OH Podiatrist Re-Running for County Commissioner
 
There’s no one better than a doctor to be in a leadership position during a pandemic, according to Wood County Commissioner Ted Bowlus, DPM, who is running for re-election on Nov. 3 as a Republican. “I do have the background. I have experience as commissioner for the last four years,” said Dr. Bowlus, who has been in practice for 30 years, running a Perrysburg podiatry clinic. “And I care a lot about my constituents. I listen to them and act on their concerns.”

If re-elected, one of his main goals is to start the Focus on the Future program, utilizing leaders in the health department, emergency management agency, and university, Bowlus said. “I have a good background in immunology and microbiology, along with my education,” he said. “I feel that can be a distinct benefit.

Breaking in Boots Should Not be Painful: NY Podiatrist
 
"Most shoes are a bit snug when you first put them on, especially boots. Boots are commonly made with leathers that can be also weather-treated causing them to be a bit tight or stiff at first," says Jacqueline Sutera, DPM. There's really no surefire way to tell whether they will require you to break them in or not. "You can get a pretty good idea by trying them on," says Dr. Sutera.

Putting on your boots and walking around in pain to break them in is not something Dr. Sutera recommends, because of the aforementioned blisters and bunions. "First off, if a shoe is too tight or narrow, strongly consider choosing a different pair to purchase," she advises. "To soften and loosen boots, try using a shoe leather softening product, specially designed for this. Also shoe trees, or having them professionally stretched by a shoemaker, are great ways to spare your feet," she says.

5. Review your contracts.
  • Take a little time at the first of the year to review your contracts with payers.
  • Make sure you are aware of any changes from the prior year, especially if those changes require a change in workflow, process or staff (re)training.
  • Note all filing deadlines for the year ahead.  
  • This is also a good time to make a chart of when your contracts come due, so you can plan ahead for any renegotiations.

Query: Understanding the NCCI Edits
 
Can anyone explain CCI edits? For example: CPT 11719 and G0127. When I enter their codes in the CCI edit check on the APMA Coding Resource Center, there is an indicator of 0. Does that 0 mean that I can code these together in one encounter such as G0127 and CPT 11719? If you code them together, is there no -59 modifier used because both have a 0 indicator?

Response: An indicator “0” means you cannot separate the two codes with a modifier. An indicator “1” would. So G0127, CPT 11721 and CPT 11719 have an allowance for ten nails each so you can never bill these codes together. CPT 11720, on the other hand, has an allowance for only 5 nails, so that is why you can use CPT 11719 or G0127 (for the other five nails) in combination with CPT 11720.

This is in response to Dr. Secord’s October 23 claim that COVID-19 is not more deadly than the flu virus and that masks are worthless. This view is not borne out by science and statistics. As of October 2, 2020, according to the CDD, there have been 299,028 more deaths than would be expected if we compared the death rates to the averages of the last 5 years. 300,000 more deaths? Dr. Secord, what one thing makes this year different from the other 5 years? And the rise did not start until about March or the number would be higher. For a 12-month period, the excess deaths will be well above 400,000 or one in less than every 1,000 people. Most years the number of flu deaths is less than 50,000. Clearly, without successful vaccinations available, Covid-19 is worse than the flu.
 
Yes, the COVID-19 virus is smaller than the holes in our surgical masks but most Covid virus particles ride in on aerosolized droplets that are bigger than the mask holes. The best advice is to wear a mask.

I would just like to congratulate Gary Gordon on his outstanding past 46 years of practice and his considerable achievements in sports medicine. I graduated with Gary in the class of 1973. It was a small class of approximately 45. We all got to know each other very well due to the class size, and thanks to Elaine Carville, DPM and her connection to the Navy Yard officers club, (as she was a retired Marine lieutenant colonel); we had many fun Friday after class times!
 
Gary, you are a real tribute to our profession. I hope your retirement is as exciting and as much fun as your professional career.

In response to the criticism of what was not done regarding a pre-op biopsy and MRI, let me caution the young practitioner that “experience-based” medicine is often better than “evidence-based medicine.” The cost and information gotten in this well demarcated lesion would be of no value as x-rays clearly showed no bone involvement.
 
To disrupt a well circumscribed and encapsulated lesion is unnecessary and can lead to infection and dehiscence. An excision biopsy gives the pathologist the entire lesion and the patient a chance to avoid a disrupting invasion with an incomplete biopsy which is a waste of the patient’s and the surgeon’s time. The additional cost is also a factor for a second operation.
 
Perhaps a tibialis posterior tendinitis exploration prior to surgery is indeed indicated and without it, there may be malpractice, but the human mind and fingers create the judgment call of an experienced surgeon.
