RELEVANT RESEARCH
Consensus Recommendations for the Use of Non-invasive Melanoma Detection

A committee of 13 dermato-oncologists participated in an online DELPHI process and developed 11 consensus recommendations regarding total body photography (TBP), serial digital dermoscopic imaging (SDDI), and reflectance confocal microscopy for use in detection of melanoma. A total of 91% of panel members strongly agreed that TBP be used in patients with dysplastic nevus syndrome and 64% of panel members strongly agreed that TBP is generally not recommended in children due to volatility of nevi and low risk of melanoma. There was less consensus regarding the use of SDDI alone or in conjunction with TBP. Overall, 80% strongly agreed that, when available, reflectance confocal microscopy is recommended for evaluation of dermoscopically equivocal pigmented lesions in cosmetically sensitive areas or in areas prone to poor wound healing.

Non-invasive techniques may facilitate melanoma detection and can spare patients unnecessary biopsies. There is no replacement for physician judgment, but these consensus recommendations may assist clinicians in appropriately implementing these tools in practice.

PRACTICE MANAGEMENT TIP OF THE DAY
7 Conveniences Patients Expect from Today's Medical Practices - Part 6

Today’s patients expect the same conveniences from medical practices that they receive from restaurants or retailers. If they don’t get them, they will find another doctor. If practices want to compete in today’s healthcare environment, they need to offer patients convenience. Here are the top 7 things patients expect from their doctor’s office.

6. Transparent pricing - Patients expect guidance on how much services will cost, what will be covered by their insurance, and what will not. If a referral is made, the patient should be informed whether it will be in network or out-of-network. Any bill sent by the office (or better yet, presented online), should be easy to read and understand.

CODINGLINE CORNER
Query: DME Authorization as a Part of Coordination of Care

Obtaining authorization for DME and prescriptions has become increasingly time-consuming. Can these processes be considered part of the coordination of care in the calculation of the E/M level?

Brad Toll, DPM, Crofton, MD

Response: The proper level of E&M coding is comprised of 3 components: History, Exam and Medical Decision-Making. Obtaining authorization for DME and prescriptions is not part of these 3 components. If you are considering the coordination of care as part of your medical decision-making, I still do not see how this falls under part of your E&M coding and would not be proper including it in the calculation of what level coding.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Pennies Add Up (George Jacobson, DPM)
From: Steven Finer, DPM

This scenario happened to me. I couldn’t believe that we were getting these checks and had to process through the credit card company. I actually threw some away.

Check for $.01

However, I kept this .01 cent check and proudly display it.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: Jack Ressler, DPM

Although business interruption insurance is definitely important to have, everyone should look into a utility interruption policy. The following was a scenario that proved very beneficial to my practice. A number of years ago, my insurance agent called me to tell me I should purchase an inexpensive policy in addition to my business interruption insurance. Of course, I agreed, not giving it too much thought. The next year, hurricane Wilma hit South Florida. My office dwelling was not damaged, but we were without electricity for two weeks. Under this scenario, my business interruption policy did not cover anything.

My utility interruption policy covered both my overhead expenses and loss of income. Remember, you are committed to pay your staff if this type of situation occurs. This was my first and only paid vacation ever!

YOU CAN'T MAKE THESE THINGS UP
The Happy Foot/Sad Foot Sign is Saved

The crane was poised to destroy a piece of Silverlake history. Bill Wyatt, sitting beneath it, was determined to not let that happen. Wyatt had ventured out to the sign—a spinning one, depicting the only relatable emotional states, joy and agony—in order to save it from landing in the massive dustheap of former L.A. landmarks (alongside the Brown Derby’s giant hat.) Alas, the sign did come down that day—but it went straight into Wyatt’s U-Haul.

Happy Foot/Sad Foot Sign (photo: Steven Blum)

It was a fortuitous event for The Happy Foot/Sad Foot sign, an ever-twirling source of local superstition. But it was close to getting the chop after its podiatrist owner, Dr. Thomas Lim, announced plans to relocate his office to Rampart Village. After hearing that the sign would be destroyed, Wyatt reached out to Lim about putting the feet up permanently above his graphic tees.
