PODIATRISTS AND POLITICS
ID Podiatrist Supports UT Alternative to ACA

Candidates for Idaho legislative Districts 28 and 29 tackled issues from education to energy during a forum sponsored by the League of Women Voters Monday night. The issue that sparked the greatest difference between Republican and Democratic candidates was expansion of Medicaid under the Affordable Health Care Act. Republican Dr. Kert Howard, a Pocatello podiatrist challenging incumbent Democrat Roy Lacey for the Idaho Senate in District 29 didn’t express total opposition to expansion in Idaho, but he said 60 percent of his patients are currently on Medicare or Medicaid.

(L-R) Dr. Kert Howard and Incumbent Roy Lacey

Howard said he supports the Utah approach to expansion which is trying to convince the federal government to let the state give low-income people subsidies to buy private health insurance instead of expanding Medicaid. "We do need to take care of indigents,” Howard said.

INTERNATIONAL PODIATRISTS IN THE NEWS
UK Charity Provides Information Packets for Diabetic Patients

The Department of Health and Social Care’s Community Podiatry Service is grateful for the continued financial support from a local charity, the Friends of the Manx Diabetic, which has enabled the production of an updated information pack on foot care for diabetics. The information packs are given to newly diagnosed diabetic patients when they are assessed by the podiatrist.
Julie Maddrell

Julie Maddrell, Clinical Lead Podiatrist for Diabetes, said: “The Podiatry Service provides assessment, diagnosis, and treatment of foot and lower limb disorders. These services are delivered in hospitals, at the Diabetes Centre on the Noble’s Hospital site, in residential and nursing homes, and in clients’ own homes. Having these information packs for diabetic patients helps promote consistent information and advice on how to look after their feet.

HEALTHCARE NEWS
Staph Aureus Scheme Cuts Surgical Infection

Practices to eliminate Staphylococcus aureus are associated with significantly lower rates of surgical-site infection, according to a new study. The approach consists of screening the nostrils of patients for S aureus, decolonizing with mupirocin and chlorhexidine baths when necessary, and administering peri-operative prophylactic vancomycin and cefazolin to patients with methicillin-resistant S aureus (MRSA).

"We feel that if people actually did implement this bundle, it could substantially reduce patient morbidity and the cost of care," said Loreen Herwaldt, MD, from the University of Iowa Health Care in Iowa City. "Furthermore, it is very important to note that we really saw the effect only with the full bundle." Dr. Herwaldt presented the results of the study here at IDWeek 2014.

MEDICARE NEWS
CMS Dispatches MAC Docs to ALJ Appeals, Which May Alter Outcomes

CMS has instructed Medicare administrative contractors (MACs) to get off the sidelines and defend claim denials that are appealed to administrative law judges. In Medicare Transmittal 543, which takes effect Oct. 27, CMS says MAC physicians should make a case for some MAC medical review decisions that resulted in claim denials, were appealed by providers up the chain, and are now scheduled for ALJ hearings. Not all ALJ hearings require their presence, but MAC physicians (e.g., contractor medical directors, employed physicians) should attend when certain planets align, CMS said.

CMS expects MACs to keep their eye on cases as they progress through the appeal process so that they know when cases headed for the ALJ are ripe for MAC participation. MACs will have to report on their ALJ hearing participation to CMS every month.

QUERIES (NON-CLINICAL)
Query: Source for Monocete EZ-Swabs

I was a long-time user of Monocete EZ-Swabs [Pedinol] to treat verrucae. It is no longer available. Does anyone know where this can be ordered from? How about a comparable product? In my 20 years of practice, I have never been so satisfied with a treatment for warts, and monochloroacetic acid always did the trick!

RESPONSES/COMMENTS (OBITUARIES)- PART 1A
RE: The Passing of Neal Frankel, DPM
From: Alan Sherman, DPM

Dr. Neal Frankel

It is with great sadness and shock that I received the news today of the untimely death of Neal Frankel, DPM. Neil was a great doctor, but is best known for being a great leader in the business of podiatry practice. He touched the lives of so many of our colleagues by building what I believe is the largest IPA of podiatry practices in the country. Neal gave lectures at many of the PRESENT conferences, and when other podiatrists were anguishing over the deleterious changes going on in managed care and reimbursement for services, Neal was negotiating with payers and teaching other podiatrists how to stand up for their rights. Neal was a tough, street-wise doctor who never took no for an answer when he knew he was within his rights. I will miss his positive attitude and bright smile. Rest in peace.

With tears in heart, I just heard that Neal Frankel passed away from a massive heart attack. Neal was at the top of my list of human beings with heart. He was my mentor. God bless him.

YOU CAN'T MAKE THESE THINGS UP
RE: Absurd ICD-10 Code of the Day

V97.33XD: Sucked into jet engine, subsequent encounter.

Sucked into a jet engine, survived, then sucked in again? First of all, that really, really sucks. Second of all, this patient is obviously Wolverine, and should be detained for imaging and posterity. (Technically, this means "subsequent encounter with a physician" not "subsequent encounter with a jet engine," but that's less dramatic.)
