IL Podiatrist Discusses Prevention and Treatment of Diabetic Foot Wounds
Diabetes can impact the body in a number of ways, especially creating potentially serious complications for your feet. “Foot care is a central component of overall diabetes care,” says Bryce Paschold, DPM, a board-certified foot and ankle surgeon and a fellow member of the American College of Foot and Ankle Surgeons. “Without precautions, even small foot problems can lead to amputation or be life-threatening.”

Visit a foot and ankle surgeon to determine if you have lost any feeling or circulation. Periodic foot exams can also help prevent complications. “Advanced therapies for foot wounds, such as the use of bio-engineered skin substitutes and negative pressure wound therapy, are saving limbs and restoring mobility for people who suffer from non-healing foot ulcers,” says Dr. Paschold.

NY Podiatrist Advises How to Buy a Running Shoe With Arch Support
Bruce Pinker, DPM says that to determine whether or not someone needs arch support, one should perform "The Wet Test," which involves stepping on a piece of paper with wet feet and then evaluating the imprint. If there is a skinny question mark-like imprint, this indicates a high arch, while an oval-like imprint is indicative of a low arched foot. An in-between imprint would be a normal arch.

Dr. Pinker adds that running shoes should be snug in the width and arch, not tight, and there should be 3/8 of an inch from the end of the longest toe to the end of the shoe. He notes the best insole materials, or orthotics, are usually a thermoplastic for sneakers. The midsole of running shoes is often made of ethylene vinyl acetate (EVA), while the toe box of running shoes is usually composed of synthetic mesh.

PA Podiatrist Interviewed About 3-D Orthotic Printing
Ben Pearl, DPM interviewed Bryan Kentner, DPM on technical aspects of 3-D printing orthotics. Dr. Kentner has traveled to Asia for advanced training in 3-D printing. He established his own lab, based in Philadelphia. He reviewed the benefits of printing. Kentner highlighted being able to modify the print contours for quick adjustments with an office-based 3-D printer and the short waiting time to produce the orthotics.

Kentner reviewed the technical aspects of maintenance with the printhead and troubleshooting. He believes that printing will become more advanced to allow more features to be incorporated into the orthotics. Other orthopedic appliances will be developed in the future such as AFOs.

4 Ways Streamlining Front Office Operations Can Drive Revenue For Your Practice
1. Let patients self-schedule online.
Self-scheduling is an excellent option for patients and front office staff alike. This type of online appointment booking streamlines workflow by reducing the number of phone calls front office staff receives daily, enabling them to focus on revenue-generating tasks. In addition, by automating your appointment scheduling process, mistakes that often transpire when patients book appointments by phone can be greatly reduced.
Practices can add a link to their appointment calendar on their Google My Business page or their website, allowing new and existing patients increased accessibility to schedule appointments directly from a quick online search. Considering sixty percent of a doctor’s appointments are booked outside of office hours, enabling this capability can also increase patient acquisition.

RE: New ICD-10 Codes
1) I feel as though there should be a separate ICD-10 code for a plantar fibroma. We all have been using M72.2 for plantar fasciitis but I suggest there should be a separate code for a fibroma. I am sure there are some of us that will use a benign tumor code but that is not reflective of the true diagnosis.
2) There should also be a code for capsulitis. Of the ICD-10 codes, the one that I use most often is M77.41/M77.42 for metatarsalgia in that any of the codes for capsulitis imply an enthesiopathy, M77.51/M77.52. These latter codes are also not reflective of the true diagnosis. I have also considered other codes but these more reflect an injury that would require a -A/D/S that also is not reflective of the pathology that I am seeing.

