APWH Holds Its 2019 Annual Conference
The Academy of Physicians in Wound Healing (APWH) held its Annual Conference last weekend in Philadelphia, PA. Organized by Steven Kravitz, DPM and Robert Gunther, DPM, this multi-specialty conference was attended by podiatrists, dermatologists, primary care physicians, vascular surgeons, physician extenders (NP, PA), and wound care nurses. The theme of the meeting was not only how to treat wounds, but how to manage their underlying etiologies.

1. Easy online appointment-setting - Patients can order anything from a book to a mattress at any time with a few clicks on their smartphone. They don’t want to spend 15 minutes on hold to make an appointment. Practices need to offer online scheduling to patients to make it easier for them to book an appointment.

Effectiveness of Arginine Supplementation on Wound Healing in Older Adults in Acute and Chronic Settings
This systematic review evaluated the effect of arginine supplementation on wound healing in older adults who are in either acute care or long-term care facilities. A total of 6 studies including 572 patients with pressure injuries were included. Of these, 5 studies found a significant improvement in wound healing in patients taking oral nutrition supplements containing arginine. Daily doses of arginine ranged from 4.5 to 9 grams total. Most of the supplements studied also contained other nutrients such as zinc, vitamin C, vitamin E, and vitamin A.
These findings indicate that arginine supplementation along with oral nutrition supplementation may be effective for wound healing. However, data on the effectiveness of arginine supplementation alone are lacking.

RE: Re-evaluation of Traditional Angular Parameters for Austin Bunionectomies

It's interesting that in 2019 we still have a lot to learn regarding bunion surgery. I routinely perform Austin bunionectomies on IM angles of 20 degrees using a modified locking 0.045 K-wire technique and will shift the met head over 50% or more. These patients walk immediately post-op.

I have been following this discussion for years now, in fact for more than 30. Several different iterations of discussion have evolved over the years based on the instrumentation including burs and motors, as well as intra-operative radiography. I have reached the point where I truly believe that there is a place for minimally invasive surgery, BUT, I also believe that it is not the panacea for every bunion patient that walks in the door. There must be a point where the surgeon decides that perhaps a different procedure may give the patient a better outcome.
This example is one of those cases; given the pre-operative appearance of this foot, I believe that a better course of action would have been a more proximal procedure like a tarso-metatarsal arthrodesis. Critically evaluating these post-operative x-rays, there is quite a bit of shortening of the first metatarsal which is likely to lead to either a second metatarsal stress fracture or metatarsalgia.

I’m surprised that you were able to get Demerol (Meperidine), as I was under the impression that the FDA banned Demerol and Darvon some time ago, along with anything else containing propoxyphene. I tried to avoid it at all costs due to the nephrotoxicity of normeperidine (the 1st pass metabolite).

I agree with most of Dr. Sherman’s assessment regarding a single certifying board. Structurally, it’s a sound solution - a single certifying board and sub-specialty boards for our sub-specialties. However, will it solve the issue of recognition by the public that podiatrists with specialty training are differentiated from “podiatrists”? Dermatologists, vascular surgeons, gastroenterologists, etc. are identified by their specialties; their general medical degree is assumed.
I question whether the public will identify a podiatric foot and ankle surgeon as simply a foot and ankle specialist, or a podiatric dermatologist as simply a dermatologist of the lower extremities, particularly when there are orthopedic foot and ankle surgeons and MD/DO dermatologists. It seems to me the least confusing solution is a single certifying board... the allopathic or osteopathic certifying board with a sub-specialty for podiatric medicine (lower extremity medicine), if necessary.

About twenty years ago, an ultraviolet office filtration system called "Healthy Climate" became available through the Lennox Corporation that promised to reduce the airborne microbes in the office. Two ultraviolet 4-foot tubes were installed just "distal" to the air handler and would illuminate automatically if at least the fan function was on, and of course, when the AC cycled on.
I think this system adds a degree of safety to the office setting. When I purchased it, it was at the beginning of the promotion, so I received a nice price break. Three years ago, I had my local AC vendor/repair people replace the tubes, which are not cheap (they only last a few years). Evidently, through my own and the servicemen's ignorance, I have had useless tubes up in my system for some time. Every two years when I have my AC/refrigerant check-up, I have them look at the tubes to see if they need replacement.
