Super Lightweight Running Shoes Often Lack Support: UK Podiatrist
“Getting the right footwear is essential,” says podiatrist Anne-Marie O’Connor, but she warns that the super lightweight running shoes which have become popular in the past five years or so don’t offer the support some runners need. “Go to a specialist running-shoe shop where you can try shoes with different levels of support and try at least three different brands."

She adds, "Comfort is everything and if you are just starting out, or don’t run regularly, go for something that’s a little more supportive and slightly more robust. Having more support within the trainer stabilizes the foot, so everything above the foot is more likely to be in alignment.” This will also help to head off ankle, knee, and even hip pain caused by over-pronation.

UT Podiatrists Participate in Prison Telepodiatry Program
To enhance healthcare access to all Utah communities, teams across University of Utah Health collaborated to introduce a TelePodiatry program for the Salt Lake City and Gunnison prisons. The TelePodiatry program is part of a larger initiative to connect University of Utah Health providers with prison populations, greatly increasing the health outcomes of individuals incarcerated in Utah.

The program will call upon the podiatry expertise of the division’s providers. Ryan Rushton, DPM, along with his fellow podiatry providers, will conduct monthly virtual clinics, ensuring patients in the program receive the same high-quality care they would receive in person. Through virtual clinics like the TelePodiatry program, providers are able to offer timely interventions and follow-up care, improving patient outcomes. With the support of University administrators and the dedicated care of clinicians like Dr. Rushton, the division foresees success for the TelePodiatry program in the coming years.

According to Dr. Pinker, the most common foot issues women over 50 face include plantar fasciitis, ankle sprains, and metatarsalgia/bursitis. "Wearing the right walking shoes can prevent or alleviate these issues by providing proper cushioning, shock absorption, and arch support," Pinker explains. "Offering a proper width and length can also be helpful in preventing the development of bunions, hammertoes, and neuromas."

Considering a Practice Change? Five Signs it May Not Be a Fit
Here are a few red flags to signal a new career opportunity might not be the right move.
5. Failure to prioritize family and personal considerations
All candidates are part of immediate and extended family scenarios whether married, unmarried, have a partner, have no partner but have children, have no partner and no children, have parents or pets, etc. You are also part of a community of colleagues that you will be leaving in pursuit of a new opportunity. From the earliest point in the recruitment process, the interviewing team must work to understand all the factors that need to be addressed beyond the specifics of the position, title, start-up package, and financial and benefit plans.
A practice's commitment to embracing your family, however that is defined, is a true window into the culture of the organization and its commitment to the wellness and work-life balance of its physicians. A recruiting team’s willingness to create a respectful environment where you can share information regarding the personal accommodations that need to be considered is critical. Invitations for spouse, partner, etc. social event participation early in the process is a great sign!

In response to the ongoing discussions about Comprehensive Diabetic Foot Exams (CDFE) not being reimbursable, I wanted to provide a different viewpoint. A CDFE certainly might be reimbursable, IF a new diagnosis, or a change in medical condition is found during the exam.
Objective, reproducible, real-time physiological measurements of foot health are provided by the innovative Arche LEAP Vitals+ Program. When a podiatrist follows the comprehensive Arche LEAP Vitals+ protocol, documents the objective abnormal findings, and creates a care plan for management, it is my expert opinion, the encounter satisfies the medical necessity requirements for billing an evaluation and management service, either alone, or in conjunction with at-risk foot care services performed on the same day, using a -25 modifier.

I have been in practice 33 years. I have used the CO2 laser and the radio wave frequency machines many times for matrixectomies. I tried NaOH as well. I have never found anything with less re-occurrence rates or side-effects than phenol.

Thank you, Dr. Gurnick. During my 55 years of practice, I too have used almost every procedure for onychocryptosis, including radio ablation. But, in the ‘70s, I settled on the phenol procedure; not the PA or phenol/alcohol procedure. Following removal of the offending nail portion, I apply 88% phenol for 30 seconds and then dry the groove. Then I repeat this one time. I don’t apply alcohol at that time. I instruct the patient to remove the dressing (4 snug 1” Band-Aids) one hour before bedtime, pour alcohol on the snug dressing, and wait 5-10 minute to loosen the adhesive; then clean the toe with isopropyl alcohol. Then apply a fresh BandAid but fairly loose and not tight; then remove that dressing the next morning and shower without the dressing. Then clean the toe with alcohol again and apply the Band-Aid dressing. The third day should require no further dressing.
I instruct the patient to allow air to the toe as much as possible by wearing a cut-out shoe or a sandal. Then apply alcohol directly to the toe twice a day (drying agent). Apply no ointments or creams and only alcohol. I allow no soaks unless there is pain which is rare. Constant air exposure allows the toe to heal faster. Shower only with no tub baths, etc. Most toes heal in 3-4 weeks some longer. People required to wear closed shoes or steel toes will take longer to heal so I recommend doing the procedure on vacation time. I do not routinely Rx antibiotics unless the patient is compromised with brittle diabetes or poor circulation. Operating on a compromised patient is a judgment call and I will not operate on many of these cases.

I was in practice for over 35 years and had the opportunity to share office space and staff with an internist. He and I became very close and I developed a very keen awareness of the pressures these generalists faced when referring patients to any specialist, not just DPMs. Thus, I saw first-hand the valid issues presented recently by Dr. Jacobs. I am sure he would agree, that does not give the insurance companies the right to pay for substandard care, nor does it alleviate the legal, ethical, and moral responsibilities of the referring physician to get their patients to the right specialist in a timely manner. And simultaneously it does not alleviate the responsibilities of the treating specialist to provide the proper care. Yes, there are theoretical and practical matters at stake here, as Dr. Jacobs duly noted, and walking that line is often a tightrope, but nevertheless everyone in this game, including the patient, must be involved in negotiating these hurdles.
This topic has gotten so heated that a recent U.S. Senate report on this matter was released yesterday and is available at: U.S. Senate Hearing Bashes Medicare Advantage Plans for Using P/A as a tool to boost profits. This report singled out the largest Medicare Part C plans, not just one, and cited 10 takeaways, which I urge everyone to read. What strikes me is that less than 10% of denied prior authorizations were appealed. The report does not reflect how many initial denials were overturned and the eventual health costs to the patients, who may have ultimately paid the price. Hopefully, this is a good start in dismantling a system which is in fact costing the taxpayers more money!
