Bunions are Common Complaint From High Heel Wearers: Aussie Podiatrist
Hobart podiatrist Chris Angel says extremely high heels pose short and long-term dangers. While teetering in the fashion statements, women have a greater risk of falling and are assured of falling from a greater height. And, in the longer term, women risk serious injury -- including skeletal problems and foot deformity.
|
|
Chris Angel |
Mr. Angel, who is the principal podiatrist at Ispahan Podiatry, said one of the most common complaints was bunions. The condition was not only unsightly, but also painful. Mr. Angel said the strain on the back caused by teetering in high heels also caused musculoskeletal problems, ranging from tight calves to Achilles tendinitis.

Unhappy Patient Arrested After Threatening NJ Podiatrist
A patient unhappy with previous care allegedly threatened to shoot staff at a podiatry office and to harm a male doctor's private parts, town police said today. Police were called on Oct. 10 at noon to Podiatric Surgical Associates with a report of threatening phone calls.
Several calls were apparently made, police said, by Jared McHugh, 34, of Somerville. The message threatening the male doctor was left as a voice mail message, said police. McHugh was charged with making terroristic threats and harassment following an investigation by police. He was taken to the county jail, with bail set at $15,000.

Query: Sulfa Allergies
Given the small quantity of sulfa in drugs like naproxen and Celebrex, how common is it for a patient with a sulfa allergy to adversely respond to these medications?

RE: Achilles Insertional Pain (Simon Young, DPM)
From: Michael M. Rosenblatt, DPM
DPMs can often get into a scenario of strictly podiatric thinking. It's not surprising, but when this happens, it can lead to missing out on important co-findings. Many of these are general medical in their impact. DPMs are often reluctant to speak with our patients about their alcohol intake. And yet this is a serious issue, which has an entire range of effects, including delayed healing and the inability for antibiotics to work properly.
I often found that despite my best efforts at cortisone injections and orthotics, it was impossible to treat heel pain successfully if the patient was hyper-uricemic. Sometimes, these patients also had highly elevated cholesterol as well. This led to my taking necessary blood tests of most of my patients who had heel pain. With Obamacare, we will be seeing more and more patients who will qualify for "regular" insurance. Many of these are self-destructive in their personal habits. Some of them will disclose that, others not. I am publishing an article on dealing with self-destructive patients in Podiatry Management. If this patient is an alcoholic, he will find it very difficult to get a satisfactory result. This will be compounded and confounded if a surgery is anticipated.

| RESPONSES/COMMENTS (MEDICAL/LEGAL ) | |
From: Elliot Udell, DPM
When Dr. Sullivan complained about the recommended tests for peripheral neuropathy being 3,600 dollars, my initial reaction was to dismiss his concern, writing that if a patient needs the tests, we had better order them and not worry about the patient's pocketbook. My apologies for being a bit cavalier in dismissing Dr. Sullivan's concerns. In this week's New England Journal of Medicine, Dr. Cheryl Bettigole shares Dr. Sullivan's concerns. She wrote an editorial titled "The Thousand Dollar Pap Smear" where she showed how a test that used to cost 30 dollars is now being billed out by some labs for a thousand dollars.
The author expressed her concerns about the impact of rising laboratory tests on the finances of individuals as wells as on our healthcare system. Dr. Bettigole wrote, "...it is increasingly clear that physicians have an obligation to be good stewards of limited resources to understand the financial effects that the orders we write have on our patients."

RE: ICD-10
From: Arden Smith, DPM
The more I read and research regarding ICD-10, the more I am convinced that ICD-10 is unworkable in anything other than a mega practice setting that has professional coders on staff. It appears to me that the loss of time and thus productivity involved with coding with this new system, along with all of the other strains on a practice in our current environment, could be catastrophic to the viability of a practice.
I would appreciate if some of my colleagues with more knowledge of ICD-10 than I have could speak on how to survive and function with this new system. How did “organized political medicine” ever let this come to pass. It just seems like another nail in the coffin of private practice medicine.

RE: The Passing of Ed Stein
I was saddened to read about the passing of Ed Stein. Ed was one-of-a-kind, the ultimate salesman, and a very nice guy. Starting in the early 1970s, we spent much time together as exhibitors and became friends. When I began exhibiting in the Mid-West, I did not know anyone. I was lucky to have my first exhibit space next to Ed. After he spoke to the podiatrists in his booth, he would bring them over to my booth and introduce them.
When I became discouraged after a number of years exhibiting in the Mid-West with little results, he would encourage me to stick with it and that it would work out. He was right. I will always have a special place in my heart for Ed and will never forget what he did for me. My love and best wishes to Joan and her family.

RE: The Passing of Ed Stein
Ed Stein is a loss to the profession, but a bigger loss to his wife Joan and his children. Ed always had a smile and a crazy joke to tell, but most important was the direction and recommendations he made to many podiatrists and to all the professional ball teams he sold to and consulted with. He made padding the foot a true art! He was a dear friend and will be greatly missed. My love goes out to his family.
Steve Berlin, DPM, Baltimore, MD
Ed Stein and his lovely wife were the neighbors I had the pleasure to meet at the first three podiatry conferences that I attended. I was "green", he gave me unimaginable encouragement. He was known to many as an icon and an idol. He was a great man. The entire Bianco Family send their sincere condolences to Joan and his family.
