PODIATRISTS IN THE COMMUNITY
Retired PA Podiatrist Had Considered Becoming a Musician

At one time, he thought he would be a musician; he also considered dentistry. But in the end, inspired by his uncle, Dr. George Helfand, Dr. Arthur (Art) E. Helfand decided to go into what he calls the “family business:” podiatry. Between him and his late uncle, there have been members of the family in the profession for nearly 100 years. An inductee to the PM Podiatry Hall of Fame, Helfand is renowned for his pioneering contributions to “podogeriatrics:” clinical care, teaching, and writing focused on older podiatry patients.

Dr. Arthur Helfand

Since retiring, he has been able to dedicate time to musical pursuits, among them playing songs from the ‘30s, ‘40s and Broadway musicals on the keyboard. In reflecting on a long and successful career, Helfand says his ability to make an impact through his work in public health and senior foot care has been immensely fulfilling, adding “I’ve been a very fortunate guy.”

E-HEALTH NEWS
Significant Meaningful Use Stage 2 Time Crunch Looms

Providers will face significant time pressures as they attempt to attest to Stage 2 of the meaningful use program. Some relaxation of requirements will help, but  eligible professionals who haven’t made sufficient efforts to prepare for Stage 2 attestation will be pressed for time, said CIO policy specialists at the CHIME15 Fall CIO Forum.

For an EHR reporting period in 2015, all Medicare providers must attest between Jan. 4, 2016 and Feb. 29, 2016. While the reporting period has been shortened to 90 days, providers who haven’t prepared will have trouble gathering information for the needed reporting requirements because the final quarter for the calendar year started on October 1.

ICD-10 UPDATE
ICD-10: So Far, So Good

Two revenue cycle management vendors who also operate claims clearinghouses say that the first two weeks of using the ICD-10 codes has been surprisingly smooth. Remittance advice will start flowing from insurers to providers with increasing frequency soon; it’s too early to say now if denials will soar.

But the initial news is good, says Ken Bradley, vice president of strategic planning and regulatory compliance at Navicure. “The initial events seem to indicate it was not a catastrophic event.” When adequate amounts of remittance advice start coming, “hopefully, data will tell us that payment will be made as expected and it will be correct,” he adds.

PRACTICE MANAGEMENT TIP OF OF THE DAY

11 Things to Know about the Meaningful Use Final Rule - Part 7

Interoperability is on the Menu

In Stage 3, more than 60 percent of the proposed measures from which providers can choose require interoperability – that’s up from 33 percent of the measures now in Stage 2. That will add additional impetus to industry and government efforts to increase interoperability within the industry, which should help achieve multiple goals. Still, HL7’s emerging FHIR interoperability standard for exchanging healthcare information electronically remains in draft form and won’t be finalized until 2017, at the earliest.

CODINGLINE CORNER
Query: ICD-10 Listed on Lab Order Form

What ICD-10 code would I use on the blood test request form to check liver function for a patient currently on Lamisil?

Steven Fessel, DPM, Nanuet, NY

Response: When ordering the lab test, you are ordering it because you are prescribing a drug for long-term use for an infection.

1) The infection = B35.1 (onychomycosis);
2) The lab is being ordered because you are having the patient use a drug that could have potential problems used for a long term, ICD-10 Z79.899 (other long term (current) drug therapy);  or
3) ICD-10 Z51.81 (encounter for therapeutic drug level monitoring).

Categories Z40-Z53 are intended for use to indicate a reason for care. They may be used for patients who have already been treated for a disease or injury, but who are receiving aftercare or prophylactic care, or care to consolidate the treatment, or to deal with a residual state.

RESPONSES/COMMENTS (CLINICAL)
RE: Pigmented Lesions on Soles
From: Joseph Borreggine, DPM

Recently, Dr. Brad Bakotic from Bako Pathology Lab gave a lecture at the 100th Annual Illinois Podiatric Medical Association Seminar last week on this particular lesion: Acral lentiginous melanoma. Click here for a description from Wikipedia which explains the finding that you have presented.

I highly recommend three punch biopsies to properly identify this lesion. Acral lentiginous melanoma is a kind of lentiginous skin melanoma. Melanoma is a potentially serious skin cancer that arises from pigment cells (melanocytes). Although acral lentiginous melanoma is rare in Caucasians and people with lighter skin types, it is the most common subtype in people with darker skins. Acral lentiginous melanoma is observed on the palms, soles, under the nails and in the oral mucosa. It occurs on non-hair-bearing surfaces of the body, which may or may not be exposed to sunlight. It is also found on mucous membranes. It is the most common form of melanoma diagnosed among Asian and Black ethnic groups. The average age at diagnosis is between sixty and seventy years.

RESPONSES/COMMENTS (NEWS STORIES)
From: Jeffrey Kass, DPM

I do not know Jack Gorman, DPM. I only know what I am reading on PM News regarding the Jury Verdict. From what is presented here, I say "shame on you, Dr. Gorman."

I find it irritating when I read cases of podiatrists who will throw their colleagues under the bus for a few bucks. I cannot conjure up any sense of "negligence" in this case as per the facts presented. If anyone here reading can, please enlighten me.

YOU CAN'T MAKE THESE THINGS UP

RE: Outrageous Feet of the Day

World's largest feet (Photo: Carlos Garcia Rawlins)

Jeison Orlando Rodriguez Hernandez, a 20-year-old man from Venezuela has broken the Guinness world record for the largest feet. His right foot measures 40.1cm (1ft 3.79in), and his left half a centimeter less. He takes U.S. size 26 shoes. Even though Hernandez is shorter, his feet beat those of the world’s tallest man, Sultan Kosen, whose shoe size is U.S. 24.
