PODIATRISTS IN THE NEWS
Diabetics Need Twice Yearly Foot Exam: MI Podiatrist

For people with diabetes, the podiatrists of the Michigan Podiatric Medical Association (MPMA) share that taking care of their feet is especially vital. More than 60 percent of all non-traumatic lower-limb amputations worldwide are related to complications from the disease, according to the American Diabetes Association.

Dr. Jodie Sengstock

“The leading cause of hospitalization among people with diabetes is foot ulcers and infections, but most of those problems are largely preventable,” says Jodie Sengstock, DPM, MPMA director of professional relations. “It’s important for those with the disease to receive regular foot exams by a podiatrist.” Diabetics need to visit a podiatrist regularly – at least two times per year - to avoid unnecessary complications.

PODIATRISTS IN THE COMMUNITY - PART 2
OH Podiatrist Sworn in as South Euclid's First Poet Laureate

The United States has one, the state of Ohio has one, and so does Cleveland Heights. Each has a poet laureate and so, now, does South Euclid. At Monday's City Council meeting, Mayor Georgine Welo swore in M.A. "Doc" Janning, DPM, 75, as the first person to hold the position in South Euclid.

Dr. M.A."Doc" Janning

It was Janning, a retired podiatrist, who first brought up the subject of South Euclid naming a poet laureate at a council meeting several months ago. "I had two choices," Welo said of selecting a poet laureate, "but the other person withdrew when they found out Doc Janning was the other (candidate)."

PODIATRISTS AND POLITICS - PART 1
NJ Podiatrist Faces Challenge for North Brunswick Township Council

The two Democratic incumbents on the North Brunswick Township Council are trying to retain their seats during the general election on Nov. 6. Carlo Socio and Dr. William Lopez will be challenged by Republicans George Callan and Richard Pender.

Dr. William Lopez

Dr. Lopez has been a resident for 12 years. He is a podiatrist. He has served on the council for one year. He is a volunteer for the NYC Marathon and provides community foot screenings in and around Staten Island. If re-elected to the council, Lopez said he would like to improve conditions for North Brunswick seniors by expanding programs and resources.

PODIATRISTS AND POLITICS - PART 2
NJ Podiatrist Running for Re-Election to Freehold Borough School Board

Four candidates are seeking three available three-year terms on the Freehold Borough K-8 School District Board of Education in the Nov. 6 election. The terms of board members Paul Jensen, Dr. Michael Lichardi, and Susan Greitz will end in December. Lichardi is a physician assistant of more than 30 years and a doctor of podiatric medicine who has lived in Freehold Borough since 1998. He joined the school board in 2009 and has served as president since 2015.

Dr. Michael Lichardi

If re-elected, Dr. Lichardi said areas of focus for him will be ensuring that the construction of the additions and renovations to the district’s facilities are completed safely with any impact to school operators minimized, continuing to advocate for fair funding from the state, continuing to work toward academic improvement and narrowing achievement gaps represented in state assessment results, and beginning strategic planning renewal with respect to potential school consolidation or regionalization legislation that may be forthcoming from Trenton.

PRACTICE MANAGEMENT TIP OF THE DAY
Back Office Billing Strategies - Part 1

1. Train select employees to set up payment plans. Often, the front desk will discover the need to set up a payment plan with self-pay patients or patients with an outstanding balance. But the actual arrangement is made by the back office staff. It’s crucial these plans be consistent and fair for all patients. You need to have one or two people in your back office who know how to correctly set up plans. Training back office staff will also alleviate the pressure on the front desk to make decisions about waiving payments—or let their emotions cloud their decision-making.

CODINGLINE CORNER
Query: Non-Union and Fracture Management

I had a patient (with Medicare + secondary) who underwent an arthrodesis of the 1st metatarsophalangeal (MTPJ) joint in mid-June. The patient did not return for follow-up after the sutures were removed at the second visit. The patient returned the first week of October (past the global period) with soreness and swelling in the first MTPJ. Radiographs revealed a non-union at the surgical site. All the hardware was intact. Treatment was immobilization in a CAM boot, and an external bone growth stimulator.

Would it be appropriate to bill for a fracture management code, CPT 28470 (metatarsal, closed, no manipulation)?

R. Alex Dellinger, DPM, Little Rock, AR

Response: The description of CPT code 28470 is for the closed treatment of a metatarsal fracture. I would not classify the non-union of this surgical procedure as a non-union of a fracture. Billing and E&M code, immobilization in the CAM walker, and x-rays at the visit after the global period is appropriate. Billing for subsequent E&M (if appropriate), and x-rays is fine, but I do not feel that CPT code 28470 applies in this case as the non-union was not a “true fracture” in the sense it was intended.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Brian Kiel, DPM

We use Dragon Medical and it does an excellent job of accuracy at normal to fast talking. It also learns new words. For accuracy's sake, I read on the screen as I type and I am able to pick up any discrepancies .

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Charles Morelli, DPM

I have been using this program for close to 20 years now. I'm actually writing this message while using Dragon voice dictation. I am not going to reread this, or by going to correct mistakes. Voice dictation has made during my nodes remarkably simple although I've had to correct mistakes on occasion especially when trying to dictate patient's names and using other medical terms that are far into the per gram. You are able to recognize that word so that the mistakes I made the future. What I've chosen to do is just not voiced dictate names but typed in manually. Has been a great asset to my practice, writing notes and doing anything that one would normally have to type

Here's what I just learned about the new version of Dragon I just...

Editor's notes: Dr. Morelli's extended-length letter can be read here. This note has NOT been edited and has been written exclusively with Dragon Voice dictation.
