PODIATRISTS IN THE NEWS
CA, MA Podiatrists Explain Epidemic of Plantar Fasciitis

Why the epidemic of plantar fasciitis? Blame high obesity rates, yoga, and casual Friday (and casual Monday-Thursday). The shift from suits to casual wear in the workplace — and the attendant move from stiff, leather-soled shoes to flexible sneakers — means people are wearing shoes that provide less stability and rigidity than in the past, said Bob Baravarian, DPM, an assistant clinical professor at UCLA School of Medicine, and director of University Foot and Ankle Institute in Los Angeles. Those mushier shoes, he said, put more strain on the fascia than other, more structured footwear.

Drs. Bob Baravarian and Kenneth Leavitt

Yoga is a culprit, too, said Kenneth M. Leavitt, DPM, chief of podiatric reconstructive foot surgery at New England Baptist Hospital. Any activity where you’re standing barefoot for a long time can cause problems, he said.

REMEMBERING HISTORIC PODIATRISTS
TX Podiatrist Finally Receives Headstone After Several Decades

Several decades after her death, a well-known member of the Wichita Falls community now has a headstone. A benefit was held to raise money for a headstone for Dr. Annie Davis Roark back in March, and after community members raised about $2,500, the stone was dedicated and placed Saturday morning at Lakeview Cemetery.

Gravestone Dedication for Dr. Annie Roark

Dr. Roark passed away in 1984, but was buried in an unmarked grave in Lakeview Cemetery behind Kiwanis Park. Roark was the first African American female to open a podiatry practice in Wichita Falls. She also owned the Isis Theater and the Roark Hotel in Wichita Falls.

INTERNATIONAL PODIATRISTS IN THE NEWS
Aussie Podiatrist Discusses Removing Glass from Your Foot

Stepping on broken glass can be incredibly painful. In between trying to clear up the blood, figuring out if all the glass has been safely removed from the floor, and whether or not all the tiny bits have been removed from your foot can be quite the challenge.

Christina O’Brien

It can also become stressful if it’s a grandchild or someone young who has stepped on glass. While there are plenty of myths and hacks for removing glass on the Internet, ranging from baking soda to white vinegar and even honey, Christina O’Brien from the Australian Podiatry Association explained to Starts at 60 that it’s always best to seek professional help in most cases. “I would definitely see a GP or see a podiatrist because I’ve pulled glass out of peoples feet before and sometimes the slivers are so tiny,” O’Brien explained.

RELEVANT RESEARCH
Comparison of KOH , Histological Exam, and Culture for Diagnosis of Onychomycosis

The authors of this study performed KOH, culture, and histologic examination of nail fragments with PAS and GMS stains in 106 patients with a clinical suspicion of onychomycosis. The frequency of positivity was 73.6% with KOH, 14.2% with culture, 30.2% with PAS, and 66.0% with GMS. The highest sensitivity was observed with GMS and KOH (100% and 92%, respectively), and highest specificity was seen with PAS (100%), followed by culture (93%) and GMS (82%). GMS also was associated with the highest diagnostic accuracy (93%). While high specificity was seen with PAS, culture, and GMS, only GMS had high sensitivity, making it the most reasonable choice for a confirmatory test.

Based on these data, the authors propose KOH as a screening test and GMS as a confirmatory test in the diagnosis of onychomycosis at their institution. Since positivity rates of different diagnostic methods may vary by center, the authors recommend that each center perform a latent class analysis to determine the most effective diagnostic strategy for their patients.

PRACTICE MANAGEMENT TIP OF THE DAY
Front Office Billing Tips - Part 3

If you want your billing and collections to be as smooth and effective as possible, you need to make sure everybody on your team is doing their part.

3.  Confirm insurance information is up-to-date. On the day of the visit, ask to see a copy of patients’ insurance cards. Insurance plans change more frequently these days, and patients often forget to contact the office to update their files. This is extremely important but often overlooked information. Otherwise, your practice will lose money since staff will have to spend time sorting through returned claims, locating and updating patients’ information, and resubmitting claims (assuming those resubmitted claims will eventually be paid).

RESPONSES/COMMENTS (PM NEWS QUICK POLLS))
RE: Should podiatrists be allowed to trim fingernails?
From: Christopher A. Orlando, DPM

I'm reading the various opinions about fingernail trimming. Unfortunately, this is a sad testimony  to our image with the general public. We hold ourselves up to be podiatric physicians and surgeons with extensive training who provide state-of-the-art medical and surgical care that is superior to that of orthopedists. There has been talk about a DPM-MD degree. Yet the public continues to perceive us as nail trimmers.

Many times, patients come in for medical and orthopedic issues, and in the course of the encounter, they say "by the way, can you trim my toenails?" It’s hard to say no. It has nothing to do with the purpose of the visit. Getting paid extra for it is irrelevant. I find it demeaning. Trimming fingernails sinks us even lower. How can we achieve parity with orthopedists when our global image is so poor? I'm out of ideas. Any suggestions? As Dr. Leonard McCoy of Star Trek would say, "I'm a doctor not a… (manicurist)!"
