PA Podiatrist Uses Sensor Technology to Measure Pressure Points in Tennis Shoes

A tennis coach is using sensor technology embedded inside tennis shoes and corresponding video analysis of a player's movements on court to gain a better understanding of how the feet perform during play. Dr. Allan Grossman, a USPTA coach and podiatrist specializing in treating tennis players at the Harrisburg Foot and Ankle Center in Harrisburg, PA, embeds a sheet fitted with thousands of sensors that pinpoint pressure points inside tennis shoes. The battery-powered device connects to a cuff wrapped around the athlete's ankle to measure pressures on the feet during activity and transmit the data, using Wi-Fi, to a laptop computer, where both doctor and athlete can see exactly where pressure points arise during on-court movement.

His findings have been surprising. "Conventional thinking is that when tennis players load on the back leg to hit a forehand or plant the foot to change direction that their weight puts pressure on the outside of the foot," Grossman says. "What we learned through thousands of scans of players of all different levels is that, in virtually every case, the most pressure is applied on the inside of the foot. That's quite surprising — it's the opposite of conventional thinking."

Five Barry Students Awarded Steinberg Scholarships

Five students from Barry University’s School of Podiatric Medicine have been awarded a Dr. Marvin and Ruth Steinberg Memorial Scholarship for the 2011-12 academic year. Sarah Haller, Sofie Pinney, Jessica Ortega, Jurgen Helmut Fernandez, and Armando Gonzalez will each receive a $1,000 scholarship to be used toward their tuition. All students were selected based upon their leadership qualities and academic merit.

Haller, class of 2013, serves as current president of Florida Podiatric Medical Student Association (FPMSA). She was recently awarded the Dr. Harvey Kopelman Award for leadership and dedication to the profession. Pinney, a former FPMSA president and a member of the class of 2012 has also served as chair of the 20th Annual Agnes Seminar. Ortega holds the position of FPMA liaison for community service and is the 2011 winner of the Dr. Patricia Kopenhaver Scholarship. Helmut Fernandez is the current president of the Hispanic Podiatric Medical Students Association (HPMSA). Gonzalez is the president of the class of 2014 and serves as the historian for the HPMSA.

Aussie Podiatrists Hit Airwaves to Promote Diabetes Awareness

A new series of advertisements are urging diabetics to get regular podiatry checks to reduce amputations. The ads, run by the Australasian Podiatry Council, feature three patients who believe their foot was saved, or would have been saved, by seeing a podiatrist. They will be broadcast on commercial television and community radio this month as part of foot health month. It is also World Diabetes Day on November 14.

Alfred Health podiatry program manager Liz Dalton, who is based at Caulfield Hospital, said toe and leg amputations were quite common in diabetics and could be avoided. Diabetics should check their feet daily for areas of redness, blisters, or discoloration, she said. “They should be looking for anything that wasn’t there yesterday,” Ms. Dalton said. “If you’re unaware of what can happen and what to look for, you can end up with ulcerations which may be slow to heal and ultimately lead to amputation.”

FL Podiatrist Receives Community Service Award

At the recent OPAL (Oustanding People and Leaders) Awards, the community service award went to Dr. Dennis Frisch, who has been practicing podiatry in Boca Raton since 1983. He is board certified in foot surgery by the American Board of Podiatric Surgery and is a trustee of the American Board of Podiatric Medicine. Dr. Frisch received his Bachelor of Science degree from the University of Florida and his Medical degree from Dr. William Scholl College of Podiatric Medicine in Chicago. He is active in the community, serving on the Board of Directors of The Rotary Club of Boca Raton where he has been a member since 1984 and served two terms as president — in 1990 and 2005.

Frisch was appointed to Boca Raton Parks & Recreation Board in 2007 and elected commissioner of the Greater Boca Raton Beach and Parks District in 2008  He has been involved with youth sports leagues including Boca Jets Youth Football on the Board of Directors, Boca Raton High School Football Booster Club and the Boca Raton Rugby Football Club, as well as being an avid rugby player. “Volunteering actually focuses my life better,” he said.

Podiatrists Slated for Medical Staff President Eligibility

The Centers for Medicare and Medicaid Services (CMS) has unveiled a proposed rule that would amend the hospital Conditions of Participation (CoP) to allow podiatric physicians to serve as president of the medical staff in hospitals in states where it is not otherwise precluded by law. APMA and its members have advocated for this change for many years.

