Scholl College Students Shine at University’s Research Poster Session
 
Twelve Dr. William M. Scholl College of Podiatric Medicine students presented their work at a university-wide event on Wednesday October 16. This was the fifth year that Scholl College students have received support for research training by an NIH T35 Training Grant.  Each student spent eight weeks this past summer with one or more mentors working full-time on research endeavors. Projects varied greatly among students and included foci in basic sciences, biomechanics, medicine, radiology, and surgery.

Scholl faculty member and research mentor Ryan Crews, MS, CCRP commented, “The caliber of research being presented from students throughout the university was exceptional, so it was particularly gratifying to receive numerous compliments on the work of the Scholl students from faculty in other programs.”

VA Podiatrist Provides Sensible Advice About Fashionable Footwear
 
Some women choose fashion over sensibility and eventually end up in the office of a podiatrist. "I tell people who have foot problems, 'Put your tennis shoe or your sandal on while you're getting ready in the morning and maybe even when you're driving to church,'" Jennifer Purvis, DPM,  with the Landmark Foot and Ankle Center, said. "And then change into your shoe right before you walk in and if you have to run errands afterward, you put that sensible shoe back on."

"The foot and ankle is a very complex structure. It is made up of more than 26 bones and ligaments," Dr. Purvis explained. "It's not normal to have any foot pain whether you're a child or an adult. It's a warning sign that there's something wrong in the body."

CT Podiatrist Sentenced To 30 Months For Defrauding Medicare
 
Deirdre M. Daly, Acting United States Attorney for the District of Connecticut, announced that Dr. Richard Sokoloff, 70, of Guilford, was sentenced today by U.S. District Judge Janet Bond Arterton in New Haven to 30 months of imprisonment, followed by 18 months of supervised release, for defrauding Medicare. Sokoloff commonly provided only “routine foot care” services to his Medicare patients, such as simply trimming or clipping their toenails, yet he regularly submitted claims to Medicare as if he had performed nail avulsion surgical procedures. Sokoloff was ordered to pay $213,676 in restitution to Medicare, which includes fraudulent claims dating back to 2008.

This looks like palmo-plantar pustulosis/pustular psoriasis. First of all, for completeness, biopsy a fresh pustule that also includes adjacent non-pustular skin. Even high potency steroid topicals don't work very well unless used religiously under occlusion, and even then, not great because even compliant patients are usually only able to follow that regimen at night.
 
I have used Cordran tape cut and applied to the affected area and topped with a low-Dye type top cover to keep it in place. If kept dry, it can be left on for several days, or if the patient prefers, can be changed daily. I say they can leave it on for several days to save patients money. As far as I know, Cordran is still name-brand only and can be quite expensive depending on insurance coverage. You have to tell the patient upfront that they may not see results for 30 - 60 days, but to not lose faith. Once under control, it will be a management situation where high potency topicals under occlusion at night may be enough, or several days of Cordran followed by several days off for a steroid holiday may produce satisfactory management of this recalcitrant condition.

I have experienced this involving an employee of 15 years. It was devastating (mostly emotionally). This person did not start out stealing. My question is: What action was actually taken against the perpetrator? In my case, and in others that I have knowledge about, it seems like the thief rarely receives consequences of any significant degree. I know that I have never had someone call as a employment reference query and ask about the 15 years of employment this person had with me. To my knowledge, there is no “reporting agency” like the National Physician Data Bank that could be used to check backgrounds.  Basically, if someone is dishonest and is going to steal, they will most likely figure a way around your system. There is no perfect answer. Stay vigilant, run occasional surprise audits, and change things up now and then.

Dr Melillo was a true gentleman. He always had a great smile and calming sense about him. He loved OCPM, and the students were better for that. We will all miss him.

Dr. Thomas V. Milillo will be sadly missed by the entire profession. His early years in podiatry were spent in Pennsylvania as an associate in practice with Dr. William L. Goldfarb while also teaching in the surgery department at PCPM. He was also active at that time politically, and became president of Pennsylvania Podiatric Medical Association. Shortly after his term as president ended, he moved to California, and subsequently to Ohio. My heartfelt condolences to Judy and the family.

Dr Melillo was a faculty member at  PCPM and my instructor. He was a very skilled, but modest man. I spoke with him at the APMA National in Wash DC last year. We had some nice laughs. He did remember me and my class. I have crossed paths with him several times over the years. Our profession and this world has lost a truly nice guy.

I was saddened to read of the passing of Thomas Melillo, DPM. I got to know Tom well over the many years we worked at OCPM together. I could say many positive things about his leadership role in podiatric medical education and the college, but the quality I admired the most was his unspoken devotion to his family. I respectfully disagree with you Dr. Udell. If you are ordering the blood work, then you should treat based on the results with respect to vitamin D levels. Who says you have to see osteopenic bone? The serum levels are informing you that the blood levels are low. That would imply they are low in the foot, like they are throughout the body. Think of it as a gross examination of a specimen versus a microscopic examination. The blood work is informing you of information that you cannot as of yet see on x-ray.
