SC Podiatrist Joins Podiatry Associates of Bluffton
 
Podiatry Associates of Bluffton has added a new physician to its staff. Derek Barker, DPM is seeing patients at the practice’s Bluffton and Beaufort locations. He comes from Michigan, where he practiced with Metro Health for the past six years. He was also a clinical assistant professor at Michigan State University College of Osteopathic Medicine.

NJ Podiatrist Seeks Seat on Council in Freehold Borough
 
Two Republicans are seeking two three-year terms on the Borough Council in Freehold Borough in the Nov. 5 election, Lori Annetta and Michael Lichardi, DPM. Making his second bid for council after running in 2008, Dr. Lichardi is a current member of the Freehold Borough K-8 School District Board of Education.

Professionally, Lichardi is a doctor of podiatric medicine and a physician associate with more than 30 years’ experience. “I have completed a fellowship program with the New York State Department of Health,” he said. “This program provided training and experience in governmental and political issues of medically disenfranchised and special needs populations. My volunteer and professional experience will bring forth a relevant perspective on borough issues.”

5 Marketing Steps Your Medical Practice Should Be Taking - Part 1
 
Marketing is no longer an optional undertaking for physician practices, particularly independent ones. As competition increases and patients take a more active role in practice selection, your viability and growth will increasingly rely on strong marketing initiatives. Relying primarily on referrals from other providers and patients is a losing proposition. Here are the first steps all practices need to take:
 
1. Know your market: Initiate a marketing study to understand how patients, prospective patients, referrers, and prospective referrers view your practice. This market study—which should be conducted by a marketing firm with proven outcomes related to helping other, similar practices grow—should evaluate how aware patients and referrers are of your practice, and what type of reputation it has in the community. The firm should use a variety of approaches to glean this information and provide you with a thorough report, including a survey of individuals in your area that align with your target patient demographics, and interviews with referrers and potential referrers. The findings will help you better understand how the market sees you, and where you need to make improvements.

Query: Post-Operative Care After Amputation
 
What is correct coding for providing post-operative care for an amputation? Are there ICD-10 codes to focus on, or a specific code for the E/M?

Response: Normally the post-op care after an amputation is bandage changes and removal of sutures. The post-op global period for amputation is 90 days. If you took x-rays after the amputation, you could charge for the x-rays (the x-rays would show the part that was amputated gone).
 
If you are speaking of wound care not related to the amputation, you would use the appropriate code for the wound care provided with Modifier -79 which is defined by CPT as “unrelated procedure or service by the same physician during the post-operative period.” It is used in the strictest sense for care that is entirely unrelated to the prior surgery that created the current global period.

I treated a 9 week old baby who had been on antibiotics for 6 weeks for the same type of a problem. The advice I got there, which worked out well, was to take out the impacted segment of the nail only, not to do an avulsion. I did the procedure with Loupes for magnification. With the impacted section removed, the nail grew back normally and the baby went on to a normal nail growth during the time that I followed the child.

In the ongoing discussions about the highly controversial proposal to have two specialty boards in the podiatric medical profession (i.e., podiatric surgery and podiatric medicine), Ira Baum, DPM remarks, “Unfortunately, it is the grassroots podiatrists who will suffer for their inaction. I strongly recommend those in leadership positions to consider these points and begin to explore options and opportunities for change”. 
 
James J DiResta, DPM, MPH further states that, “Many graduating podiatric medical students would benefit from an Intensive Podiatric Medicine Residency option. Providing this option for our graduates who do not want to be surgeons can have real value IF done correctly. That would work if we were able to engage Dr. Levy in this process in creating such a program.” 
 
I suggest that the best way to address these and related matters is to engage in major strategic planning and include leaders in the profession to once and for all determine the direction of this growing, exciting profession. As said in Alice and Wonderland, “If you don’t know where you are going, how will you know when you get there?” Certainly if it was thought I could help, as Dr. DiResta hints, I would be glad to do so at any level.

I am always pleased to see some discussion of gout. I am curious if the readership would care to comment on its cousin CPPD or calcium pyrophosphate disease? To my knowledge, there is no definitive test and the diagnosis is only by symptoms and x-ray.
