PODIATRISTS IN THE COMMUNITY
MI Podiatrist to Participate in Rotarian Pancake Breakfast

This year’s program, “No Place for Hate: An Anti- Bullying Campaign,” supports the youth in Berkley schools. The anti-bulling campaign began in the fall of 2012. Since that time, the program has educated the community on the negative effects of bullying, and the importance of stopping it, through guest speakers at schools and community events.

Dr. Lee Hoffman

Proceeds from this year’s event will go toward supporting the campaign. Dr. Lee Hoffman, a podiatrist at NorthPointe Foot & Ankle in Berkley, has been an active member of the Rotary Club of Berkley for more 30 years. As part of the 61st Annual Pancake Breastfast, he will be serving pancakes that morning along with his fellow Rotarians.

QUERIES (CLINICAL)
Query: Unknown Skin Lesion

This skin lesion began two weeks ago about the area of a tarsal tunnel release performed about one year ago. We see this patient bi-weekly for another problem. One week ago, the lesion was one half the diameter. It is pruritic. The patient is a 40 year old female Iraq war veteran, home from the war five years.

Unknown Skin Lesion

The lesion has not responded to Lotrisone creme Rx'd two weeks ago at its first appearance. Any help or advice would be greatly appreciated.

RESPONSES/COMMENTS (CLINICAL) - PART 1
From: Dave Mullens, DPM, JD

Last year, a client in my law practice had reimbursement issues regarding the use of PRP. As a result, I had to perform an extensive review of PRP. Much to my surprise, despite thousands of anecdotal reports and “articles”, there wasn’t one double-blind study published in a peer-reviewed journal on the subject of PRP. Why not?

The insurance companies are successfully arguing that the doctors (mostly podiatric and orthopedic surgeons) use PRP without a scintilla of hard data supporting its use - just because the PRP is a money-maker. Although PRP may be a wonderful adjunctive treatment, we are living in the time of evidence-based medicine. The university hospital-affiliated podiatric surgeons reading this can and should do the sophisticated study that will withstand academic scrutiny and will tell, one way or the other, whether PRP is effective.

RESPONSES/COMMENTS (CLINICAL) - PART 2
From: Estelle Albright, DPM, Simon Young, DPM

The foot looks like impetignized psoriasis. Try treating for impetigo.

Estelle Albright, DPM, Indianapolis, IN, estellealbright@hotmail.com

If this condition is confirmed by biopsy to be psoriasis, it should not be treated by podiatrists without a dermatology background. This should be seen by a dermatologist or rheumatologist specializing in psoriasis. You need to rule out psoriatic DJD. Oftentimes, meds such as methotrexate, and TNF blockers such as Enbrel and Humera are needed.

RESPONSES/COMMENTS (NON-CLINICAL)
From: James E. Rogers, DPM

The problem that I have seen with many practices is the hiring of "office managers".  As a solo practitioner, I have three main employees - receptionist, chair side assistant, and billing clerk (and part-time help as needed). I AM THE OFFICE MANAGER. I open all mail to see payment patterns of insurance carriers, list and make all deposits, pay my bills, etc. Sure, it takes a little more time, but it equals a tight ship. We have a checks and balances system that works well. Embezzlement? Not in my practice.

RESPONSES/COMMENTS (OBITUARIES) - PART 1A

I graduated from PCPM in 1975. As entering students, our vision of our future profession was very limited. By the time my class began our clinical rotations, two young energetic new teachers arrived. They were both residency trained, approachable, intelligent, well read, and talented. Most importantly, their goal was to teach, and that they did well. They brought something new back east - a new way of looking at our profession and a new way of evaluating foot problems. More importantly, they acted as a spark that stimulated many of us to become energized.

Their names were Drs. Guido LaPorta and Tom Melillo. About three years ago, I saw Tom at an annual APMA meeting. He remembered me as if it was just yesterday. I am just one of the many products of his teaching. I thank these individuals for their energy in leading our profession to the place it is reaching. The future of our profession is limitless thanks to teachers like Thomas Melillo. I'm very thankful for his presence in my life, but so saddened by his loss.

RESPONSES/COMMENTS (OBITUARIES) - PART 1B

I was shocked and saddened to learn of the untimely passing of Tom Melillo. While many remember Tom as the perennial president of the Ohio College, I would like to go a bit further back in time. Those of you who might be some of the more gray hair graduates of PCPM (now Temple) will remember Tom as one of the leaders of the Surgery Department at the school. I was very honored to have served as one of Tom’s residents. I would like to remind everyone of an article that Drs. Melillo, LaPorta, and Olinsky authored in JAPMA in 1974 that detailed all the x-ray criteria for decision-making in hallux valgus surgery. This landmark article formed the basis for much of our thought processes in bunion surgery. Tom will be sorely missed. My deepest condolences to Judy and his family.
