, compared to 0.17 percent of total claims rejected based on CMS end-to-end ICD-10 testing conducted last year. The agency also disclosed that 0.11 percent of total claims submitted were rejected due to invalid ICD-9 codes versus 0.17 percent of total claims rejected based on end-to-end testing.

Query: Acrosyringeal Metaplasia (Acrosyringeal Nevus)
My patient is a 36 year-old, bed-confined female with advanced multiple sclerosis. The lesion covers a significant area of the plantar surface of her right foot and has been present for more than one year. Serum exudates ooze from the lesion creating a crust. The crust was removed and three punch biopsies were obtained. The pathology report states: Acrosyringeal metaplasia (acrosyringeal nevus). No microscopic images were included.

The literature on this diagnosis is sparse at best. How would one treat this, keeping in mind that the patient is unable to leave her residence except by medical transport?

RE: The Passing of Harry Edward Confer, DPM
Dr. Harry E. Confer, 68, peacefully left this Earth on Saturday, Oct. 24, 2015, in the comfort of his home in Irvine, Calif., after a courageous battle with leukemia. Harry pursued his doctorate degree in podiatry from the Ohio College of Podiatric Medicine.

Upon graduation in 1977, he moved to Southern California, where he set up several private offices, passionately practicing until June of this year. Harry will be remembered fondly as a compassionate doctor, loving husband, proud father, caring brother, and selfless individual as he was all this and more.

Do a 1st MPJ fusion with a possible bone graft to maintain the ray length.

Remove the screw, re-establish the length of the 1st metatarsal, and fuse the joint. If you cannot re-establish length, take a bone graft from the dorsum of the calcaneus as an interposition graft. Take freeze dried bone and place it in the graft donor site.

Tell the patient her correction will be performed in 2 stages:
1. A distal metatarsal osteotomy to attempt to bring the head over in a more rectus postion. You may have to resect the bone graft to obtain the maximal result.
2. After surgery #1 is healed, I would fuse the first MTP.

This does indeed seem absurd, especially if they are paying you for the taking and interpretation of the original x-ray. You should check with your provider representative.
Here's another slant on this: The radiologist who takes the x-ray has minimal if any clinical information on this patient. If based on your studies, will the carrier pay the radiologist for reading your studies when they've already paid you? How can the radiologist be the watchdog or...

Recently, I sent a letter to our clients regarding a Novitas Local Coverage Determinant affecting the States of NJ, PA, MD. DC, DE, TX, CO, OK, AR, LA, and MS. Because there was some confusion regarding the missive, today I sent out an additional letter to clarify, and to provide the most current LCD number (the previous LCD was recently retired to accommodate for ICD10; however, the content was otherwise unchanged). The updated LCD number is #L35013.
Regarding the LCD contents, podiatric clinicians in Novitas jurisdictions should be aware that no lab testing is required prior to debridement (11720 / 11721) while patients are being treated for fungal nail infections, as long as the features of onychomycosis are present. Testing is only required when patients are not candidates for treatment, when treatment is unsuccessful (thereby requiring continued post-therapy debridements), or when debridements will be frequent and occur over a long duration (12 in a 24 month period).
A final point that should be made is that this LCD currently does not affect jurisdictions outside of the above listed states. Over the next year, however, it is exceedingly likely that it will be adopted by the other Medicare administrators. For this reason, physicians should check with their state’s administrator routinely.
