IL Podiatrist is “Head Haunt Master” at Halloween Venue
Dr. John Shoudel said that the Boo Crew Haunted House in Buckhart (four miles east of Rochester) is going to be scarier this year, too. Shoudel, podiatrist and Associate Clinical Professor of Orthopedics at SIU School of Medicine in Springfield, has been putting on the Haunted House for the Rochester Lions Club since 1999. In his role as “Head Haunt Master,” he oversees and helps with a crew of volunteers who construct and light the venue, create special effects, apply makeup, act in horror scenes, and staff the ticket and waiting area.

“We have our own property so we have time to work on our sets — and we have people from all over central Illinois who volunteer here. We attract … the best people in the industry,” who, said Shoudel, help draw visitors from all over central Illinois, St. Louis, and Chicago.

Insurance Premiums: Is $100 the Next Obamacare Hurdle?
A large majority of adults say "$100 a month or less" is the highest monthly premium they can afford to pay for health insurance in 2016, according to a survey released Wednesday. The HealthPocket.com report found that 57 percent of respondents named that price range when asked about coverage affordability. The second-most common answer was $200 per month, which was the response of just 17 percent of respondents.
Cost, or perceived cost, is a prime reason that the remaining uninsured people give when asked why they do not purchase health insurance, even as the Affordable Care Act has brought the uninsured rate down to record lows. And Obamacare plans routinely cost more than $100 per month before the federal subsidies available to many enrollees are factored into the price. Those subsidies are available to people with low and moderate incomes — but only if they buy plans on government-run insurance exchanges such as HealthCare.gov. About 85 percent of exchange customers qualify for such financial aid.

11 Things to Know about the Meaningful Use Final Rule - Part 10
It Ain’t Over Until Congress Says it’s Over
Congress had been urging CMS to delay issuing Stage 3 rules; now, a confrontation appears likely. Sen. Lamar Alexander (R-TN) was furious, issuing a press release immediately after CMS dropped the rule, accusing CMS of “rushing its rule…doing a disservice to more than 500,000 doctors, thousands of hospitals and millions of patients.” There’s also support for a delay in the House, and Congress is expected to review the rule and perhaps change it through the Congressional Review Act.

Query: Candida Antigen Injection for Warts
When doing a Candida antigen injection for warts, is the correct procedure code, CPT 95115 (professional services for allergen immunotherapy not including provision of allergenic extracts; single injection)? What would be the HCPCS code for the injectable?
James Hatfield, DPM , Encinitas, CA
Response: The real answer starts with a question: Aren't you destroying a wart? CPT 17110 is defined as "destruction (e.g., laser surgery, electrosurgery, cryosurgery, chemosurgery, surgical curettement), of benign lesions other than skin tags or cutaneous vascular proliferative lesions; up to 14 lesions." Aren't you destroying this using chemosurgery?
I do not recommend CPT 95115 (professional services for allergen immunotherapy not including provision of allergenic extracts; single injection). There is no "J" code for this to bill.

Query: Help Simplifying Registering New Patients
From: Ken Meisler, DPM
I'm wondering if anyone has suggestions for solving an issue we have in our office about entering new patient information into our EMR program.
Our office sees many new patients a day. We tried to get patients to download the new patient information sheet from our website, fill it out and fax the information a day or two before the visit so all of the information could be entered in the computer before they arrived. Many patients preferred to fill the form out when they...

Query: Associate Billing Under Owner's Tax ID
I wonder how prevalent the following problem is, and how others are dealing with it. I'm taking in a new associate, who will likely purchase my practice. In trying to get him contracted with the plans in which I'm a participating provider, we're informed that at least one of the panels is closed to new providers. I'm a solo practitioner, but do my billing and send claims to managed care under my own individual Tax ID number.
I'm told that one solution would be to have him bill under my Tax ID number, even though it's an individual, not a group Tax ID. I've got some concerns about doing that, and it may also only be a temporary solution. If and when I turn the practice over to him and possibly retire, my Tax ID will evaporate, and he'll be on his own trying to get on panels that are closed.

From: Brian W. Fullem, DPM
The advice provided in the Buffalo News by Dr. Ioli is incomplete at best. Rest, stretching, and ice are options that only help the symptoms. The medical evidence shows that eccentric strengthening exercises and extracorporeal shock wave therapy are two of the most beneficial treatments for Achilles tendonosis. Degeneration of the tendon is the main pathology in most tendon disorders.
Immobilization is one of the worst treatments and should be reserved for those cases that are resistant to treatment. Keep in mind that when an area is immobilized, there will be an associated atrophy of those structures that must be addressed following the immobilization. Strengthening the hip abductors and core musculature will also have a great impact on improving Achilles tendonosis. Iontophoresis is a modality that may help with symptoms, but is lacking in medical-based evidence.
