UK Charity's Foot Care Gives Homeless Man Hope
 
A man who had been living in his car for four months has spoken about the life-changing support he received from a charity's foot clinic. "When I first came to Forgotten Feet, I was homeless," Steve, from Plymouth, recalled. "My feet were in pain, I was struggling to walk, it felt like I was kicking my toes against a brick wall."

Forgotten Feet, founded in 2013 by Deborah Monk, provides free foot care to people who are homeless or socially isolated. Forgotten Feet now runs dozens of free clinics across the UK. In Plymouth the sessions are held every other Saturday morning and organizers say they can see up to 10 patients per session, but the numbers are increasing and so the charity is appealing for more qualified podiatrists to volunteer.

OBITUARIES
RE: The Passing of James D. Heddens, DPM
 
James D. Heddens, DPM, podiatrist emeritus, crossed from this life to new life at the active age of 88 on October 14 in the beautiful Iowa River Hospice house surrounded by his children. Jim was a pioneer in the early 1960s, the first podiatrist in Iowa to secure hospital surgical privileges in Davenport, Nevada, Marshalltown, Pella, and Newton.

His determination opened the door for many podiatrists across the state to perform foot and ankle surgery in local hospitals. He later served as president of the Iowa Podiatric Medical Association, two terms on the Podiatry Examining Board, and many years in peer review. Throughout the 1980s and ’90s he sponsored numerous podiatric preceptorships, equipping new doctors to succeed and serve Iowans with excellence.

Second: conversion rates relative to orthopedics in the same system. Third: is anyone aware of potential avenues podiatry could take regarding potential discrimination in this situation?

RE: Sacred Myths

There are beliefs that people hold which are undeniably ludicrous. Folks with little money in their pockets will venture down to the paddock to watch the horses in the next race walk by and intuit by their gait and the spring in their steps which horse is most likely to win the race. Others will read the racing form and decide that a pound variance in the weight that the 1,200-pound horse is carrying will make the difference in the race while ignoring the fact that this is the first race another horse will run with Lasix and butazolidin aboard.
 
We sanctify myths in podiatry just the same. The belief that if podiatry changed or disappeared, care of the diabetic foot would also come to an inglorious conclusion. Diabetic foot care is not our sacred domain. And if we do it better, it is probably because of immersion and the fact that we have...

Dr. Oloff... Rod Tomczak, DPM, MD, EdD's comments filled the vacuum created by the APMA's knee-jerk reaction to a very friendly offer from the Accreditation Council for Graduate Medical Education (ACGME) to sit down and talk. You attacked the messenger. 
 
The only reason for this appears to be people wanting to "protect their turf from the unknown." Does anyone actually think podiatric residency directors would no longer be necessary if we sat down with ACGME and talked? 
 
Dr. Tomczak's insights are always enlightening and spot-on, even when painful to some. Thank you, Dr. Tomczak.

RESPONSES/COMMENTS (CELLULAR TISSSUE PRODUCTS)
RE: OIG and Cellular Tissue Products

In a recent Medical Economics, there is an interview with David Tawes, MA, an HHS-OIG Regional Inspector General. During this almost half hour interview, Mr. Tawes cites the many issues involved with cellular tissue products (CTP) utilization. In addition to much of the literature now available on compliance on the use of CTP, this interview is a must listen for all wound care providers. As with the new LCD and other related publications, this interview should be used as an educational tool to assist providers with proper documentation.
 
The OIG interview should not be viewed as a “scare tactic” disincentivizing wound care providers to use CTP when they are medically necessary. Tawes cites the largest investigation of one case resulting in one non-professional company, most often soliciting nurse practitioners who submitted $1.2B worth of claims. One other interesting statistic is that Medicare Advantage patients received a disproportionately lower number of CTPs as compared to traditional fee-for-service. This interview may be heard here.

Yesterday at my physical therapist's office, there was a student DPT assisting. I asked him if he considered other professions before starting PT school. He told me that he shadowed PAs, DPMs, and PTs. The two podiatrists he shadowed discouraged him from podiatry. One DPM said that he had wished he had gone to medical school and became a dermatologist. The other DPM said that he should have gone into engineering instead of podiatry.  As Walt Kelly said in his Pogo comic strip, "We have met the enemy, and he is us."
