“Bunions aren’t as common in younger people, but they are usually genetic, and usually when they start to develop in younger people, they’re just not as painful and bothersome until you get older,” said Brian Dix, DPM, a podiatrist and orthopedic surgeon with Avera Orthopedics.

“What’s unique now, though, is we used to have to make all those cuts by hand. So now there are jigs and instruments out there which we can put onto the foot and help get the correction that we need. But it’s reproducible every time to get the correction to correct the second and third metatarsals and then to correct the bunion,” Said Dr. Dix.

It is hopefully the beginning of setting up the ability for early testing, early intervention, and public education programs. It was made clear by the DOH that podiatry here is a vital part of the team, with primary care and vascular specialists, as an approach for patients against these conditions.

Ultrasound Workshop Highlights AAPSM Meeting
The America Academy of Podiatric Sports Medicine (AAPSM) had several hands-on workshops at their stand-alone meeting this past weekend in Washington, DC. Diagnostic ultrasound was presented by Garry Ho, MD and Joel Zarzuella, DPM.

Dr. Ho highlighted the value of locating the anatomy in the sagittal plane on the foot for orientation before transitioning to the transverse plane. Also highlighted was the honeycomb-like appearance of small nerves in the foot, such as the medial plantar nerve, and the value of ultrasound for hydrodissection of nerves.

3) Analyze insurance denials and develop processes to reduce them.
Is there misuse of modifiers? Are denials related to medical necessity? Outdated codes? If you make the effort to track the denials and see where you’re making mistakes, the time spent will be rewarded with higher and prompter payments. Similarly, have processes in place for correcting and refiling denied claims in a timely manner.

Query: Direct Contracting Entity
If a podiatrist sees a patient who has Medicare as primary insurance and also has a direct contracting entity (DCE) that is primary, are there any problems getting paid? Who do you bill?

Response: DCEs typically take precedent as they have taken over the contracts for the “primary.” It can be confusing but frequently, a DCE will negotiate a lesser cost to the subscriber and take a small “percentage of the pie” for the management. I would reach out to the DCE and to your Medicare representative and verify who has primary contract rights so you can be paid fairly.

RE: Thoughts on Single Board Certification and Added Qualifications

In his Inferno, Dante says the fourth circle of hell is greed. Greed is worse than gluttony, the third circle, but better than anger, the fifth circle. Greed is characterized by never having enough and we are constantly in search of more. It doesn’t seem wrong to bring home as large a salary as possible for a family. We live in a society that rewards hard work. Maybe the epitaph, “He worked himself to death” is not so bad since it supposedly reflects admirable unselfishness.
A workaholic is a functional member of society, so we think. But are we practicing beneficence and non-maleficence toward ourselves? Maybe Charlie Brown said it best, “All I want is my fair share.” I begin to worry if there are not enough surgical cases for me because a non-certified podiatrist is suddenly the same as a real board certified surgeon because he or she somehow has an added qualification. Am I interested in protecting the public or am I interested in protecting my...

I am so profoundly sorry to hear of the passing of Dr. Gary Bauer. I had the pleasure of working as a Lab Instructor at TUSPM for the Cadaver Surgery Course directed by him. Dr. Bauer was the consummate master of anatomy who was always willing to unselfishly share his knowledge, experience, and time for any student. He was also an avid sailor and I will always cherish the fond memories of our families spending time together boating on the Chesapeake Bay. I will keep him, Maggie, and his children in my thoughts and prayers. Please accept my sincere condolences.

I met Gary when we were co-residents at Harrison Community Hospital. I was from NY and Gary a native Michigander; and he made me feel like family. He was soft spoken and a gentle soul. He was also an academic and scholar and we often drove down to Wayne State Medical School to read journals.
We had many adventures together such as the time he built a sailboat. The first time we put it in water, he was reading the manual on how to sail while he was at the tiller! One time, several of the residents decided to go cross country skiing. Gary had the car keys but lost them somewhere on the trail, and did not realize that until it got dark and it was time to go home. We had to return the next morning at dawn to hunt for the keys. We found them in the parking lot!
He was too young to pass and he will be greatly missed.
