Harlem Physician Group Opens Offices within FCNY
Joel Sturm, Chief Operating Officer and Eunice Ramsey-Parker, DPM, MPH, Clinic Administrator of the Foot Center of New York (FCNY), have announced that the Harlem Physician Group, PC (HPG) has joined the the Foot Clinics of NY (FCNY). HPG’s physicians and physician assistants provide primary and urgent medical care. Geriatric care will be added to HPG’s services in the near future.

FCNY doctors provide complete podiatric and physical therapy services. The Harlem Physician Group will provide all surgical clearances for FCNY patients. In addition, HPG will also provide health services to NYCPM students, acting as the student health center providing examinations, inoculations, and treating minor illnesses. The Harlem Physician Group’s services are available to all NYCPM and FCNY staff as well.

TX Podiatrist Discusses How to Avoid Runner’s Toe
For seasoned runners, being prepared from head to toe is key to their success, so when they lose their toenails due to a common ailment known as runner’s toe, it can be an impediment to their routine. According to a podiatrist at Baylor College of Medicine, there are some prevention methods that runners can take to help avoid toenail loss. “You don’t need to be a marathon runner to have this problem,” said Dr. Ronald Lepow, assistant professor of orthopedic surgery at Baylor. “Regular runners very frequently experience toenail issues, and it really comes down to ill-fitting shoes.”

The later stage of toenail of runner's toe can be what is referred to as a subungual hematoma, or a blood blister. This can be uncomfortable and may require treatment, which could include removing the nail or part of the nail under a local anesthetic. Because this issue is caused by the toe or toes slamming up against either the end or top of the shoe, Lepow said that running shoes should be about a half-size larger than regular street shoes. This prevents friction as the toes move up and down while running.

First-ever Hands-on Dermal Filler Lab Held at DERMfoot West
On October 15, 2016, faculty members Drs. Bret Ribotsky, Annette Joyce, and Joel Morse took part in a ground-breaking 8 CME aesthetic/medical cadaver lab event where participants learned how to use dermal filler injections for pedal augmentation as well as the art and science of tangential nail fold biopsy under the direction of Daniela Gutierrez, MD.

Arch-filled Insoles for Diabetic Patients Could be Adversely Affecting Balance
A study by the Balance Enhancement Ulcer Prevention (BEUP) research group, led by Dr. Joanne Paton, Podiatrist and Research Fellow of the National Institute of Health Research (NIHR), found that insole designs containing an arch fill generated increased body sway and reduced balance when the wearer was standing still, while those without an arch fill did not. The study was carried out to explore whether different insoles affected balance in people with diabetes. Lack of balance is a major problem for older people with diabetes, a third of whom fall each year – rising to half for those with prior foot ulceration.

The results of Dr. Paton’s analysis also contradict previous suggestions that soft insole materials shield the sense of awareness under the soles of the feet required to help maintain balance, and therefore appear safe for people with diabetes to wear. Entitled "Getting the right balance: Insole design alters the static balance of people with diabetes and neuropathy," Dr. Paton’s study forms part of a five-year research program funded by the National Institute of Health Research (NIHR) to develop and test a novel dual purpose insole that both enhances balance and reduces foot ulcer risk in people with diabetes.

UK Podiatrist Faces Bullying Allegations Over Sexual References and Bad Language
A Glastonbury podiatrist accused of bullying and intimidating colleagues over a number of years is facing a hearing at the Health and Care Professions Council. It's alleged that former St. Dunstan's School pupil Timothy Pursey used inappropriate sexual language and swore at colleagues. He's facing a Conduct and Competence Committee in London this week to determine his fitness to practice.

Health IT Grows in Importance in Final MACRA Rule
The final MACRA rule announced on Friday by the Department of Health and Human Services includes a new Quality Payment Program tying physician payments to quality of care, which will require providers to rely heavily on healthcare information technology, according to CMS Acting Administrator Andy Slavitt. “The Advancing Care Information category will modernize, streamline and replace the Medicare EHR Incentive Program for eligible professionals (also known as ‘Meaningful Use’),” states an ONC fact sheet on the Quality Payment Program and HIT.
According to Slavitt, the focus is on rewarding high-value, patient-centered care, which will be reflected in $1 billion in payments to providers in 2017 for better quality healthcare. However, at the same time, he lamented the fact that technology doesn’t yet support physicians in the way that they need. “For physicians to be successful in value-based care, it requires technology that’s easier to use, simpler and more connected, less burdensome and intrusive, that supports the complexity of medicine but also simplifies by delivering exactly what patients and doctors need when they need it—and no more,” Slavitt said.

Rate Increases for Health Plans Pose Serious Test for Obama’s Signature Law
Finalized rates for big health insurance plans around the country show the magnitude of the challenge facing the Obama administration as it seeks to stabilize the insurance market under the Affordable Care Act in its remaining weeks in office. Market leaders that are continuing to sell coverage through HealthCare.gov or a state equivalent have been granted average premium increases of 30% or more in Alabama, Delaware, Hawaii, Kansas, Mississippi, and Texas, according to information published by state regulators and on a federal site designed to highlight rate increases of 10% or more.
Most of the 10 million people who currently get coverage through an insurance exchange such as HealthCare.gov don’t pay the full premiums because they receive subsidies from the federal government that are pegged to insurance prices in their area. As many as nine million people currently buy individual coverage without using the site, but at similar prices, and most of them aren’t eligible for subsidies.

I would take a biopsy or look for some area of pressure (a lump in the shoe) under the lesion you are referring to as IPKs that are pressure and friction related. Many non-weight-bearing “poromatous” lesions are porokeratoses or poromas that would be revealed on biopsy.
Dr. Bakotic and I published an article on this subject that may be of interest. Email me for a copy if you cannot locate it. Dermoscopy may also help in your clinical appraisal in these cases.

I just returned from The DermFoot seminar in a Los Angeles and can say that I was incredibly impressed with the way it was organized and the caliber of the speakers. To many of our colleagues, this was the first opportunity in this profession to learn how to use injectable fillers on a cadaver foot. Based on this information provided and the hands-on learning, this will likely change many attendees' practices and incomes. Although I have been doing some of these techniques for years, I learned some new and valuable treatment options that I will implement in my practice. I think that every podiatrist should get this experience and to me, it was well worth 10 times the price I paid for attending.

Dr. Andersen's method of soaking feet in concentrated tea solutions will control excessive sweating. The tannic acid in the solution controls the hyperhydrosis. Many patients, however, find making such a brew time-consuming. They are often not happy with the discoloration caused by soaking in concentrated solutions of some teas. In our practice, we have been highly successful in treating both hyperhydrosis and bromhydrosis with Blaine Labs' tannic acid pills.
Two tablets can be dissolved in a couple of pints of water, and if the patients soak daily for three days for 30 minutes each time, the problem will be gone. The only note of caution is that since these pills are packaged in the same way oral supplements are packaged, in addition to the written caution on the bottle, we tell each patient not to store the medication near any of their oral meds, and we reinforce the warning that this is not to be taken orally.