Response: I absolutely agree with Dr. Hoffman. There should be a separate ICD-10-CM code for a plantar fibroma. It appears that M72.2 is a “catch all” phrase, i.e. plantar fascial fibromatosis/plantar fasciitis. Since a plantar fibroma is a benign neoplasm of connective tissue, you would assume that it would be classified in the ICD-10-CM code set D21.2_, Benign neoplasm of connective and other soft tissue of lower limb, including hip. While we are on the topic of ICD-10-CM code M72.2, ICD-10 has been in existence for 7 years. The whole idea of ICD-10 was specificity. It makes absolutely no sense that despite the fact that plantar fasciitis can occur in either foot, M72.2 does not demonstrate laterality.
As far as capsulitis is concerned, once again I agree with Dr. Hoffman. When you look up capsulitis in the ICD-10-CM manual, it directs you to enthesopathy. What does enthesopathy mean? The place where a tendon or ligament meets your bone is called an enthesis. Enthesopathy is a “catch all” term for conditions that affect these connection points. Enthesitis is when they get inflamed and become painful because of injury, overuse, or disease. There is an ICD-10-CM code for bursitis. Wouldn’t you expect there to be a more specific ICD-10-CM code for capsulitis? I do. It is a common condition that we encounter in the foot.

It is with great disappointment that I read the final selection for nominees for the ACFAS board that the nominating committee put forth. Women in medicine have made great strides since my early days in residency. I was questioned in a residency interview regarding my ability to be a doctor since I would likely have babies and stop working. In my final week of residency, one of my residency directors asked to see me in his office. His topic of conversation was to explain to me why he didn’t think women should/could be doctors.
I realized early in my professional career that women needed to have a seat at the table. I began my career in academics, but transitioned into private practice later on. It is with great pride that I observe the amazing, talented women who have taken leadership positions in podiatry. There are a plethora of talented women who could greatly contribute to this board.
Women deserve to have a seat at the table and a seat on the board of ACFAS. We are starting to see great awareness of the benefit of board diversity in the C-Suite world. Studies have been done that show the benefit of this diversity in the business world. This is about more than bridging the gap in gender equity. Research shows that women have more desired leadership qualities than men, perform higher than their male counterparts in resilience, self-development, honesty, and integrity. I promise you ACFAS would be better off, as well. This takes making conscious choices. We can and need to do better.

I worked with H. Coleman Scheuller many times while I was on active duty in the Air Force. “H” stood for Henry and no one ever called him Henry. Coleman spent most of his Air Force career in the reserves. He was known to fill in for active-duty personnel quite often. Several times he went to the Pentagon Clinic for months at a time. While he was at the Pentagon, he would bring his surgical patients to Andrews Air Force Base where I was stationed. On many occasions, I would assist him. It was always good to work with him and see his techniques. One of the downsides of being in the military is that you rarely had another podiatrist to work with to get exposure to other ways of doing things.
After surgery, we sometimes went to his favorite restaurant “Golden Corral.” It was always amazing to see a 6-foot-tall person who likely weighed 170 pounds keep up with the 400-pound regulars at Golden Corral. I never got my money’s worth but he sure did.
Coleman also did a six-week assignment at Kessler Air Force Base in Biloxi Mississippi. I followed him for a second six-week assignment after he left when Kessler was without a podiatrist. One of the differences between the reserves and being active duty was that he volunteered to go there while I was ordered to go there. Anyway, it was my pleasure to go to Kessler after him, knowing I would not be getting any surprises. I always enjoyed my time working with Coleman. I understand the loss his family and friends must feel.

It is always very sad to read news such as this about anyone, let alone someone in our profession. Let's hope the doctor gets the help he needs. Seeing these stories brings up the question again as to why such negative news should be posted on this forum. Though this type of story is very sad and invasive in one's personal tribulations, it is articles like these that can protect us all against possible false representations this person can portray. Case in point; when I sold my main practice back in 2016, a fellow podiatrist expressed great interest in purchasing my practice. He reached out to me after seeing my classified ad in the practice for sale section I posted in PM News.
He wasn't just making a casual inquiry, he was interested in making the purchase in an expedited way. His terms and timetable for purchase raised a red flag in my mind. After my conversation with this podiatrist, I did some research by way of Google. It didn't take long for me to find his name being associated with an investigation he was part of concerning his office staff. He was later convicted of this crime. I believe the outcome was posted on this forum. Had it not been for public information posted about this perspective buyer, the sale of my practice could have put my staff and patients in a potentially dangerous position.
