PODIATRISTS AND FOOTWEAR
CA Podiatrist Provides Some Halloween Trick or Treat Footwear Safety Tips

“Trick-or-treating is essentially an exercise in walking, as little witches, wizards, and werewolves tramp from door-to-door in search of both treats and frights,” said Ami Sheth, DPM, President-Elect of the California Podiatric Medical Association (CPMA). “Wearing unsafe and uncomfortable shoes during trick-or-treating is not only dangerous, but sure to make for a short night, and very unhappy little ones. Save the princess slippers, Frankenstein boots, and oversized hairy rubber werewolf feet for playtime at home; instead have your little ones wear comfortable, well-fitting shoes that are already broken in, with good arch and heel support (preferably with non-skid soles) and with strong, solid, straight, and firm lacing.

Dr. Ami Sheth

“Preventing falls while trick-or-treating is also important,” said Dr. Sheth, a podiatric physician and surgeon in private practice in Los Gatos, CA and a mother. “The University of California Davis Medical Center reports that falls are a leading cause of unintentional injuries among children on Halloween. Wearing properly fitting shoes can help reduce falls. Other measures that can help diminish falls during trick-or-treating are to make sure costumes stop at the ankle and are loose enough to walk with ease. Don’t wait until Halloween to have children try on the costumes for the first time.”

INTERNATIONAL PODIATRISTS IN THE NEWS
UK Podiatrists Set to Begin Prescribing Medicine

Prescriptions are set to be issued by physiotherapists and podiatrists for the first time under a new scheme. The medics will be given prescription pads after completing a university course. Under a previous system, physiotherapists and podiatrists who qualified as “supplementary prescribers” had to secure a complicated three-way agreement with the patient and their GP.

Emma Brewin

Emma Brewin, an acute diabetes specialist podiatrist at Edinburgh Royal Infirmary, is among those who have completed the course. She said: “Diabetic foot infections can be limb and life-threatening so they require rapid and aggressive management. My independent prescribing status allows me to prescribe antibiotics without the limitations of a clinical management plan, so patients have timely access to the drugs they require.”

PODIATRISTS AND STUDENT LOANS
Podiatrists Among Doctors on Unpaid Student Loan List

There are 846 doctors and dentists in 43 states who have been named by the U.S. Department of Education on a public list in a desperate effort to get them to repay their defaulted student loans. And because the loans were federally guaranteed, it's taxpayers who are left with the bill. ABC's New York station WABC launched its own investigation and tracked down several doctors on the list, including podiatrists, according to the Department of Education.

Many doctors on the list have paid back thousands of dollars. For a complete list of doctors who have outstanding student loans, click here

HEALTHCARE NEWS
Aussie Doctors Implant Calcaneus Made by 3-D Printer

Doctors used a 3-D printer to help build a Melbourne man a new heel bone and save him from losing his leg to cancer, in a world-first procedure. Len Chandler, 71, is back on his feet after the Melbourne team used scans of his left heel bone to create a 3-D image of his right one. They then used the 3-D version to help construct an exact replica of the bone, the calcaneus, where a tumor had taken over.

Patients with advanced cancer in the calcaneus often lose the leg below the knee, as it is too difficult to replace the highly complex bone, which must move in tandem with the shin and foot bones. But the combined efforts of St. Vincent’s Hospital surgeon Prof. Peter Choong, Melbourne biotech company Anatomics, and the CSIRO to build Chandler a new one are at the forefront of body-part development.

QUERIES (MEDICAL-LEGAL)
Query: Necessity for NPI Number

I would like some input about the legalities of working in a podiatric office setting as an employed licensed podiatrist without having the employer use my NPI number. I am in good standing with Medicare and have a current state license. Would I be allowed to administer general podiatric care and most office procedures such as simple ingrown nail excisions, warts, biopsies, mycotic nail care, orthotic casting, etc.? Does the podiatric employer have to be present in this type of situation seeing as how any  procedures allowable by Medicare or other insurances would be billed under the employer NPI numbers.

Name Withheld

Editor's Comment: PM News does not provide legal advice. When billing Medicare as a group practice, both the NPI number of the group and rendering physician must be provided or the claim may not be paid. If the rendering provider is a licensed provider, the employer need not be present when the services are rendered.

QUERIES (NON-CLINICAL)
Query: Source for Battery Packs for Portable Tourniquets

Does anyone know where to get an equivalent battery pack for a Delfi PTS portable tournaquet? It looks like it should cost about $15-20, similar to the types of battery packs for cordless phones or other devices. The list price through the manufacturer is well over $200.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Kudos to PICA
From: Daniel Chaskin, DPM

PICA is more than a company that just pays for attorneys to defend you. They also answer all your risk management questions. How do I know they care?  I contacted PICA with a question. PICA's president, Ross Taubman, DPM personally called me back. To have the person in charge of an insurance company personally call you back tells me that PICA cares about me the policyholder. Thank you PICA.

RESPONSES/COMMENTS (PODIATRY MANAGEMENT)
From: Connie Lee Bills, DPM, Dennis Shavelson, DPM

I echo Dr. Kirby’s remarks wholeheartedly.

Connie Lee Bills, DPM, Mount Pleasant, MI, staff@familyfootcare.biz

Kevin and I have consensus when he states: “Let's hope podiatry isn't becoming a one-trick-pony show of surgery-only treatments being offered to their patients.” Biomechanics remains a viable Plan B “cut and paste” when and if patients are contraindicated or not ready for a procedure. DPMs who are passionate and dedicated to foot surgery should, in those cases, ally themselves with colleagues who are passionate and dedicated more on external biomedical engineering and consider a cross-consulting relationship.

RESPONSES/COMMENTS (OBITUARIES)
RE: The Passing of Neal Frankel, DPM
From: Jon Purdy, DPM

I met Neal in Chicago in 2001, and he became a very close friend. Neal was the definition of a good, kind, and genuine person. As many have stated, he was a highly accomplished person. As of last year, he had performed 10,000 bunionectomies and read the entire 2,000 plus pages of the Affordable Care Act. He was no doubt a genius. Last week, we had dinner together and I was marveling at all he did in a week. He was the type of person who only required three hours of sleep a night, and was dedicated the rest of the day to our profession when he wasn’t with family and friends. He was always available to answer questions and help, and his help was invaluable. I just can’t believe he’s gone. I will miss you my friend.

YOU CAN'T MAKE THESE THINGS UP
RE: Mrs. Malaprops Visits the Podiatrist

Recently, I asked a patient if she treated her heel calluses with anything. Her reply was that she uses a "pompous stone." It sounds kind of arrogant, if you ask me!
