Telemedicine is Now a Standard, Not an Option: TX Podiatrist
Houston Methodist, an eight-hospital health system in Texas, was prompted into doubling down on innovative processes to improve the patient experience due to the ongoing COVID-19 pandemic. Nicholas (Shetal) Desai, DPM, chief medical innovation officer at Houston Methodist, tells HealthLeaders that the system adjusted to the circumstances created by the virus and outlines the three-step process of implementing digital health strategies for patients.

Before the pandemic began, telemedicine was "nice to have," according to Dr. Desai, but now "digital virtual care is no longer an option; it is a standard core benefit." He says that telemedicine visits at Houston Methodist rose from a couple of thousand visits per month pre-COVID to 240,000 visits between March and August, including around 140,000 virtual urgent care visits. "People are leveraging virtual [care,] even in the walls of the hospital, to minimize exposure and risk," Desai says. "I think that has changed the entire landscape of how we view digital continuous monitoring and then how you access patients so you can protect them, minimize exposure, and keep medicine moving."

VA Podiatrist Interviews Olympian on Staying Healthy During the Pandemic
Ben Pearl, DPM interviewed Gary Hall Jr. this week for his biweekly Fit Foot U program. The segment included Hall's commentary on overcoming the challenges of diabetes in sports, doping, harassment, and keeping physical and mental health during the COVID-19 pandemic.

Hall noted that he has come across two different types of people regarding fitness during the pandemic. “There are those that say I just finished Netflix or HBO and those that tell him I'm in the best shape of my life,” says Hall. He encouraged a routine for exercise with small goals that are within reach. Dr. Pearl created the format to have a forum for athletes and doctors to discuss issues related to health, fitness, and sports medicine.

David Yeager, DPM, podiatric surgeon, has been providing complimentary foot care to children through the Illinois Elks Children’s Care program for the past 14 years. The Illinois Elks Children’s Care Corporation has been providing services, treatments, and equipment needed to ease a child’s daily burden in coping with a physical problem.

Many cases are very complex and require custom designed braces, wheelchairs, and treatment programs. Sometimes, it may simply be the provision of corrective shoes or physical/occupational therapy services for three to six months.

MA Podiatrist Running for Seat on Board of Selectmen
David Tarr, DPM is among 4 candidates running to fill a vacancy on the Board of Selectmen. Dr. Tarr has a plethora of experience in the field of health and safety. The 12-year resident of Dunstable is a podiatrist with an office in Lowell, who also volunteered with the Dunstable Fire Department as an EMT for over a decade.

Tarr said his experience in public safety has led to him wanting to focus on improving local schools, supporting local police, growing the town’s fire department, and ensuring residents are safe. “Politics isn’t my inherent skill, but helping people and getting that inner reward of knowing that you helped somebody is more important than the power of local politics,” Tarr said.

NC Podiatrist Provides Boot Buying Tips
Look for boots that are slightly higher than ankle height. These pairs promote ankle stability and support and have an advantage in snowy areas where your feet are most likely to get wet. "A style that crosses the ankle can provide extra stability which can be helpful on uneven or slippery surfaces. Knee-high boots may be necessary for anyone trudging through very deep snow," says says Jane Andersen, DPM, a podiatrist based in Chapel Hill, NC, and the chair of the APMA communications committee.

Seek pairs with a low heel instead of one that's completely flat. Winter boots with textured soles and a small heel offer traction and help elevate the foot out of the water and snow. Dr. Andersen recommends boots with a 1/2-inch to one-inch heel. "This can be beneficial for patients with tight muscles and foot issues, such as Achilles tendonitis and plantar fasciitis, by taking some pull off the muscles."

Patients Can Read Your Clinical Notes Starting November 2
Starting November 2, all patients in the United States will have immediate access to clinical notes and thus will be able to read their doctors’ writings, as well as test results and reports from pathology and imaging.The 21st Century Cures Act mandates that patients have fast, electronic access to the following types of notes: consultations, discharge summaries, history, physical examination findings, imaging narratives, laboratory and pathology report narratives, and procedure and progress notes.
An outstanding concern about open notes is that it will cause more work for healthcare professionals. Traditionally, doctors have written notes using medical lexicon, including a lot of abbreviations and jargon for efficiency’s sake. Now that patients will read the notes, will clinicians have to spell out things in lay terms, alter their writing so as not to offend, and generally do more work? William Harvey, MD, chief medical information officer, Tufts Medical Center, Boston, acknowledged that that may be the case.

Medicare Moves to Speed DME to Diabetics, Others
A new proposed rule would provide more therapy choices for Medicare beneficiaries with diabetes and make it easier for durable medical equipment manufacturers to make their products available to beneficiaries, the Trump administration said.
The proposal, announced Tuesday by the Centers for Medicare and Medicaid Services, would streamline the process for Medicare coding, coverage, and payment for durable medical equipment, prosthetics, orthotic devices, and supplies (DMEPOS).

There are many factors to consider in coming up with an effective part-time/reduced modality policy that works with the physician practice. The following are some concepts to consider:
1. The best time to come up with an effective policy is in advance, before anyone is ready to make a switch.
• Under such circumstances, the various financial, scheduling, and related issues can often be addressed in a less emotional and more thoughtful manner.

I want to thank Jeffery Kass, DPM for understanding my intent. A roadmap to permanently increase scope for every podiatrist nationwide:
1. If DOs were able to become equal to MDs, regarding hospital privileges, license, etc., so should podiatrists.
2. My intent is to create a political roadmap for the APMA to forward to each state component. Each state lobbyist hired by a state podiatric medical association should receive a copy of the Pennsylvania emergency resolution. They can lobby for similar or the same emergency scope as Pennsylvania.
3. Once this is achieved on a national level, there should be no reason why this cannot continue after the COVID-19 crisis ends.
4. The ultimate result will be that our profession will no longer be divided into those who cannot and those who can treat the ankle in New York, Connecticut, South Carolina, and other states. After all, if a podiatrist can treat the whole body with a referral from an MD or DO (lawfully delegated by an MD or DO), then the ankle is just another part of the body.
The beauty of this idea is there is no downside I can think of. After all, if Pennsylvania can have podiatrists join as first responders regarding emergency care, and if podiatric residents can do so, then there is no reason during a pandemic that states should prohibit podiatrists lawfully delegated by an MD or DO from doing so.
