Plantar Fasciitis Often Mimicks Other Foot Conditions: MD Podiatrist

As if the first steps out of bed in the morning aren't torturous enough already, many people suffer stabbing pains in their feet as they limp their way to the bathroom. That morning symptom is a hallmark of plantar fasciitis, a common source of heel pain, but it could be other things: It could be a stress fracture, a broken bone, a tumor, compression of a nerve or tarsal tunnel syndrome," said Neil Scheffler, president of the Baltimore Podiatry Group. If it is plantar fasciitis, treatment options range from stretching to surgery.

Scheffler said he advises his patients not to go barefoot at all, and to put running shoes on even to walk to the bathroom in the morning. If that doesn't help, patients might wear a night splint to gently stretch their calf and Achilles tendon while they sleep, or custom orthotics to correct any biomechanical problems. Physical therapy also helps to get more blood to the tissue to promote healing.

TUSPM Professor Recommends RIICE for Winter Ankle Sprains & Fractures

Serious wintertime injuries are not problems solely for athletes. “Ankle sprains and ankle fractures are much more prevalent this time of year for everyone. Ice and snow create the impetus for injury by allowing the foot to twist on the leg in such a way that ligaments and bone are damaged,” said Howard Palamarchuk, assistant professor of podiatric surgery.

According to Palamarchuk, initial treatment for these injuries should include rest, ice, immobilization, compressive wraps, and elevation (commonly known as RIICE). He advises that any ankle or foot injury with pain and swelling beyond 48 hours be checked out by a podiatric physician.

APMA STATE COMPONENTS IN THE NEWS

CPMA Well-Represented in the First Modern House Call for Women

From October 22-October 24, 2010, podiatric medicine played an integral part in a ground-breaking event entitled 'Modern House Call for Women' in Long Beach, CA. The event, an innovative project of the first lady of California, Maria Shriver, was designed to provide medical, dental, podiatric medical, chiropractic, vision, financial and educational services to underserved women in the Southern California area. The Los Angeles (LACPMS) and Orange County (OCPMS) Podiatric Medical Societies, under the direction of Jan Winter, executive director for LACPMS, supplied podiatric physicians, residents and students to provide foot and ankle screenings, education about lower extremity health, including diabetic precautions, and when necessary, performed minor surgical procedures on the women.

Dr. Franklin Kase, the podiatric medical director for the event and Chairman of the Health Policy Committee for the California Podiatric Medical Association, stated that "the goal of the project is to provide women with necessary medical and non-medical education so that they can have some of the tools needed to become active participants in their own health and well-being and improve their and their families' lives. This is an opportunity for podiatric medical physicians to give back to their community."

Query: Getting Paid for L4360

Does anyone have a list of ICD-9 (diagnoses) that are covered when billing L4360 (pneumatic walking boot)? According to the NGS DMAC LCD, if I am reading it correctly, only ICD-9 718.47 (contracture of joint, ankle/foot) and ICD-9 728.71 (plantar fasciitis) are indicated for payment. We typically dispense them to patients who have broken their foot and/or ankle. That doesn't seem to be working anymore.

Response: You are definitely misreading the policy. Night splints (L4396) have the requirements you listed.

For walking boots, either L4360 and L4386, there is no specific ICD-9 inclusion. There are orthopedic and neurological requirements with an exclusion for use of walking boots to off-load an ulcer. Under these latter circumstances, the walking boot should be billed as a non-covered service, A9270, with a "GY" modifier (non-covered service). No ABN is required.

For the L4360 and L4386, you must use both the "KX" modifier and then either the "RT" or "LT" modifier. Also be sure your name (in upper case) and NPI are in boxes 17 and 17B (or their electronic equivalent).

RE: WebMD Survey

I want to inform the readership that I just participated in an invited on-line market research survey from WebMD. Because I do a great deal of wound care, I was asked to comment in detail regarding particular wound care items and spent about 25 minutes doing so for a promised $45 honorarium.  At the final screen, I was informed that the survey was closed. Of course, they now have my entire survey completed for free.

This seems to be fraudulent and unethical.  I wouldn't recommend that anybody take WebMD surveys in the future if this is their typical behavior. Has anybody else had this happen to them?

I am truly overwhelmed at how many of you wrote to me regarding my post about orthopedists opting out of Medicare. It is apparent to me that there is a lot of dissatisfaction in our profession. Quite a few of you told me that I have a unique situation and that you could never do what I did. Many of you wrote and said that you are at your breaking point and cannot stand the way you are being "forced" to practice but are afraid to drop your insurance contracts for fear of losing patients. And some of you just offered kudos for what I, and others, have done.

May I say that what I and many other doctors who have dropped out of insurance participation have done is not out of reach of any practitioner. The only thing keeping you stuck is...
