MI Podiatrist Discusses Tarsal Tunnel Syndrome
According to Conway McLean,, DPM, "Tarsal tunnel syndrome, like its counterpart carpal tunnel syndrome, is classified as an entrapment neuropathy. Naturally, there are many causes for tarsal tunnel syndrome, from a biomechanical one to various tumors or masses developing within or around the tarsal tunnel. One of the more common structural reasons is a ganglionic cyst, which is an outgrowth of a tendon sheath or a joint capsule. These are benign growths but will cause problems if the cyst impacts some neighboring structure, like the nearby nerve."

"Treatments vary, obviously depending on why patients have nerve compression in this tunnel. Typically, these involve removing or resolving the pressure to the nerve. This may involve injections for a ganglion, or surgical removal in the case of some type of mass or tumor. Because of the benefits of this sports taping, improving one's biomechanics can often resolve one's symptoms. But this is often not a simple task. This may require some type of stretching program, but most beneficial is the use of prescription, customized orthotics that are placed inside one's shoes," says Dr. McLean.

NY Podiatrist Honored by NYPD’s Shomrim Society
Raphael Lilker, DPM was recently awarded the Maimonides Award from the New York City Police Department’s (NYPD) fraternal society of Jewish police officers, the Shomrim Society. This award was named for Moses Ben Maimon, the 12th Century physician, Jewish scholar, and philosopher whose contributions to health and wellness are still felt today. This award is given to those who have significantly improved people’s well-being and health and are evaluated based on their impact in the NYPD and society. Dr. Lilker is an honorary police surgeon who has continuously provided the highest quality of care to NYPD Officers, NY State Troopers, as well as local fire and EMS personnel.

Dr. Lilker continues to serve as a senior member of Hatzalah, a non-profit emergency medical response organization that responds to local public need. Donlon and Dr. Lilker at the event were Mr. Joel Sturm, Chief Operating Officer and Vice President of the New York College of Podiatric Medicine, and Mr. Jason Krawitz, Attorney for the Legal Bureau, Civil Enforcement Unit of the NYPD.

1. Will your strategy beat the market? - Each year, perform a thorough analysis to get a clear picture of how the competing forces in your specialty are likely to play out. To grow at an accelerated pace, you will need a meaningful point of difference that can’t be easily copied or nullified. Mindlessly copying another practice’s business model is not a strategy — it is a recipe for mediocrity.

Query: Continuing Skin Substitute Therapy
I was treating a Medicare Advantage patient's venous wound with skin substitutes. During this time period, she broke her wrist and was admitted to a rehab facility. Now that she is being discharged, she would like to resume treatment, as there was much improvement. The concern I have is that she has since changed to regular Medicare AND it is outside of the 12-week treatment period. Is there any way to continue treating her? Does the change of insurance matter?

A recent article which appeared in Medscape, which has now been picked up by many local papers, suggests that as we age, our balance ability in particular our ability to unilaterally balance, deteriorates, placing us at increased risk for falling.
By measuring how long one can stand on one leg (with eyes open or closed), the researchers found some interesting findings. This is another call for podiatrists to be on the forefront of biomechanics and suggests that waiting until patients fall to assess for fall risk is too little, too late. Examining patients for unilateral balance should be done prior to patients reaching their 60th birthday, suggests the author.

Dr. Khoury, I'd be happy to share my experiences. Like Dr. Vargas, I co-own a virtual assistant company which offers virtual front and back office support specifically tailored to the needs of medical practices. Our VAs, primarily based in the Philippines, are college-educated, and many are nurses or have advanced educational backgrounds, which brings an added layer of understanding to medical administrative tasks.

Our VAs are trained in handling phone calls, scheduling, insurance verification, prescribing, and prior authorizations. Everything is related to the perceived value brought to the table. In their minds, a degree which is universally recognized by the public, VC firms, or other businesses, the MD (DO) can author research on the entire body. The DPM has limited scope which can be covered by orthopedic research, at least in their minds.
Perhaps submit research that is blind as to the authors' names and degrees. It would not be for actual publication but to have it read by someone who is curious. If of high enough quality and the publication inquired, then it would be revealed that the author(s) were DPMs. I may be way off base, I'd appreciate thoughts on this idea.

I have to partially disagree with Dr. Tomczak. There is no question that the MD degree may get doors open that another doctorate degree may not. I also know at least 2 MDs who either decided not to pursue residency programs or who quit their surgical residency in midstream to pursue other areas of interest, such as medical IT, etc. This area is growing by the day as physicians with clinical experience and those in medical school see how difficult clinical practice is.
However I know many DPMs who, for one reason or another, pursued work outside the clinical arena in the pharmaceutical industry, orthopedic equipment or orthotics and prosthetics industries, and some even the teaching profession, at one level or another. Other DPMs are working for insurance companies as investigators, others as...

RE: Time to Unionize

Have you ever been told by a Medicare Advantage plan that their panel is full? It's time for all podiatrists to unionize. One does not have to look far to see that we are being discriminated against, by not allowing those that want to participate in the Medicare Advantage plans. How do you think Medicare Advantage plans are funded? The Center for Medicare and Medicaid Services pays private plans on a per capita basis. In 2023, CMS paid the Medicare Advantage plans 454 billion dollars, amounting to more than 1/2 of Medicare spending last year.
If the federal government is utilizing taxpayers’ money to subsidize Medicare Advantage plans, then it should allow all licensed doctors (including podiatrists) to be allowed to treat Medicare Advantage patients without being told that the panel is filled or the certification status is questionable. The use of federally funded money (taxpayer funded) dictates that anyone who wishes to participate should be able to do so!
With unionization comes strength, and allows legal representation in a class action lawsuit against all the Medicare Advantage plans that prevents any podiatrist the ability to get on these plans using taxpayers’ money… thus, preventing future earnings potential. Now is the time to step up and take action. Unionize now and stop the HMOs from taking advantage of you!
