PODIATRISTS IN THE NEWS
Sedentary Lifestyle is Literally Killing Us: CA Podiatrist

"Our increasingly sedentary lifestyle is literally killing us," says podiatrist Parminder Jagur, DPM. "Human beings were made to move. We are hardwired to walk and run. If you hold a baby as young as two months old upright with their feet on a flat surface, they will lift one foot and then the other. This involuntary 'walking reflex' is innate in us. Because walking comes so naturally to us, we tend to take it for granted.

Dr. Parminder Jagur

"Studies show that a 30-minute walk offers astounding benefits. Not only does it reduce the risk of coronary heart disease, diabetes, and cancer, but it also improves blood pressure, lowers risk of obesity, builds stronger bones, strengthen muscles, supports joints, slows mental decline, improves sleep, and enhances mental well-being," said Dr. Jagur.

PODIATRISTS IN THE COMMUNITY
MN Podiatry Facility Receives Hospital Advocacy Donation of Chair

Patients at Little Falls Orthopedics may notice new chairs that can make their time at the facility safer and more comfortable. Through funding from the CHI St. Gabriel’s Hospital Advocates organization, the facility has been able to speed up its acquisition of another podiatry chair for foot care.

(L-R) Sarah LeMieur, Dr. Daniel Enderlin, Dr. Kristin Jackson and Advocates’ member Diane Tamm.

The chairs cost $6,000 a piece and are designed to ergonomically hold patients as staff works on their feet, Little Falls Orthopedics Administrator Tracy VanHercke said.

IN THE COURTS
FL Podiatrist Settles Injury Dispute with Drilling Company

An underground utility firm contracted by the City of Vero Beach for work along A1A has reached an out-of-court settlement with one of two island residents who filed separate lawsuits claiming they were injured due to the company’s negligence. Prominent podiatrist Keith J. Kalish, 57, and Coastal Drilling and Backhoe, Inc. of Jensen Beach, reached an agreement on Sept. 25, according to Indian River County court records. The case was scheduled for trial Oct. 16. No details of the settlement were disclosed.

Kalish filed the suit after allegedly tripping over a coiled steel cable while jogging past Coastal Drilling’s workspace near the 900 block of A1A. During his pre-trial statement, Kalish said that he sustained serious and permanent injury to his right leg during the fall which occurred on Sept. 14, 2016. He contended that Coastal Drilling negligently maintained the worksite by placing the coiled cable partially in the sidewalk.

PRACTICE MANAGEMENT TIP OF THE DAY
Back Office Billing Strategies - Part 4

4.  Follow up on accounts receivable daily. Run insurance aging reports and review anything more than 60 days old. You may have to call the payer in some cases, but most claims pay within 21 to 30 days of submission. If this is not the case, you need to investigate what’s going on.

QUERIES (NON-CLINICAL)
Query: Chart Reviews

It seems now that the chart review companies - namely Ciox Health and Change Healthcare which work for Fidelis and Affinity are now getting more aggressive and asking for over 100 charts now at a time or more (in some cases over 200). When I told Ciox that I charge 25 dollars per chart, they said they would tell Fidelis this and they would either lessen the number of charts asked from me which was 115 or pay for the charts. Fidelis called my office and stated, "we don't negotiate and we don't pay [for charts]." It seems that as practitioners, we are at their mercy with no support no defense. Any constructive thoughts would be appreciated. I have left 10 messages and emails with my provider relations rep. with Fidelis and have received no reply.

RESPONSES/COMMENTS (MACRA)
RE: MIPS Participation
From: Michaele A Crawford, DPM

I have been collecting data for participation in the MIPS program for 2018. I have recently found that I have been exempted for reporting for 2018. The majority of my eligible patients have switched to the Medicare advantage plans. I received a small bonus of 1.29% for 2017 reporting. This bonus will amount to an estimate of less than an $800 increase to my bottom line for 2019 since it does not apply to the advantage plans. It would certainly ease the load on my office to not record this data anymore, as well as free up treatment room time.

I am willing to sacrifice $800. My concern is that if I do not report for 2018, and discontinue collecting the data, what is coming down the road in changes to this system? Will I just end up going back to reporting in 2019 or 2020? It would be aggravating to have to reinstate the protocols and retrain staff. Is there anything else I should be taking into consideration? I would appreciate feedback.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Ty Hussain, DPM

Though the topic is revolving around median income for podiatrists, the question has tangentially moved about paying associates. I have been in practice a while now and have seen both sides of income for associates. Unfortunately, income is based on the area of practice. Larger cities with more competition are invariably different (lower) than areas of much needed practitioners.

You have already “incentivized” this individual with a percentage of “bonus” on top of a “no questions asked” salary. Doing research and perusing numbers and stating what one desires is best left with an answer of “start your own shop and experience what the rigors entail with all you have stated as hard work.”
