OH Podiatrist’s Winning Photo to be Featured on Wildlife Legacy Stamp
 
A colorful photograph of a hummingbird clearwing moth, an essential pollinator, will be featured on the 2025 Ohio Wildlife Legacy Stamp, according to the Ohio Department of Natural Resources (ODNR) Division of Wildlife. The winning photo was taken by Roger Friedman, DPM, of Westlake, Ohio. Dr. Friedman photographed the moth in northeast Ohio’s Lake County.

Dr. Friedman’s moth photo was chosen as the winner of the 16th annual legacy stamp photo contest based on originality, composition, technical excellence, and overall beauty. The contest had 84 entries, and submissions were required to feature a pollinator.

INTERNATIONAL PODIATRY IN THE NEWS
MV Diabetes Launches School of Podiatry in India
 
MV Diabetes, in collaboration with the Academy of Physicians in Wound Healing from the U.S., is launching a one-of-a-kind School of Podiatry. The School, operating under the banner of Diabetic Foot Research India, will provide training for doctors and paramedics on podiatry skills, including nail care and comprehensive diabetic foot care practices currently utilized at MV Diabetes, Royapuram.

The inaugural training session to be conducted from October 23 to 25 will host 25 physicians, including a couple of paramedics, from across Tamil Nadu. The three-day course will provide participants with six credits from the Tamil Nadu Medical Council. The curriculum will cover the fundamentals of diabetes, diabetic foot care, surgical interventions for foot issues, and the latest treatment advancements in diabetes management. Anthony Iorio, DPM, Adjunct Professor, New York College of Podiatric Medicine, is one of the faculty in the first program.

QUERIES (NON-CLINICAL)
Query: Virtual Services
 
Does anyone have direct firsthand experience using virtual services, specifically for front desk/front office duties such as phone calls, scheduling, and insurance verification? And if so, please provide your recommendation. Most of the companies that I’ve found all advertise these services, and more. We are having extreme staffing issues and have exhausted all avenues to find staff such as online ads, temp agencies, and recruiters. The time and effort spent to train these employees just for them to leave after a short time is draining. If these virtual services are in place and work as they should, I would hope that it would alleviate a lot of the burden on the existing staff.

A recent post in PM News pertaining to insurance reimbursement compliance said, "Any abnormal findings on the LEAP Vitals Exam, i.e. dry and xerotic skin (L85.3) caused by sudomotor dysfunction, is a significant risk to a patient with diabetes. It therefore warrants a care plan." To prevent confusion of business names, this is not a stated opinion of the 501(c)3 not-for-profit LEAP Alliance.

I stand by my partner Alan Bass, DPM, whose opinion is absolutely correct. Each patient encounter should have at a minimum an appropriate history and physical with components of lower extremity systems including but not limited to dermatology and must also include neurovascular and a MSK examination. Any changes in patient history or PE should be well documented and incorporated into the note.
 
But the change in history is not what will get you paid for a separate E/M nor is documenting a change in the physical examination. It is that last part, the management, what exactly did you do? If all you did was document a change in history, nope. If all you did was document a change in the PE, again, no dice. You must document all 3 issues, ...

I wish to second all of the comments made by Dr. Caringi. From the time I graduated from podiatry school to this day, I have always had an in-office laboratory where I have made orthotics dating back to leather and cork through Rohadur, Polydor, and now finally Carbonfiber.
 
Yes, we have lost that aspect of our practice to the OTCs, pre-fabs, and yes even some of the laboratory produced accommodative orthotics. It is a shame that podiatrists don't control the true biomechanical physician crafted corrective orthotics market.

I read Dr. Caringi’s letter with a bit of sad remorse.  Biomechanics was a very important aspect of the DPMs’ training “back in the day”. Like Dr Caringi, I also performed a biomedical exam, gait analysis, and did plaster of Paris impressions. For the most part, all of my orthotics were custom made, meaning I listened to the patient and made individual adjustments. Rarely, no two devices were identical. Sometimes I had to make adjustments over and over again until I got it just right. 
 
The patient had a true prescription which could be the basis for further devices. One size fits all is a travesty to our profession. Patients with severe deformities need those to function. Not all patients are surgical candidates.  Patients would gladly pay several hundred dollars if you could make a device that really works.  
 
And by the way, at least in California, orthotics are not considered a real prescription hence a class action suit would be fruitless.

I graduated from OCPM in 1980 with Greg and practiced near the Toronto area for over 40 years until I retired in 2002. My practice was a successful one with thousands of loyal patients, standing out from almost all of the others within a 50 mile radius using the same mantra as Greg's. Proper biomechanical exam, non-weight-bearing plaster casting of each and every orthotic patient whether they were 8 years old or 80 years old. I was lucky enough to have a 3 year residency, surgically trained (with fellowship) podiatrist come up from the U.S. to take over my practice when I retired. He practiced in the U.S. for 5 years before coming up here to settle.
 
It took a full year of hammering away at the importance of biomechanics as an adjunct to all his surgical training. It took awhile but it sunk in. He had virtually no training in biomechanics. I was shocked but was relentless, teaching and mentoring about the principles of gait analysis, a thorough biomechanical exam, and...