The proposed rule, titled Medicare and Medicaid Programs; Reform of Hospital and Critical Access Hospital (CMS-3244-P) reflects CMS' commitment to the general principles of the President's Executive Order 13563, released January 18, 2011, entitled "Improving Regulation and Regulatory Review." CMS acknowledges that in many states, "the laws underscore the widely held conclusion that the education, training, and experience of podiatric physicians are similar to that of their allopathic and osteopathic colleagues with respect to serving in such a hospital leadership position.

Reconstructive Foot and Ankle Surgery Fellowships (Two Available)

Each of these two fellowships is a one-year experience during which the Fellow will develop a rational approach to revisional and reconstructive foot and ankle surgery.  The Fellow will function as a clinical instructor and participate in the teaching of Podiatric medical students and residents, and have ample opportunity to be involved in clinical and basic research projects.  The fellowship will provide the candidate, further expertise in Charcot reconstruction, soft tissue coverage, trauma and deformity correction.  The fellow is expected to complete two clinical or basic research projects during the year.

Duration:  1 year (July 1, 2012 - June 30, 2013) Stipend: $44,100 Application DeadlineNovember 21, 2011  Interviews:10/1/2011 – 12/12/2011 

Requirements:  Completion of 3 year residency; a commitment to a part-time/full-time academic career in Podiatric Medicine and Surgery. Texas License eligible (Test date April 16, 2012 with an application deadline of February 16, 2012).  ABPS Qualification eligible in Foot & Rearfoot / Ankle Surgery (Test dates TBD).

Ulcers on both sides of an ankle are rare in my experience and probably need 2X/ week attention or at least weekly care. I would set 4-6 weeks as a goal for reducing the wounds at least 25%.

I would make sure to eliminate pressure under the areas such as boots or tight socks, which press on the wounds or are in a position which presses his legs to a bed, foot stool, or to the other leg.

The TightRope is not a new idea. See the old Campbell's ortho text. Surgeons have been trying to tie mets together for a long time. In my opinion, it didn’t work then and it still doesn’t work over the long run. Regarding the dispersion of pressure using a plate (Arthrex) -- one of the few excuses that is espoused for using a TightRope in a bunion procedure is to avoid morbidity of excessive bone surgery and soft tissue dissection in an older population. I don’t know about this specific plate because I have never used it, but  common sense tells me that it would take a quite significant dissection to get one on. Someone convince me differently.

Surgical excision has a great propensity for reoccurrence within a year. I suggest using Verapamil gel 15% -3 or 4 times daily. Although this is not conclusive, it is a treatment. I suggest doing excision, then follow-up with the Verapamil gel. You can have it compounded for a substantial cost decrease over one of the national known companies promoting this treatment.

A clinician can ultimately decide which of the various treatments he wants to use but don't assume the "lump" is a fibroma. Make sure to biopsy the lesion prior to these treatments. If the lesion is in reality a sarcoma and one is jabbing the lesion a bunch of times, this could turn into a nightmare.

RE: Stay Away from Trimming Toenails

There have been some letters in podiatry news about the follies, scandal, and bugaboo going around about podiatrist billings for routine foot care, and in particular, nail trimming. Is there any question why our allopathic colleagues regard us with a certain disdain, and often times, we turn out to be the least wanted on the team? A good warning is this: stay away from trimming toenails. That's not a cockeyed doctrine! The reason is that it makes you look timid, among other things.

Not trimming toenails also keeps you out of trouble. Our basic vision and endeavor with the new order for things to come should be to further the profession and to keep on advancing regardless of whether we have to go over, under, or through our detractors, or even if need to hire them. Fortuitously, there are many unemployed physicians with plenty of vim and vigor who would enjoy working with a podiatrist. We should have only one motto: L'audace, L'audace, toujours L'audace! So be audacious as well as bodacious.

The American College of Foot and Ankle Surgeons (ACFAS) calls attention to its position statement, the “Credentialing and Privileging of Podiatric Foot and Ankle Surgeons and Guidelines for Surgical Delineation of Privileges” document, which provides a current overview of what hospital Credentialing and Privileging Committees are looking for in granting privileges for surgeons.
