Change in This Week's PM Quick Poll
Shortly after we launched this week's quickpoll on Saturday afternoon, we modified the choices to reflect the number of meetings instead of the specific meetings you plan to attend. If you were one of the 45 podiatrists who responded to the initial question, we ask you to redo the poll. As always, we count only the last response received from a particular IP address in our tabulations. We apologize for any inconvenience.

NY Podiatrist Wants to Build Shoe Museum in Manhattan
This Park Ave. pad has a lot of sole. Celebrity podiatrist Dr. Suzanne Levine is selling her office of 25 years — a maisonette near 78th Street that has brought in actresses, directors, and heads of state. Now she’s looking for a creative buyer for the $1.25 million medical space, bringing her one step closer to fulfilling her dream of opening the city’s first shoe museum.

“This is the shoe capital of the world,” Levine told The Post. “I feel it’s my purpose in life to build a shoe museum in New York City, where you have access to all kinds of shoes.”

Prevalence of Vitamin D Deficiency in Patients with Foot and Ankle Injuries
Vitamin D deficiency has been identified as one of the most common causes of fragility fractures and poor fracture healing. Although rates of vitamin D deficiency have been delineated in various orthopedic populations, little is known about the prevalence of vitamin D deficiency in patients with foot and ankle disorders. The goal of this study was to identify the prevalence of vitamin D deficiency in patients with a low energy fracture of the foot or ankle.
Hypovitaminosis D was common among patients with a foot or ankle injury seen at our institution. Patients with a low energy fracture of the foot or ankle were at particular risk for low vitamin D, especially if they smoked, were obese, or had other medical risk factors. Given that supplementation with vitamin D (+– calcium) has been shown to reduce the risk of fragility fractures and improve fracture healing; monitoring of 25–OH vitamin D, and supplementation should be considered in patients with fractures.

| QUERIES (EMR) | |
Query: SkyCare
I received a phone call on Friday. I was advised that the caller represented SkyCare (previously Platinum MD) and they wanted information to see if I qualified for a $15,000 to $20,000 refund from CMS if I billed Medicare or Medicaid over $20,000 during this past year. Has anyone else been contacted or does anyone have experience with this company? What were your results?

| RESPONSES/COMMENTS (CLINICAL) - PART 1 | |
From: Robert T Morris, III, DPM
In short, reactions are not common and the data is questionable. Brackett writes: "apparent cross-reactivity responses to sulfonamide-containing drugs likely represent multiple concurrent, rather than linked, drug hypersensitivities." Additionally, Knowles expounds "reports showing lack of cross-reactivity balance the few case reports suggesting cross-reactivity." These quotes are per the articles cited below, which were found through a simple google search (which will lead you to PubMed.org - a wonderful website with a wealth of academic literature).
Brackett CC. Sulfonamide allergy and cross-reactivity. Curr Allergy Asthma Rep. 2007;7(1):41-8. [ncbi.nlm.nih.gov/pubmed/17504660]
Knowles S, Shapiro L, Shear NH. Should celecoxib be contraindicated in patients who are allergic to sulfonamides? Revisiting the meaning of 'sulfa' allergy. Drug Saf. 2001;24(4):239-47. [ncbi.nlm.nih.gov/pubmed/11330653]

| RESPONSES/COMMENTS (CLINICAL) - PART 2 | |
From: Simon Young, DPM
Thank you, Dr. Rosenblatt, for the informative article and post. We must look at the whole patient. Not every heel pain is plantar fasciitis, not all 1st MPJ pain is due to a bunion. Although I am a fan of the KISS (keep it simple) principle, if symptoms persists, start looking for other causes.

From: Barbara Aung, DPM
I am a solo practitioner. Two years ago, I became a certified coder and a certified medical auditor through AAPC. I am also a member of the APMA Coding Committee.
I do understand your concerns, and definitely, at first glance, it does seem that ICD-10 for practitioners will be extremely challenging. Podiatrists, as a whole, have been...

| RESPONSES/COMMENTS (MEDICAL/LEGAL)) | |
From: Peter Bregman, DPM
I did not notice an MTHFR test or a serum fasting insulin test. Standard Vit B is usually not a culprit. Of course, an epidermal nerve fiber density test is also important for small fiber neuropathy. Having said that, I would like to add that in light of negative blood work in the face of peripheral neuropathy, there is a very good chance that you are dealing with nerve entrapment(s). EMG/NCV tests are not to be relied upon.

RE: The Passing of Ed Stein
I met Dr. Stein once or twice during the New York Clinical Conference. He was very nice and cordial. I urge people to tell a mentor, friend, or colleague what you think of them when they are alive, as well as memorializing them after their deaths.
