Ankle Sprains Most Underdiagnosed Injury Seen in OR: DC Podiatrist

“I have always believed that ankle sprains are the most undertreated injury seen in the emergency room,” says Sheldon Laps, DPM, a DC podiatrist. “X-rays are usually taken, and if they are negative, patients are usually sent home with crutches and advised to ice the area and stay off it until they are able to walk on the foot.”

Dr. Sheldon Laps

Dr. Laps tells his patients to avoid exercise until they can walk briskly and pain-free on the ankle. He also suggests wrapping the ankle before exercising, and stretching before and after. After exercise, ice the ankle for five minutes if it still tends to swell after a workout. Runners especially should keep their mileage and intensity low when they start back, and run alternate days on a flat surface, such as an outdoor track, before resuming hard training or serious competition, he says.

Don't Try to Remove Peeling Skin From Your Feet: CT Podiatrist

The seasons can play a role in roughing up your feet, with summer typically leading to peeling and blistering and winter to dryness, cracked heels, and peeling. So when your dogs start barking over the scaly skin situation going on down there, it can be super tempting to take matters into your own hands by picking. Don't, per Jennifer Tauber, DPM, a podiatrist in New Canaan, Connecticut.

Dr. Jennifer Tauber

"The biggest mistake that people make is trying to remove the peeling skin on their own, either by using some type of instrument, which can lead to bleeding and infection, or an over the counter remedy that may be too harsh for the skin and cause more irritation than good," says Dr. Tauber.

PODIATRISTS AND SPORTS MEDICINE
Don’t Ignore Heel Pain in Sports: CT Podiatrist

According to Robert Weiss, DPM, "A large majority of the athletes we are evaluating are complaining of heel pain. If one part of the body is injured (especially the foot), the other parts compensate for it. This, in turn, may set off a chain reaction of additional injuries. The powerful heel cord-tendon attaches to the back of the heel from the calf. Quick starts that are required in so many sports, along with lack of conditioning, often cause injury."

Dr. Robert Weiss

"In younger athletes, the heel growth plate gets injured and often this can continue into adulthood. In adolescent athletes (and some adults) the problem occurs in the arch muscles and tendons—otherwise known as “plantar fasciitis.” One of the causative factors is the cleated athletic shoes. The body also needs dynamic muscle balance and good biomechanical foot control to limit overuse injuries. Your body will become less able to protect itself once injury symptoms appear," says Dr. Weiss.

INTERNATIONAL PODIATRISTS IN THE NEWS
Bahamas Podiatrist Discusses Edema of the Feet and Ankles

According to podiatrist Monique Mitchell, "Swollen ankles and feet are common complaints and are not usually cause for major concern, particularly if you have been standing or walking a lot. But feet and ankles that stay swollen or are accompanied by other symptoms could be a sign of serious health problems. Here are some possible causes of foot and ankle swelling, what to do about it, and when to see the doctor or podiatrist:"

Dr. Monique Mitchell

"Pregnancy complications, foot or ankle injuries, lymphedema, venous insufficiency, blood clots, infections, heart, liver, or kidney diseases, or medication side-effects, including hormones such as estrogen, calcium channel blockers,  steroids, antidepressants, NSAIDs, and diabetes medications," says Mitchell.

PRACTICE MANAGEMENT TIP OF THE DAY
Top Tips for Succession Planning - Part 6

Positioning yourself and your practice for a smooth transition takes time and thoughtful planning.

6. Set the records straight
  • An often-overlooked part of succession planning is compliance with applicable medical records laws.
  • Physicians are deemed to have legal custody of a patient's medical records, and must comply with certain laws when selling a practice.
  • Transferring custody of those records, informing patients of the transfer of custody and their related rights, executing a medical records custodian agreement to satisfy state and federal laws, and notifying the state of the transfer should all be part of a comprehensive succession plan.
  • This process is also a good time to determine whether you have any medical records that can be purged.

MEDICAL-LEGAL TIP OF THE DAY
Presenting Your Medicare Case to the ALJ - Part 17

44. What should you do with your expert before the hearing? Write out all the questions you want to ask your expert in front of the ALJ. Then write out all the answers to those questions. Review all the questions and answers with your expert before the hearing.

45. Why is it a bad idea to ask your expert any new question during the hearing? You may not like the answer. Most healthcare attorneys will tell you they never ask their expert a question during an ALJ hearing if they don’t already know the answer to the question.

RESPONSES/COMMENTS (MEDICAL-LEGAL)
RE: Compliance by Podiatric Physicians with the ADA
From: Leonard A. Levy, DPM, MPH

Nearly 40 million Americans are individuals with disabilities.(1) Since podiatric medical practices are likely to include patients with disabilities, it is important that they are prepared to treat such individuals. Not only do such patients often experience poorer outcomes, but the podiatric physician may be unprepared or uncomfortable treating those with disabilities or perhaps even overwhelmed at the often-complex regulations surrounding their care (or perhaps even unaware that such regulations exist).

The Americans with Disabilities Act (ADA) is a comprehensive federal civil rights law designed to ensure equal access and protection for individuals with disabilities. Under the Act, medical practices are required to ensure that...

RE: Outrageous Shoe of the Day

Feel the Roman "IV" on the heel.

The Ancient Romans were best known for their lasting construction and design. The back heel represents the ‘I’ and the front the formations of the ‘V’ for the conquering warrior wearer.
