PODIATRISTS AND HUMANITARIAN EFFORTS
Operation Footprint Celebrates 44th Year of Operation with a Surgical Mission in India
 
Operation Footprint (formerly the Baja Project for Crippled Children) completed a surgical mission in Agra, India from September 6-10, 2022, its second at this location. This represents 44 years of ongoing humanitarian activity since its inception, interrupted only by the Covid pandemic. The group will resume activity in India, and in Honduras in January 2023. Operation Footprint has performed over 3,000 major surgical procedures in Mexico, El Salvador, Honduras, and India, has evaluated over 30,000 patients, and has trained over 200 residents.

The recent Agra mission was aided by a $62,000 grant from Rotary International to enhance the surgical suite at Agra’s Center for Trauma and Joint Replacement. In addition to Marc Benard, DPM, the surgical team included co-directors Drs. Bruce Werber, Bhavesh Shah, Matt Roberts and Jeff Yung, and faculty member Dr. Neeta Hasmukh. Twenty-four major reconstructions were performed, including for neglected congenital clubfoot and neurologic cavovarus and equinus deformities. In the past 12 months, Operation Footprint has also rolled out its on-line CFAR (Complex Foot and Ankle Reconstruction) lecture series, with topics presented by both its own directors as well as nationally recognized guest lecturers and expert panelists.

Donation to BUSPM Boosts Biomechanics and Gait Studies
 
A grateful patient of Joseph C D’Amico,  DPM, DSc has at his direction donated substantial funds to support Barry University School of Podiatric Medicine’s (BUSPM) work in biomechanics. Dr. D’Amico is a guest lecturer in pediatrics and sports medicine at the college.

Specifically, this gift will help sponsor the Richard O. Schuster, DPM Memorial Biomechanics seminar which for the first time will be held this year in Florida at BUSPM on November 19th and 20th. MGH Podiatry Research Students Have Study Accepted for ACFAS Poster Presentation
 
Timothy Cheung (SCPM), Khanh Phuong Sieu Tong (SCPM), Kayla Obradovic (NYCPM), and Shanay Fischer (NYCPM), podiatric student researchers with Massachusetts General Hospital Podiatry, have had their research project titled, “Does the First Intermetatarsal Angle Reduce in First Metatarsophalangeal Implant Arthroplasties?” accepted for the poster presentation at ACFAS 2023. The students conceived and completed their project on their own with little advisory input from the attendings.

Under the direction of Norman A. Wortzman, DPM, and with the assistance of Adam Landsman DPM, PhD, MGH Podiatry initiated a research arm five years ago. Part of the MGH mission is to encourage students to perform research as an integral part of their careers. The MGH student research team currently consists of seven podiatric students.

PPMA Educates Walkers on the Importance of Podiatry at The ADA’s Step Out Walk 
 
On Saturday, October 8th, the Pennsylvania Podiatric Medical Association (PPMA) representatives volunteered their time to spread awareness to participants at The Annual Step Out Walk benefitting The American Diabetes Association. Led by PPMA Past President Sabrina Minhas, DPM and the Temple University School of Podiatric Medicine (TUSPM), PPMA provided literature to participants at the walk to raise awareness of the importance of podiatric care for patients with diabetes.

Hundreds of individuals in the Philadelphia area registered for the walk and were greeted with beautiful fall weather at The Philadelphia Naval Shipyard. These participants learned the importance of engaging a podiatrist to be a part of a diabetic patient’s healthcare team.

1. Rest/Paid Time Off
 
Rest is vital to the longevity of physicians and advanced practice providers (APP). Rest does not just include 7-8 hours of sleep per day but also taking mental rest, spiritual rest, and social and emotional rest. It is crucial that organizations coordinate their efforts to encourage staff to take the rest they need to rejuvenate to perform at their best in support of their patients. This might include taking days off, half-days, vacation time, and various types of mental and physical exercise programs.

RE: The Passing of Thomas J. Cianciolo Jr., DPM
 
Dr. Thomas “Dr. Tom” J. Cianciolo Jr., 90, of Clarence, a well-respected podiatrist for over 50 years who served patients across Western New York, passed away Saturday, Oct. 1, 2022. He served in the U.S. Army from 1953 to 1955 as a sergeant in the counterintelligence corps.

Dr. Cianciolo graduated from the New York College of Podiatric Medicine in 1962. Dr. Tom was a Fellow of the American College of Foot Surgeons and was affiliated with Sheridan Park Hospital and Sheehan Memorial Hospital. In 2001, he closed both offices and worked part-time, attending patients at nursing homes.

Query: Punch Biopsy of the Skin
 
What is the CPT code for a punch biopsy of an unknown, skin lesion at the foot or ankle? What ICD 10 diagnosis codes might be appropriate for this CPT code?

Response: CPT 11104 Punch biopsy of skin (including simple closure, when performed); single lesion
CPT 11105 Punch biopsy of skin (including simple closure, when performed); each separate/additional lesion (List separately in addition to code for primary procedure)
ICD -10-CM diagnosis – D49.2- Neoplasm of unspecified behavior of bone, soft tissue, and skin
 
The reason to use D49.2 is that you are doing a biopsy on abnormal tissue BUT do not know the etiology yet. This code is there to say it could be skin, soft tissue, or bone. It covers everything you might biopsy. Using the benign neoplasm, in my opinion, is wrong. The reason it is inappropriate on biopsy day is that at the time of biopsy you do not know if it is benign. Therefore, based on ICD-10-CM guidelines, it is inappropriate to apply a diagnosis when you don’t know that diagnosis yet. However, if you wait on the pathology and it is a benign neoplasm, then it is appropriate to bill this a week or so later, holding the claim with the definitive diagnosis.

Time Out of Office= Approximately 3.25 hours ( 30 min travel + pre-op- 2.25 hours for procedure- 30 min post-op and travel back to office).
 
Hourly rate comes to $1035.17 / 3.25= $318.51 (Medicare) $398.14 (private Insurance)"
 
Dr. Marino left out of the hourly rate: the number of post-operative visits (no reimbursement) and the cost of your staff to schedule and pre-certify the procedures. With those items, your hourly rate is much less than $318.51/hr. You could also add in the cost of an appeal when the insurance only pays for the bunionectomy and not the other procedures, stating that the other procedures are included in the fee paid for the bunionectomy.
